Establishment of Anti-Missile Base (Constitutional Aspects): Study 3.072

Proximal Femoral Fractures - Nurse Problem with Aging of Population
ŠTECÁKOVÁ, Martina
This bachelor thesis deals with the nursing issue of proximal femoral fracture in the context of aging of population. Recently there has been an increase in the number of cases of proximal femoral fracture in the elderly people above all. Age and general state of health influence significantly the entire postoperative period and self- sufficiency of the patient. The objective of the thesis was to research factors related to proximal femoral fracture, to find out about mobility and at the same time about self-sufficiency of the respondents before and after the trauma. Then, to point out the areas of self-care in which the client has the largest deficit, how the client manages the individual areas and whether his own efforts and efforts of the nursing staff can help him to return to the normal life. These objectives were achieved in the thesis. Four hypotheses were set. Hypothesis 1: Proximal femoral fracture in clients above 65 occurs more with women than with men, was verified. Hypothesis 2: Proximal femoral fracture occurs most frequently with clients living alone after the fall at home, was also verified. Hypothesis 3: Clients who are fully mobile and self-sufficient before the trauma, usually return directly to their original environment immediately after the discharge from the trauma department, was not verified. Hypothesis 4: Self-care in hygiene in the postoperative period is the most difficult task for the respondents, was not verified. The theoretical part deals with anatomic structure, vascular and neurological supply of proximal femur. The other part describes particular types of fractures together with clinical picture and treatment possibilities. Then old age with all its aspects is described. An important area is the nursing part, focused on preoperative preparation and postoperative care. To verify the hypotheses, quantitative research was chosen. A standardized interview was designed for patients. An anonymous questionnaire was designed for nursing staff. Further on, content data analysis was applied. The results of the work point to the deficit in the area of self-care, which influence ability of the elderly to return to the normal life. If nursing staff and physiotherapist were familiarized with these results by means of a workshop initiated by the management of the Clinic of Orthopaedics and Traumatology, it might lead to the improvement of the nursing care and it also might become a stimulus for more intensive rehabilitation. Mapped self-care deficits will be then used to create educational material to facilitate the provision of particular services by home care agencies that will compensate for these deficits when the patient returns to his home.

Cooperation between a nurse working at a children{\crq}s ward and an anaesthetic nurse in care of a child patient in pre-operation, per-operation and post-operation periods
ŽÁKOVÁ, Lucie
I selected the topic of the diploma thesis ``Cooperation between a nurse working at a children{\crq}s ward and an anaesthetic nurse in care of a child patient in pre-operation, per-operation and post-operation periods{\crqq} because I am very interested in the work of nurses and in preparation of children and their parents in these periods. These entire periods and the associated activities are very demanding and burdening for all the involved parties. The cooperation and communication between the anaesthetic nurse and the children{\crq}s nurse is crucial for the child to cope with the period. The objective of the diploma thesis was to ascertain the opinions of anaesthetic and children{\crq}s nurses on their cooperation, and to find out whether the cooperation between anaesthetic and children{\crq}s nurses influences satisfaction of a child{\crq}s needs in the concerned periods. The theoretical part of the diploma thesis addresses paediatrics and segmentation of childhood, anaesthesiology and the specifics of childhood, children surgery and nursing care of children in three operation periods in general terms. The practical part is divided into a quantitative and a qualitative subpart. The quantitative subpart is focused on cooperation between anaesthetic nurses and nurses working at the children{\crq}s ward, and uses the data collected by means of a questionnaire, while the qualitative subpart ascertains whether the cooperation between nurses has an influence on the needs of children in the concerned periods, and uses the data obtained from observation of children and interviews of nurses. The summary of the results revealed that hypothesis 1 ``Anaesthetic nurses and nurses working at a children{\crq}s ward cooperate in the pre-operation period{\crqq} was confirmed. Hypothesis 2 ``Anaesthetic nurses and nurses working at a children{\crq}s ward cooperate in the post-operation period{\crqq} was confirmed. Hypothesis 3 ``Anaesthetic nurses are interested in improvement of cooperation with nurses working at a children{\crq}s ward{\crqq} was confirmed. Hypothesis 4 ``Nurses working at a children{\crq}s ward are interested in improvement of cooperation with anaesthetic nurses{\crqq} was confirmed. The qualitative subpart revealed that cooperation between nurses influenced satisfaction of a child{\crq}s needs in the concerned periods. The findings of this diploma thesis may be used by children{\crq}s and anaesthetic nurses or by other persons interested in this issue for the sake of better preparation of a child for the operation period, better satisfaction of a child{\crq}s needs in the concerned period, and provision of more comprehensive care in the period in general.
Sectio caesarea on women´s request
VOJÍKOVÁ, Lucie
ABSTRACT Cesarean section by request of a para Cesarean section is a surgery terminating pregnancy in the third term and during the delivery. This surgery has been known already from the before Christ period. Cesarean section currently belongs to the most often carried out obstetrics surgeries. Much untrue and misleading information about the Cesarean section spreads among pregnant women; such as that a woman can decide herself whether to undergo the Cesarean section or to deliver her child vaginally. Pre-operative examination and an examination of health condition of the para to undergo the surgery precede the surgery itself. The medical action consists in providing the para with anaesthesia and opening uterine cavity through abdominal wall. After arrival of the child there follows uterine cavity revision, closing and sutura treatment of the section. All the above described is done under a strict asepsis and antisepsis in the presence of erudite doctors and nurses. The Bachelor{\crq}s Thesis set three targets. The first one was to find out if women are informed about the legal regulations related to a Cesarean section. The second target was to find out whether women prefer natural delivery to Cesarean section regardless of the fact whether they were giving birth or were pregnant. And the third one was to find out an interest of women in Cesarean section by request regardless of medical indication. Corresponding hypotheses were assigned to the targets. Hypothesis No. 1 says: Women are not sufficiently informed about the legal regulations regarding Cesarean section by request. Hypothesis No.2 says: Women prefer vaginal delivery to Cesarean section. Hypothesis No. 3 says: Women are not interested in Cesarean section by request without medical indication. The research was carried out using a questionnaire containing 21 questions answered by women in-patient at the Department of Gynaecology and Obstetrics or attending Obstetric out-patient department of Nemocnice České Budějovice a.s. Based on the research hypothesis No. 1 was prove as 43.75 % of questioned women sad to be informed about the legal regulations. The remaining two hypotheses were proved by the research. The conclusion of this thesis is finding out that the questioned women were rather well informed about legal regulations regarding Cesarean section by request. The women also mostly agreed that vaginal delivery is less risky and they would like to terminate their pregnancy by natural delivery. The research also found out that women are not interested in Cesarean section by request. This thesis could be used as an information booklet for pregnant women interested in Cesarean section. They could receive complex information about the Cesarean section.
Psychosocial support of children around the surgery planned operations
BLÁHOVÁ, Ilona
The study is focused on sufficient psychosocial support for children before and after planned surgical operations. This area of nursing has been still underestimated although medicine and nursing has been developing very fast in the Czech Republic. The importance of this support is often forgotten even if the contact between a child and his mother [parent] in such a difficult situation can definitely bring many positives. Unfortunately, children indicated for an operation are separated from their parents just at the hospital receptions or entering the operating room. The following preoperative preparation usually miss any psychological aspects and satisfactory explanation to children. They are often frightened and cry so it becomes very difficult for a nurse anesthetist to calm them down. Therefore the premedication and anesthetics have to be much stronger. Furthermore the psychic burden and total discomfort can cause many complications during the operation itself or in the postoperative care. The aims of this work was to find out if parents are interested in active attendance in preoperative preparation and postoperative care of their children, map out anaesthetist opinions on the fact that parents can accompany their children up to the operating room and compare advantages and disadvantages of it. In addition I tried to compare the occurrence of postoperative complications with children they have psychosocial support of their parents and those who are unaccompanied. This work was also focused on international comparison of the level and range of psychosocial care of children surgical patients among the Czech Republic, Hungary and Austria. All aims stated at the beginning were fulfilled. Within quantitative research five hypotheses were stated: 1/ Parents are interested in active attendance in preoperative preparation of their children indicated for operations. 2/ Parents are interested in taking part in postoperative care of their children. 3/ Anaesthetist nurses prefer children accompanied by their parents to the operating room. 4/ There are much less operative and postoperative complications if children are supported by their parents. 5/ Other countries (Austria, Hungary) encourage parents to accompany their children for operations much more than it is in the Czech Republic. Two research questions focused on the comparison of the level and range of psychosocial support of children surgical patients in the Czech Republic and abroad were defined for qualitative data analysis. 1/ Are there big differences in psychosocial care of children surgical patients in the Czech Republic and abroad (Austria, Hungary)? 2/ Are there good conditions for solid psychosocial care of children patients in the Czech Republic?
