This master’s thesis aims to analyze the possibilities of failure of human reliability on the example of an induction heater from company Roboterm spol. s.r.o. in Chotěboř. The objective of a thesis is a review of a literature of human reliability assessment. Based on the analysis of the operation and a literature review and choose the appropriate method to perform the actual assessment of the reliability of human error. Final object of the work is to propose measures to increase human reliability.
Assessment of human reliability in a working system

The present thesis focuses on the issue of assessing the reliability of the operator while operating CNC laser Adige LaserTube 8. The objective of this study was to perform a review of literature, which concerns the issue of assessing the reliability of the human factor. On the basis of the analysis was to choose the appropriate method in order to identify and assess risks in their workplace and then to assess the reliability of the operator. The practical part is to propose suitable measures to increase the reliability of the operator.
Draft methodology for investigating the causes of aviation accidents caused by human factor in general aviation

This Master´s thesis examines Investigation of aircraft accidents / incidents, which were caused by human error. My thesis is focused on fixed wing aircrafts with maximum take off weight up to 2500 kg. Practices of Aircraft accident / incident Investigation are described in Annex L13 which is document published by Ministry of Transport of the Czech Republic. This document provides some support for investigators but does not contain methodology which should be used to discover human error by which the accident / incident was caused. This thesis has two major purposes. First purpose is to analyze Final reports of investigations and suggest improvements which can be made. Second goal of this thesis is to create own methodology for investigating the causes of aviation accidents / incidents caused by human factor.
Human factor in the aircraft operations

This bachelor´s thesis examines air accidents which is caused by human error. From selected air accidents are pointed out mistakes, which is caused by flight crew, their continuation, increase, main causation and formation air accident. Study of human mistakes and influence phase of flight are shown in diagrams. The conclusion of the bachelor thesis includes possible solving to elimination human error and increasing air traffic safety.

Considerable attention is paid to the errors that may occur during the management, their analysis and proposals for their elimination, respectively their complete disposal. The main idea is to understand the thinking of the air traffic controller as the human element in an organization and applying theories to reduce the errors in the air traffic control.

The master thesis is focused on the reliability of the human factor in the operation of the production machine. In the first part the basic legislative requirements for safety and reliability of work are presented. In the second part, the reliability of the human factor is discussed along with a description of selected methods for assessing the reliability of the human factor. The research section then concludes with a proposed methodology for human factors assessment for the manufacturing process. In the practical part, the proposed methodology is applied to the selected process, which includes risk identification and outputs from the selected methods. The thesis then concludes by recommending preventive measures to eliminate the identified risks.
Human factor in air transport

This dissertation examines the problems of human error in civil aviation. Its aim is, first and foremost, to evaluate types, origins and frequency of particular mistakes in piloting technique of aeroplanes. The dissertation comprises a study of the commonest human errors as presented by the examples of European airlines. The conclusion of the dissertation details proposals of method and approach on eliminating human error and thus increasing air travel safety.
