Early mobilization in critically ill patient

Introduction: The most of critical ill patients is immobilised for short or longer period of time. This depends on the character of their illness, medication or another organ support. As it has been proved during critical illness and forced immobilisation there is an significant reducing in muscle strength, exactly in 3 to 11 percent with every another day of immobilisation (46.) It is necessary to say that early mobilisation is not standardised approach in Czech medical environment. Aim of tthe work: The main target of this diploma is to figure out whether early mobilisation in critical ill patients means any significant difference in their functional status in comparison to those given standardised physiotherapy. Another target of this research is to set up safe and structured protocol for leading early mobilisation and to determine whether exercising with cyclo ergometer is linked to higher risk of side effects and to state the most probable reasons for delayed or not performed rehabilitation. Research methods: This diploma thesis is based on randomised research. Participants in the control group are undergoing standardised rehabilitation, intervention group is undergoing extended rehabilitation with cycle ergometer. Title Case study of physiotherapy treament of the patient with the diagnosis of dermatomyositis. Objectives The aim of this work is to process knowledge for dermatomyositis and evaluation of approaches which are presented in the literature mentioned below. Second objective was to evaluate the impact of physiotherapy treatment for this disease. Methods The general part of the thesis includes theoretic knowledge about dermatomyositis. In the second part of this thesis I processed the case of a patient diagnosed with dermatomyositis under the leadership of an experienced physiotherapist for 10 days Results We have achieved quite a significant improvement. Treatment included physical therapy and pharmacological treatment with corticoids. Corticoids have partial portion of the over-all result. Keywords Dermatomyositis, polymyositis, muscle weakness.

Title: Case study of physiotherapy treatment of a patient after total knee replacement surgery. Thesis aim: Thesis is divided into two parts, the first part is theoretical and has aim to show an overview of anatomy, kinesiology and biomechanics of the knee joint and to show general information about the osteoarthritis and the patient's surgery. The second part is the case study where aim is to show the examinations and treatments that were provided to the patient and analyse the results. Clinical findings: The patient was a 71 years old male with diagnosis of total knee replacement surgery, performed 7 days ago from the time we met. The patient had limited ROM in the operated leg into flexion and extension and the area around the knee was swollen and warm. Also, he had some weakness on the muscles of the operated leg. Procedures: All the procedures that used were based on my knowledge acquired from Charles University in Prague, Faculty of Physical Education and Sport. These were post isometric relaxation, joint mobilisation, strength and length tests and strengthening exercises based on my knowledge. Result: After 7 days of therapies the patient's operated leg had physiological mobility into flexion and extension, the muscles gained strength and oedema was reduced but not completely. Monitoring postural and respiratory function in patients with Duchennne muscular dystrophy

Duchenne muscular dystrophy is a progressive muscle disease that is still incurable. The disease affects all systems in the body and is therefore a need for a comprehensive approach. It manifests itself in childhood muscle weakness. Over time, there is a postural and respiratory changes affecting posture and the respiratory system. Depending on the changes of these functions leads to certain complications. Among these complications rank poorly stereotype walk, spinal deformities such as scoliosis, bad stereotype affect breathing, leading to vital lung function and many more. These complications are present in a particular time sequence and can be assigned to a specific patient age. This thesis is focused on the monitoring of postural and respiratory function in these patients. The work is the evaluation of these changes when they occur and in which the patient's age. Patients are rehabilitated life and physiotherapy is an integral part of treatment. It focuses possibilities of surgical treatment of scoliosis. Title: Case study of physiotherapy treatment of a patient with Dermatomyositis. Aims: A summary of theoretical background, study of comprehensive rehabilitation methodology, proposal of therapy and evaluation of its effect in a patient with dermatomyositis. Methodology: Review of literature and case study was made during practice in Instute of Rheumatology in Prague from 9. 1. to 3. 2. 2012. It has two parts. Theoretical part sumarizes etiology, epidemiology, clinical manifestation, therapy and use of physiotherapy in dermatomyositis. A case study is presented in the special part. Therapeutical approach was prescribed according to the medical history and physical examination and its effect was assessed using kinesiology analysis at the end. Results: In my patient back pain was decreased and the trunk stability was increased. Also the overall physical fitness increased as well as the stereotype of gait and endurance. Positive motivation and results of therapy the patient improved patient access to rehabilitation in general. Muscle imbalance in students of physiotherapy

The muscle imbalance is a phenomenon which was first described by professor Janda. Some muscles have a tendency to shorten themselves, others to loose strength. The consequence of these tendencies is a formation of imbalances which may negatively affect motion senses as well as a whole motion apparatus. Predisposing the formation of muscle dysbalance is predestined by the superiority of tonic or phase fibers in the individual muscle groups. The disruption of coordination between muscles can lead to a formation of problems with motoric apparat. Postural changes and functioning problems can be deepened with lowering of deep stabilization as well as with inappropriate ergonomics. The physiotherapist often deals with patient's muscle imbalances, but that doesn't mean he can avoid the formation of these imbalances in his own body. The physical requirement for this kind of job demands the right ergonomics of work which lowers the risks. The advantage of physiotherapist is in his theoretical and practical knowledge, which he can apply on his own body, balance the muscle groups in his body and thus eliminate the risk of formation of functional problems and painful conditions. The main goal of this work was to map the occurrence of muscle imbalances in the selected group of students of the physiotherapy. The second goal was to suggest some precautions that would help to avoid the functional problems caused by the muscle imbalances. The study was focused on students of the first year of the physiotherapy discipline on the South Bohemia University. There were 37 participants from whom there were 33 women and 4 men. Due to the specific focus on the muscle imbalances the only muscles that were examined were shortened muscles and weakened muscles in connection with the quality of involvement into the motion stereotypes and also hypermobility. Due to the fact that muscle imbalances and functional disorders are closely related to the quality of the stabilization system and breath there was also evaluated the breathing stereotype and the ability to activate the diaphragm. The occurrence of the painful areas in muscles was mapped by the palpation examination. The students were asked targeted questions in order to find out if any of them had regular problems with for example backache, knees and so on and if they had an accident or underwent a surgery of the motion system. The students were also asked about the sports activity, if they do these sports on regular basis and which kind of sports they do specifically. For a complete evaluation of the student's endurance they were marked with the BMI index, the WHR index and the Ruffier index. In almost all cases there were some sorts of imbalances. In the evaluation of the upper body imbalances (upper crossed syndrome to be exact) there were only 2 students who didn't have this dysbalance. On the contrary there were 10 students with all sorts of imbalances and thus the upper cross syndrome was proved. In the evaluation of the lower body imbalances (lower crossed syndrome) all students had some kind of dysbalance, but there were only three students with proven lower cross syndrome. From the evaluation of the breathing types and diaphragm tests was proven that only eight students can activate diaphragm and diaphragmatic breathing. Due to positive occurrence of some kind of muscle imbalances in every student's body it was recommended to accept some sort of precautions. But these precautions can't be used universally, because every combination of different imbalances requires individual approach. This work can be used by professionals as well as by students of physiotherapy. This work can also be extended by another research which would compare the results with, for example, the occurrence of imbalances in higher classes of physiotherapy or in another discipline.
