Operace vestibulárního schwannomu vede u většiny pacientů ke vzniku jednostranné akutní periferní či kombinované vestibulární léze v důsledku chirurgického přerušení obou větví vestibulárního nervu. Pooperačně se stav projevuje poruchami posturální stability, závratí, oscilopsií a někdy i vegetativním doprovodem. Trvání vestibulárních symptomů je závislé především na centrální kompenzaci, při které hraje dominantní roli mozeček. Kompenzace je ovlivněna různými faktory, ty jsou dané stavem pacienta a velikostí tumoru, vlastní funkčností vestibulárního systému, a dále jsou to faktory navozené terapeutem. Terapeutickou snahou je rychlé navození kompenzačních mechanismů a zkrácení doby kompenzace. Cílem práce bylo podrobné prostudování vlivů ovlivňujících kompenzaci a aktivní ovlivnění procesu vestibulární kompenzace jako takové. Mezi základní používané postupy patří časně zahájená řízená vestibulární rehabilitace, která u většiny pacientů vede k rychlejší rekonvalescenci. I při maximální péči se u některých pacientů nedaří dosáhnout plné kompenzace, což vede k trvalému funkčnímu deficitu projevujícím se nejčastěji posturální instabilitou. Prehabituace je možný způsob vedoucí k urychlení kompenzace. Při chemické prehabituaci je aplikováno ototoxické antibiotikum intratympanálně, což vede k funkčnímu... Vestibular schwannoma surgery causes in majority of patients the unilateral peripheral or combined vestibular lesion due to surgical interruption of both branches of vestibular nerve. It manifests postoperatively with postural instability, vertigo, oscilopsia and even with vegetative symptoms. Central compensation, in which cerebellum plays dominant role, influences duration of the symptoms. Factors affecting compensation may be divided into several groups (general health status of a patient, the tumor itself, actual state of function of vestibular system, external factors induced by a therapeutist). Therapeutic aim is to induce compensatory mechanisms as soon as possible and to reduce overall duration of the compensation. The aim of this study is to consider predictive factors influencing central vestibular compensation and to influence the vestibular compensation itself. Early operated vestibular rehabilitation is a basic procedure. It leads to faster recovery in majority of patients. Even with maximal effort the full compensation is not always achieved. It results in permanent deficit of vestibular function manifested with postural instability.
