Nutrition in endometriosis

Endometriosis is a chronic gynecological estrogen-dependent disease affecting women of reproductive age. The disease is characterized by the presence of endometrial glands and stroma in ectopic locations, especially in the small pelvis, on the ovaries, peritoneum, rectovaginal septum and bowel. However, any organ except the heart and spleen can be affected by endometriosis. The presence of the endometrium outside the uterus gives rise to inflammatory deposits - endometriomas. These deposits, like the endometrium in the uterus, undergo changes during menstruation. However, unlike the endometrium in the uterus, this endometrium in ectopic sites has no way to leave the body, resulting in aseptic inflammation, scar formation, adhesions and subsequent painful adhesions. Typical symptoms of endometriosis include dysmenorrhea, dyspareunia, dyschezia and dysuria. The main clinical symptoms are chronic pelvic pain and infertility. However, endometriosis can also occur asymptomatically. Endometriosis can be divided into four subtypes according to the localization - peritoneal endometriosis, ovarian endometriosis, adenomyosis and deep infiltrating endometriosis. In my bachelor's thesis, I will focus on the topic of endometriosis and its impact on a woman's quality of life, pregnancy, and childbirth. Endometriosis is a gynecological condition characterized by the pathological presence of uterine lining, called endometrium, outside the uterine cavity. It can affect almost any organ in the human body, most commonly the adnexa or pelvic cavity. Ectopically located tissue is subject to cyclic hormonal changes and can form adhesions or cysts, which can be a source of severe pelvic pain. It is thus a chronic condition that affects a woman's daily life, especially her reproductive health. My work will consist of two parts - theoretical and research. The aim of the theoretical part will be to briefly describe and summarize current basic knowledge related to endometriosis, including the definition of the condition, its causes, symptoms, basic classification, and subsequently diagnostic and therapeutic procedures. Lastly, I would mention the issue of conception in women with endometriosis and the impact of this disease on the quality of life. It is a chronic inflammatory estrogen-dependent disease associated with a variety of clinical manifestations, the most common of which are pain and fertility disorders. The treatment of this disease is rather symptomatic and does not always provide the desired effect. Physiotherapy can be used both in perioperative care and as a complementary treatment method aimed at improving the quality of life and influencing some painful symptoms. The aim of the bachelor thesis is to create a comprehensive overview of endometriosis focusing on its origin, clinical manifestations, and treatment options including physiotherapy. Another goal is to conduct a systematic review of clinical studies examining various physiotherapeutic techniques in the treatment of endometriosis and determine their therapeutic effect. Awareness of women with endometriosis about influence of nutrition factors on this disease

Endometriosis is chronical, inflammatory and very painful disease which contains with estrogen. This disease affect about 10 % of female population in childbearing age. Etiology and pathogenesis of this illness is largely unkown. 20-40 % of women who suffer from endometriosis have concurrent problems with sterility and 50 % of women suffer from various forms of sexual dysfunction, the most common of which is pain. It is assumed that, that symptoms of endometriosis might be affected by diet interventions. Objectives of this thesis: 1. Determine awerness of women in fertile age with endometriosis about influence of nutritional factors on this disease. 2. Determine, if women with endometriosis were instructed by their doctor (gynecologist) about influence of nutritional factors on this disease. 3. Determine, if women with endometriosis changed their eating habits. Part of the research were 212 women with diagnosed endometriosis, which were addresed with using of questionnaire in Nemocnice České Budějovice and electronical questionnaires. The effect of endometriosis on pelvic floor function

Thesis focuses on the comparison of results obtained from the questionnaires MEDI-Q, PFIQ-7 and a questionnaire designed by the author between a group of women diagnosed with endometriosis who have not undergone surgery and a control group without dysmenorrhea. The theoretical part is dedicated to the etiology and clinical symptoms of the disease, mentioning risk factors, classification, diagnostical methods and treatment options. The thesis also mentions the typical postural findings in patients with endometriosis after a literature review. Methodology: A total of 110 women were included in the study, 49 with endometriosis and 61 in a control qroup. Women completed the above-mentioned questionnaires electronically on the 1st day of their menstrual bleeding. Recruitment of subjects was mainly through the social network. Results: Significant differences (p <0.001) were demonstrated between the two groups using the MEDI-Q and PFIQ questionnaires with the endometriosis group achieving higher scores. Different types of endometriosis have varying distributions of symptoms within pelvic floor dysfunction. Midwife Care for Woman with Endometriosis

This bachelor thesis deals with the topic of midwife care for a woman with endometriosis. The thesis is divided into theoretical and practical part. The theoretical part is mainly focused on the endometriosis disease. The theoretical part describes the definition of endometriosis, its causes, diagnosis, manifestations, treatment options and midwife care. For the practical part, two objectives were chosen. The first aim was to find out how women with endometriosis perceive midwife care. The second aim of the thesis was in turn to find out how midwives perceive the whole issue of this disease. The bachelor thesis had three research questions. The first research question focused on how women with endometriosis perceive midwives' care. The second research question explored how the lifestyle of women with the condition has changed. The third research question looked at how midwives perceived the issue of endometriosis. The aforementioned objectives were achieved through a qualitative research investigation in the form of semi-structured anonymous interviews. Data collection for the research inquiry took place from January to March 2023. In the practical part, two research sets were chosen. The first research set consisted of six midwives who encountered women who had been diagnosed with endometriosis during their practice. To preserve their anonymity, the midwives were referred to as PA1 to PA6. The second research group consisted of eight women with endometriosis who were referred to as Ž1 to Ž8 in the thesis. The data collected was transcribed and then further analyzed using text coloring. Main categories and sub-categories were established for data analysis, these are further described in the research section of the thesis. The first research set consists of two main categories and four subcategories. Two main categories and six subcategories were obtained from the informants' statements of the second research set.
Womenś awareness of endometriosis in the Czech Republic

The theme of my bachelor thesis is called "Awareness of women about endometriosis in the Czech Republic". The thesis consists of two parts - theoretical and practical. In the theoretical part, I deal with the anatomy and physiology of female genitals. The next chapter focuses directly on endometriosis. Contains subchapters related to with the characteristics, origin, clinical symptoms, diagnosis, treatment, etc. Last but not least, I focus on the role of the midwife in the care of a woman with endometriosis. In the practical part, I examined women's awareness of endometriosis. To collect the data, I used an anonymous quantitative questionnaire, which I shared using social networks. The questionnaire could be filled in by a woman of all ages living in the Czech Republic. 106 respondents took part in the research, who filled in a questionnaire in the period from 10 February 2020 to 9 March 2020. Before starting the practical part, I set out hypotheses that I was able to confirm or refute thanks to the obtained questionnaires. My main goal was to find out how many women know what is endometriosis. Based on this goal, I set a hypothesis, which was confirmed and that more than half of the respondents know the disease. Assessment of the endometriosis in the pelvis.

Introduction: The main goal of our work was to compare the diagnostic accuracy of ultrasound examination and magnetic resonance imaging (MRI) in the assessment of deep endometriosis in the pelvis. Our second aim was to establish a learning curve of a sonographer and a radiologists (non-experts). As an introduction to the subject we have published 3 reviews on the diagnosis and one narrative review on the classification systems in the evaluation of endometriosis has been submitted for publication. Methodology: Reviews were written as narrative reviews based on PubMed search and scientific societies recommendations. We have offered participation to all patients in the endometriosis centre with high suspicion of deep endometriosis, who then underwent examination by ultrasound and MRI by expert and non-expert before their surgical treatment, all findings described according to the consensus IDEA (international Deep Endometriosis Analysis group, 2016). Surgical and histological findings were used as a reference standard. Learning curve was defined as an improvement in accuracy in three blocks, into which the patients were assigned in the chronological order. Rare cells in diagnostics and monitoring of gynecological diseases

In the last two decades there is an enormous effort to discover a non-invasive marker to diagnose, predict and monitor therapy effect of malignant diseases. Circulating tumor cells (CTC) have the ability to fulfil all these criteria. The introduction of the monothematic thesis deals with the problematics of CTC in general and its application in clinical medicine. It is followed by the author's first publication, which reviews the current status of CTC in gynecological malignancies. The next publication is an original article about CTC in patients with endometrial carcinoma. It reports a successful isolation of vital CTC in 75% of tested patients and identifies CTC as an individual marker of the disease without correlation to the stage, grade or lymph nodes involvement. The second part of this thesis deals with endometriosis, a benign but often recurrent disease worsening the life quality of women in reproductive age. The multicentre study presents a successful isolation of circulating endometrial cells (CEC) in patients with histologically proven endometriosis with various stages and symptoms. Endometriosis, its aetiopathogenesis and influence on female's fertility

Endometriosis is an estrogen-dependent disease of women of reproductive age. It is often considered as one of the main causes of female infertility. Its development and progression are influenced by several exogenous and endogenous factors. It is a heterogeneous disease and one of the most important tasks is to identify the candidate genes. That will allow us to better understand why and under what circumstances endometriosis develops. Because not all women with endometriosis are infertile, I focused in this thesis at selected candidate genes, their alleles and epigenetic dysregulation that could link endometriosis to infertility. From the available literature WNT4, HOXA gene cluster, ESR1, PGR, FN1, VETZ, GREB1 appear to play a major role in the pathogenesis of endometriosis. These genes also play an important role in various biological pathways and their dysregulation leads to low implantation / complete implantation failure, insufficient oogenesis, embryogenesis, and infertility in patients with endometriosis. Identification of genes whose dysregulation induces the development of endometriosis in the population is important, given the increasing incidence of endometriosis and infertility rates in recent years.
