Possibilities of rehabilitation in children with cleft defects

The cleft defects of orofacial complex belong among the most frequent congenital defects. These defects are affecting functions of orofacial complex and then complications of food intake, breathing, phonation and mimics can occur. On that ground the treatment needs complex care. The members of multidisciplinar team are plastic surgeon, orthodontist, paediatrician, geneticist, anaesthetist, anthropologist, phoniatrician, speech therapist and psychologist. The aim of the work is to describe physiotherapeutical techniques, methods or concepts, which can reduce, clear off or prevent disorders of orofacial complex functions since newborns. Currently it can support physiological motory development of children. The assimilation of physiotherapy into care complex could improve therapy for children with cleft defects. Basal stimulation in preterm infants from the perspective of occupational therapist

This thesis is about the parents knowledge about the Basal stimulation concept. In the theoretical part we focus on the preterm infants, the classification and problems associated with immaturity. In following chapters we focus on the Basal stimulation concept and its components. Further we discuss about particular stimulations to use for preterm infants and confirm them with foreign studies. In the end of theoretical part we emphasise the importance of parent education and participation parents in the care of child. In the practical part we focus to achieve the target of this thesis and to confirm or refute the hypotheses. We tried to find out the parents awareness in the Hospital University Motol about the Basal stimulation concept in the care of preterm infants, in application of its components (kangaroo care, orofacial stimulation , mummy position, nest position) and in knowledge about the concept usage
Application of physiotherapy in treating myasthenia gravis

Myasthenia gravis is an autoimmune disease affecting the neuromuscular transmission. It manifests in muscular weakness and tiredness, which characteristically fluctuate and change throughout the time. The most typically affected muscles are extraocular, mimic, oropharyngeal and the muscles of lower and upper extremities. An examination of affected groups of muscles and preserving the function of respiratory muscles are essential. The danger of respiratory difficulties is aggravated by worsening physical condition, which comes as a consequence of muscular weakness and with the onset of the myasthenic crisis. This paper is aimed at using physiotherapy in treating a myasthenic patient. Considering the patient's condition, a respiratory physiotherapy, orofacial stimulation and kinesiotape may be used in therapy of these patients. The case study mentions also sensorimotor stimulation and nordic walking to improve physical fitness. The aim of the therapy is timely prevention of potential complications and their solving. The paper contains a case study of a myasthenic patient.
Effectiveness of co-operation of physiotherapy and speech therapy in school-age children with cerebral palsy

This Bachelor thesis focuses on a co-operation of physiotherapy and speech therapy in school-age children with cerebral palsy (CP). In the first part CP is described as an illness - its forms, origin and impaired communication ability of these children. Next is the description of basic methods used in physiotherapy. In speech therapy I mentioned important factors of working with these children. The next section is dedicated to a practical part of the thesis, which is a quality research formulated by case studies. The research file is comprised of three children suffering CP - two girls and a boy, all three attending the Kaňka citizens' counselling centre. I gathered all the information using an entry kinesiological analysis, aspection, insight into their medical documentation and results given by their speech therapist. The first goal of my work was to describe diagnostic and therapeutic procedures used in physiotherapy and speech therapy in school-age children with CP. The second goal was to describe the differences in effectivity between a speech therapy following a physiotherapeutic care and speech therapy conducted in isolation. Every patient from my research file underwent seven therapies - entry kinesiological analysis, two speech therapy interventions with no previous physiotherapy and four different physiotherapies followed by a speech therapy intervention. The results were assessed based on my observation and a speech therapist evaluation. I myself view these therapies as successful ones, because my work showed that a co-operation of physiotherapy and speech therapy is very important, and it has its positive results. For example, the children's sitting poses were improved during speech therapy, their face expressions were more relaxed, their breathing in speech therapy exercises improved or their phonation time was extended.
Current approaches in the speech therapy for children with Down syndrome

The thesis discusses the specific therapeutic methods and techniques that can be used in the context of speech therapy for children with Down syndrome who suffer from symptomatic speech disorder. The thesis includes the complete terminological definition of Down syndrome etiology, diagnosis and detailed elaboration of the therapeutic methods. It also includes complementary therapies that contribute to the overall development of the child's communication skills. The main objective of the thesis is to analyze the speech therapy. It may be an inspiration for speech therapists, parents, teachers in kindergartens and primary schools who take care of children with Down syndrome and want to contribute to improvement of communication skills of the children. The thesis can be a starting point for further scientific work that deals with similar issues. Speech therapy of children with developmental dysarthry

This thesis "Speech Therapy of Children with Developmental Dysarthria" deals with dysarthria and specific therapeutic methods that can be used in the context of speech therapy for children with this specific type of communication disorder. Part of the thesis includes the complete terminological definition of dysarthria etiology, its classification, its diagnosis and a detailed treatment of therapeutic methods. This thesis also includes supplemental methods, which is important for good child developmental and positively influences child speech. The main objective is to introduce potential therapeutic methods and subsequently analyze the frequency of their use in speech therapy. This thesis can be used as inspiration for speech therapists, who will lead therapy with children who have dysarthria.

Eating Habits of People with Multiple Disabilities

This thesis aims at description of basic eating habits of children with multiple disabilities and mapping methods for acquisition of these habits. The thesis then describes the physiological way of eating as well as individual parts of the involved digestive system. The objective is to describe, on the basis of contemporary domestic and world literature, individual learning strategies of most elementary eating habits, like chewing or swallowing, and compare these theoretical knowledge with my practical experiences acquired by working as a personal assistant for children with multiple disabilities in their home and school environment. Special attention is paid to orofacial stimulation in relation to the acquisition of individual eating habits and the possibility of using the concept of basal stimulation to improve their quality. The paper proceeds with alternative ways of nutrition for the persons with multiple disabilities, respectively percutaneous endoscopic gastroscopy and nasogastric tube. The thesis includes three case studies of children with multiple disabilities documenting the way for increasing the quality of lives of people with multiple disabilities by using treatment methods; special attention is paid on the food intake.
Use of orofacial stimulation in occupational therapy

This bachelor thesis aims to design examination and therapy the occupational therapist in the orofacial area of selected adults with a neurological diagnosis. The bachelor thesis is divided into two parts - theoretical and practical. The theoretical part deals with the anatomy of the orofacial area and related etiology. It outlines the main problems which in this area may occur. Specifically, salivation and swallowing disorders, muscular disorders and communication problems. It also describes the possibility of examination which the occupational therapist has. Finally, this part deals with methods of intervention. The practical part contains the methodology. It contains my proposal for examination and therapy. Examination, which I proposed, is covering various areas of orofacial complex and consists of five parts. Using this examination, we can define the problem with isolated oral movements, identify changes of organ, assess the degree of salivation, swallowing and speech process. The therapeutic unit is composed of the surface and point massage according to the concept of basal stimulation and of exercises for the lips, cheeks and tongue.
