Specifics of crisis situations with a large number of patients with thermal injury.
MADĚRÁNKOVÁ, Adéla
This thesis deals with an emergency incident in which a burn trauma occurred in a large number of people. The theoretical part of the thesis describes both the legal framework of emergencies and especially the recommended procedures of the components of the Integrated Rescue System for dealing with this type of emergency. A large part of the theoretical part of the thesis and the whole practical part is focused on the procedure of the emergency medical service. The document shows how burns are treated from the point of view of emergency medicine, but also from the point of view of disaster medicine. The practical part of the work responds to the Concept of dealing with an emergency with a large number of patients with thermal injuries in the Czech Republic, published by the Ministry of Health in 2020. I focused on how paramedics know it. Does their knowledge affect the length of practice or the level of their acquaintance with the Concept? I found out using questionnaires filled out by 42 rescuers from different regions.

Background: The thesis follows up the comparison of fear of pain, pain intensity, emotional attachment, anxiety and depression in patients with after burn trauma and healthy individuals. Aim of thesis: The aim of the work was to compare the fear of pain in patients with after burn trauma with healthy individuals and to find predictors of pain; to find significant predictors od pain intensity and fear of pain in patients with after burn trauma. It also focuses on the evaluation of the relationships between pain intensity, fear of pain and sociodemographic variables (age, employment, premorbid intellect) and clinical parametrs of burn trauma (extent of burns, lenght of hospital stay, location of burns, psychiatric comorbidities); evaluation of the relationships between pain intensity, fear of pain end emotional attachment in multi regressin in patients and in health controls; evaluation of the relationships between pain intensity, pain fear and anxiety / depression in patients and healthy individuals, as well as evaluation of the relationships between pain intensity, pain fear and health perception in patients and healthy individuals. Methods: The collection of data for this bachelor's thesis took place at the Department of Burn Medicine, 3rd Faculty of Medicine and Hospital Královské Vinohrady in Prague. The...
Treating a patient with burn trauma by ERS
SCHLOSSER, Matouš
The topic of this thesis is treating the patient with burn trauma by ERS. This issue is still discussed and guidelines for treating this injury are constantly updated The theoretical part deals with anatomy and physiology of skin as well as burn pathophysiology. This part of work also provides information about assessing burn severity, first aid of burn trauma for laics and treating burn trauma in pre-hospital care by ERS. In the end of theoretical part the differences in treatment between adult and pediatric patients and their transport to specialized center  are described. The research part has two main goals. First goal is to find out the level of knowledge paramedics have about burn trauma treatment. The other goal is to find out which equipment is available for paramedics in pre-hospital care and whether it is adequate. Eight paramedics participated in the research. Four respondents were from South Bohemian region and four respondents were from Vysočina region. To achieve these goals a method of qualitative research has been used. Data for the research had been gathered by using semi-structured interview Research found that most of the paramedics have knowledge to adequately treat burn trauma and that equipment in ambulance cars is sufficient.
Treating a patient with burn trauma by RZP crew
SCHLOSSER, Matouš
This bachelor thesis is about treating patient with burn trauma by RZP crew. This topic is still discussed and guidelines for treating this injury are always being updated. The theoretical part is about anatomy and physiology of skin. The basics of burn patophysiology are also mentioned there. Next part of theoretical part is about evalating of burn severity. In thesis there are summarized laic fist aid of burn trauma. End of theoretical part is about treting burn trauma in pre-hospital care by RZP crew. The research part has two goals. First is to find out the level of knowledge of paramedics of South Bohemian region and region of Vysočina about problematics in this area of expertise. The second goal is to find out which equipment is at disposal for paramedics in pre-hospital care and if it is adequate. Four paramedics from South Bohemian region and four paremedics from region of Vysočina participated in this research. To achieve these goals a qualitative research has been used with data gathered by using a semi-structured interview. Research found out that most of the paramedics have adequate knowledge to adequately treat burn trauma. Also equipment in ambulance cars is adequate.

Rybářová, Kateřina ; Svěcená, Kateřina (advisor) ; Uhlířová, Jaromíra (referee)
of the Thesis: The aim of this bachelor thesis is to describe the occupational therapy interventions for adult people with burn trauma and to identify the differences between occupational approaches for adult patients with burn trauma in the Czech Republic and Sweden. The process of treatment of people with burn trauma is described from medical and occupational therapy point of view in the theoretical part. The partial occupational interventions, which are used for the aforementioned target group, are also discussed. The summarized information about Czech and Swedish health service is also provided and analysed, including the statistical data about hospitalized patients with burn trauma in those two countries. The partial differences in the occupational therapy for adult patients with burn trauma in the Czech Republic and Sweden are described in the practical part. The differences were determined by using various procedures of methods of qualitative data collection. Occupational therapy has not been a common part of the treatment process of burn trauma in the Czech Republic yet, although it is very likely it would positively affect the quality of patients' lives. The thesis presents arguments in favour of changing this situation and defends occupational therapy as a part of treatment process. Key...
Compartment syndrome in pre-hospital urgent care
BENEŠOVÁ, Lucie
This bachelor thesis focuses on conditions associated with compartment syndrome development in prehospital care. Pressure increase in a closed anatomical space is characteristic for this condition, leading to circulation and neural sensitivity affection in the specified area. In the prehospital care, we may meet with the compartment syndrome in fractures, burns, allergic reactions, traumas caused by an accident, electrotraumas; further in crush syndrome, traffic accidents, craniocerebral injuries and other conditions associated with swelling. This thesis focuses on burns, fractures, electrotraumas, intra-abdominal hypertension and conditions caused by snake bite. Inconsiderate or inadequate manipulation with affected patient may worsen his/her condition and lead to deterioration of his/her health condition or even fatal consequences. The aim of this thesis was to map the theoretical and practical knowledge of non-medical health care professionals working in prehospital care, in treatment of conditions able to delay compartment syndrome development or even to accelerate it. Data collection for the research was performed using questionnaires and semistandardised interviews. Questionnaires were filled by 20 randomly chosen respondents. 12 respondents were interviewed. Correct answers evaluation is listed in the tables in the practical part of this thesis. Interviews transcript is a part of the practical part as well, with colour coded correct and incorrect information provided by the interviewed medical rescuers. A sample test and list of posed questions with sub-questions are included in the attachment. Both parts of the research were performed with medical rescuers and general medicine nurses working at medical rescue services of southbohemian region and the region of Vysočina. The research found that the term of compartment syndrome is well known by medical rescuers and general nurses working at medical rescue services. However, they are not aware of its association with prehospital care. In some cases, the answers of the responders differed significantly, as for treatment of acute conditions associated with this problem, from the recommended procedures according to the literature. Furthermore, the research analysed the type and amount of used fluids in pre-hospital care. This question showed interesting findings as well. Lastly, the research analysed the options of acute fasciotomy in circular neck burns in prehospital care. The respondents interpreted this procedure as life-saving, as this condition is life-threatening for patients and quick transport without releasing incisions is very risky. It is a procedure that should be known to medical rescuers, at least at the theoretical level, and considered life-saving, even if they do not have sufficient competences to perform it. Respondents showed interest and effort for deepening of their knowledge about this procedure during the research. This situation may be improved by additional training in traumatology, surgery and intensive care not only in medical rescuers working in medical rescue services, but in students of these specialisations as well. This thesis could be a base for deeper investigation of this topic, currently insufficiently known; furthermore, for increasing specialised knowledge of medical rescuers in treatment of oedematous conditions and increasing of health care quality in prehospital acute care.
The level of education of the patient with burn trauma
VLKOJANOVÁ, Lucie
Burn trauma is as old as the mankind itself. Forwarding information, whether oral or in any other form is important nowadays. Most of the patients want to be informed about their health condition, treatment procedures applied by physicians and nurses, methods, options and regimes that have to be adhered to in home treatment. The Bachelor Thesis focuses on education of patients with burn trauma. Patients? view on the issues in question, but also that of the nurses taking care about burnt patients is important. The aim of the work was to find whether patients with burn trauma have sufficient information on the follow-up care and whether nurses educate patients with burn trauma about the follow-up care, what methods nurses apply on education of patients with burn trauma. We asked the following research questions before the research started: ?Do patients with burn trauma have enough information on follow-up care??, then: ?Do nurses educate patients with burn trauma?? and: ?What form of receiving information about burn trauma and the follow-up care do patients prefer?? The above questions were answered by a quantitative research by the method of semi standardized depth interviews with patients of the Burn Medicine Clinic of the University Hospital Královské Vinohrady in Prague and with the caring staff, general nurses from a standard department thereof. The interview with patients consisted of 20 depth questions and that for nurses of 17 depth questions. The interviews were anonymous. All the obtained information was processed into case studies and then into category charts. The research confirmed all the 3 aims of the Thesis. Patients with burn trauma do not have sufficient information on follow-up care even though nurses do educate them about such care. We also tried to find what kind of methods nurses used for patient education. The solutions proposed are: to improve awareness of prevention and first aid, to provide more information on treatment regime during hospitalization and subsequent home care in the form of leaflets or brochures. Apart from being costly the care about patients with burn trauma is also mentally and physically demanding. On the other hand patients hospitalized at the BMC for a long time deserve to be informed on their trauma. The nurses mentioned a lack of time for education. It is however clear that if patients had information in written oral education would not be so time consuming then. None of the patients required permanent presence of a nurse, but they did require information on the treatment methods they were undergoing, about which they did not have any information.
Treatment of burn trauma in prehospital emergency care
HASMANOVÁ, Ilona
The number of serious thermal injuries keeps increasing. Their treatment requires fast, first-class professional, well-organized service, with an adequate material and personnel provision, capable of dealing with even a sudden occurrence of a high number of afflicted persons in one location. The treatment of such injuries requires an interdisciplinary team of cooperating professionals, including the care provided in specialized facilities. I have selected the topic of treatment of trauma caused by burns because it is highly relevant for everybody as it may afflict each person at any time. Treatment of burns in field conditions is far from easy and it has its specifics. The specific features consist in somatic aspects of nursing activities but also in mental aspects. The objective of the thesis was to summarize theoretical information about treatment of burns in the conditions of pre-hospital care and to map the knowledge and skills of paramedics working for the Emergency Medical Service. The thesis consists of two parts: theoretical and practical. The theoretical part explains the term of pre-hospital immediate care and identifies the scope of its application and the providers of such care. The burns are presented and clearly classified based on many aspects and the thesis describes their incidence and, last but not least, also the prevention. The thesis also deals with specific features of nursing care in the conditions of pre-hospital immediate care from the viewpoint of nurses and subsequently also with the field treatment of trauma caused by burns. The practical part used quantitative research, method of inquiring with a questionnaire. The results indicate that paramedics are completely familiar with the procedures to treat trauma caused by burns, that that the cooperation between the Emergency Medical Service and specialized burns centers is sufficient and that the method of intraosseous infusion to children is not sufficiently used. This means that all of the three hypotheses have been confirmed. The results of the research suggest that more attention should be paid to treatment of burns not only in classroom but mainly in a practical part of the training. It is because burns may result in life-long consequences and their treatment in pre-hospital immediate care shall be therefore of the highest professional quality.
