Application of the principle of autonomy in the field of death and dying of seniors in the Czech Republic

Diploma thesis "Application of the principle of autonomy in the field of death and dying of seniors in the Czech Republic" primaly deals with issues related to the right to self-determination, human dignity and desires associated with the end of life. It seeks to answer the question of the extent to which the autonomy of the individual can be respected and to participate in end-of-life healthcare decisions through the legal concept of living will, even if the person is in a position that does not allow to express the will. Although this institute is firmly rooted in the legal order of the Czech Republic, awareness of this legal concept is insufficient among the public and rarely used, unlike the Federal Republic of Germany, where the institute is usually applied by seniors. The aim of the work is to reflect the current issues of dying and death from an ethical point of view and provide comprehensive information on the current possibilities of exercising the right to self-determination, which can significantly support the quality of human care in the terminal phase of life.
The Needs of the Dying and Their Care-Taking Family Members in a Mobile Hospice

This bachelor thesis focuses on identifying the needs of the dying and family caregivers in a mobile hospice. The aim of the thesis is to find out what the needs of patients and carers in a mobile hospice are and how the hospice meets these needs. The bachelor thesis is divided into theoretical and practical parts. The theoretical part deals with the services of mobile hospice, the most common problems that are solved for patients, their caregivers and also for the bereaved. The practical part focuses on the research and its results. The research was conducted using semi-structured interview and observation method. The research revealed that the most common problems are concerns about pain, loneliness, and securing medication that will control other negative symptoms of the illness. In addition, the concern of the family and hospice staff is important to the patients. Psychological support, help with care, medical devices are most important for carers. The research part of the thesis showed that mobile hospice helps to meet the needs of the dying and their family members. All respondents were maximally satisfied with the care provided by the mobile hospice. The results of the bachelor thesis will be provided to mobile hospices in the South Bohemia region, where they can be interpreted as feedback to the entire mobile hospice team.
Essential factors in the solution of ethical dilemmas in the Hospice care of St. Kleofáš, o.p.s.

The diploma thesis focuses on the topic of signifiant factors influencing the final solution of ethical dilemmas in Hospice Care St. Kleofáše, o.p.s. The thesis provides an overview of the specifics of hospice care and non-profit organization Hospice St. Kleofáše, o.p.s. The aim is to find out signifiant factors influence the solution of ethical dilemmas in this organization and which of the identified factors coincide with selected values from the Code of Ethics for Hospice Employees. The diploma thesis describes hospice care, social work in a mobile hospice, ethical issues and selected values from the Code of Ethics for Hospice Employees. It also describes the process of identifying factors influencing ethical dilemmas and the results of semi-structured interviews. The aim of the work is to show the issue of decision-making ethical dilemmas.
Relationship between the state and selected NGOs in the field of providing paliative care in the Czech Republic

The aim of the thesis is to describe the role of a sample of NGOs in the process of changing the current situation in the field of palliative care in the Czech Republic. The essence of palliative care is to ensure the highest possible quality of life for the patient and his family. The present state of care is not sufficient for the number of palliative beds or home care service teams to meet the needs of an aging Czech population. Almost the exclusive care provider are NGOs, which also take on the role of initiator of a change in the persistent situation. Based on the data from semi-structured interviews with ten respondents, the thesis method of the case study reveals what is causing the unsatisfactory situation, what proposals its solutions civil society offers and what range of means to achieve change.
Training and Education of Volunteers in the Home Hospice "Cesta domů" - the Homecoming

The issue of death and dying affects everyone, it is not easy and yet a natural part of everyone's life. Hospice care offers help and support a dying man and his close on the last trip "home". This thesis describes the possibilities of a preparatory course for the volunteers who play an irreplaceable role in hospice care. The practical part describes the specific example from the conditions of the Czech home hospice - The Home Coming, which is being commented on the ground of literature and my personal experience. The outcome of the research is the feedback for the management of the home hospice - The Home Coming, including recommendations for changes of the preparatory course.

In our diploma thesis "Home hospice and palliative care" we deal with home hospices and, to a lesser extent, inpatient hospices. We also discuss palliative care, futile treatment, euthanasia and previously expressed wishes. The work also deals with legislative laws or regulations regarding hospice palliative care and health care in general and ethics. The work mentions mobile hospice teams, palliative care counseling teams in hospitals, care for terminally ill patients. The importance of the presence of loved ones at the time of the patient's death, the lack of palliative beds in the Czech Republic and the little public awarness of hospice care, previsouly expressed wishes and other home hospices as an alternative to euthanasia are determined. Current forms of care focused on dying people in a home environment

After 1989, we have seen a great development of hospice and palliative care in the Czech Republic. At that period, the first hospice institutions came into being which later broadened their concepts on providing also the at-home care. The main aim of this thesis is to characterize the available concepts of the care of the dying patients in their home environment in the Czech Republic. The initial theoretical section introduces the definitions of the basic concepts of palliative and hospice care; the characteristics of the key institutions providing the at-home care; and deals with the hospice and palliative care legislative. The practical part, based on the use of the qaulitative research design and semi-structured interviews, helps to show the experience of various providers of palliative services. The analytic part depicts two concepts of the care of the dying people, which were identified on the base of the linterviews and which are currently available to patients. This diploma thesis is focused on the issue of home hospice care in Blansko and its surroundings and it presents existing information about palliative care. The text preferably examines the current state of awareness of this care among the general public and its availability in South Moravian region. In addition the text offers suit-able marketing plan for development of St Martin's Hospice in Blansko, three action plans are evaluated.
The issue of palliative care in hospice and mobile facilities in the Czech Republic

This rigorous thesis pursues evolution of specialized palliative care and looks at the same time into problems of contemporary legislation and current legal frame of for care in mobile hospices in the Czech Republic. It thus builds on my previous master thesis on euthanasia, which discussed the Act on dignified death submitted to parliamentary debate by Zlatuška, M.P. (ANO) on May 27, 2016. Euthanasia is often perceived as an alternative to palliative care. There are several different forms of palliative care - hospice palliative care, special beds/stations for palliative care in hospitals and care in a mobile hospice. The analytical part of the thesis will include analysis of palliative and hospice care issues based on existing documents and secondary analyses. Methodologically, qualitative approach based on semi-structural interviews with relevant actors from political as well as healthcare spheres will be used. Political actors will be represented by the Ministry of Health and health insurance companies. Attention in the healthcare sphere will be focused on providers of palliative care and founders of hospice facilities and mobile hospice care facilities.
