Efficiency of Robot-assisted Therapy through the Device Armeo Spring in Patients after Stroke in Acute Phase of Early Rehabilitation

This diploma thesis deals with the issue of robotic-assisted therapy, namely with the mechanical exoskeleton robotic device Armeo Spring, and it is focused on an acute stroke population. The aim of this study is to evaluate the efficiency of a robotic-assisted therapy using Armeo Spring device and to compare it with the efficiency of a common individual occupational therapy focused on improving motor function of upper extremity of patients with the acute stroke. There were 19 patients involved in the research, who were hospitalized in early rehabilitation acute beds. They were selected on the basis of entry criteria and further divided into two groups. An experimental group included 10 participants (n=10), a control group included 9 participants (n=9). The patients in the experimental group using Armeo Spring device underwent four to five 30-minutes therapies per week for three week period, the control group underwent the same amount of individual occupational therapies focused on improving motor function of a paretic upper limp. Each participant, evaluated before and after the intervention, underwent the total number of 12 therapy sessions. Bilateral Upper Limb Training in Patients with Central Hemiparesis

Background: Bilateral training is an intervention used in patients with hemiparesis. This training is based on the movement of both (bi) sides (lateral) of the body simultaneously. There exist a lot of articles focused on specific methods of bilateral training. However, the used terminology is inconsistent. The aim of this thesis is to describe the neurophysiological background of bimanual and bilateral methods, review and clear out the used terminology, and describe the different techniques included in bilateral training. Methods: Relevant literature was searched in PubMed, ScienceDirect and OVID Evidence Based Reviews. The inclusion criteria were the availability of fulltext, publication date after 1980, English language and a connection with medicine. The searched keywords were bimanual or bilateral training, metod and therapy. Results: From 1021 articles, only 132 articles meet the inclusion criteria. Five main rehabilitation interventions are used in practice: a) cooperative activity of both arms, b) synchronous movement of both arms c) Mirror therapy, d) an activity, where the impaired hand assists the paretic hand and d) robotic therapy or therapy with mechanical devices. INTRODUCTION: By the age of 65, every driver holding a valid driving license is required to undergo a compulsory medical examination according to Act 361/2000 Coll. The purpose of the examination should be to examine not only physical, but also psychosocial and cognitive factors that can affect the ability to drive safely. However, the extension of the validity of a driving license may have a considerable impact on the preservation of the existing self-sufficiency. GOALS: To determine the relationship of anamnestic data on the state of driving a motor vehicle with the state of cognitive functions. As a secondary goal, the work asks whether there is a correlation between the results of the Montreal cognitive test (MoCA-CZ1) with newly created memory screening tests (POBAV, ALBA and DOZNAT). METHODOLOGY: The research was based on a qualitative examination of the 39 drivers older than 65 years. Drivers were assessed by the state of cognitive function according to the Montreal Cognitive Test, and were then examined by "new short cognitive tests" targeting different types of memory. The Occupational Therapist's Intervention for Adult Patients with Neurogenic Dysphagia Abstract of the Thesis: Dysphagia is a serious disorder, often caused by a cerebrovascular accident; in many cases it may be its only or primary symptom, and even the cause of death. The main objective of this thesis is to raise awareness of swallowing disorders among occupational therapists. Despite the fact that the occupational therapist's intervention for patients with swallowing disorder is common practice among occupational therapists abroad, this issue still is not well known among occupational therapists in the Czech Republic. There is also a considerable lack of Czech literature dealing with swallowing disorders from the occupational therapy point of view. This thesis therefore not only presents theoretical findings in this field but also recommends practical procedures for occupational therapists. The theoretical part of the thesis describes functional anatomy of the swallowing structures and swallowing physiology and pathophysiology. It analyses different causes of dysphagia with a focus on neurological disorders, defines the term dysphagia and other related terms, and briefly explains several classification methods of dysphagia. The issue of driving a car in patients after stroke - the possibility of occupational therapy intervention

The issue of driving a car after stroke is getting increasingly common, especially because of the high stroke incidence concerning productive age patients. Nowadays, driving is considered a matter of course which intervenes our leisure time activities and work performance. Self-sufficiency and independency of a patient is one of the domains of occupational therapy, which is why the occupational therapist is a part of an interdisciplinary team. The theoretical part of the thesis defines terms relating to stroke and further neuropsychological deficits. It describes problematics of returning to automobile driving and legislative aspects. Possibilities of occupational therapy interventions relating to driving are proposed as well. The practical part of the thesis stands on a survey research, conducted in the Czech Republic and Belgium. The survey was aimed at occupational therapists who work with neurological patients and focuses on the issue of returning to driving and possible occupational therapy interventions. The conclusion also describes and compares practical Czech and Belgian occupational therapy with patients who have undergone a stroke. Woman after breast cancer surgery in productive age and their return to employment from the view of occupational therapy

Breast cancer in women of productive age is a huge problem of the current population. Approximately 52% of women, which were diagnosed with breast cancer in 2013, were aged 20-64. The aim of this thesis is to discover the influence of surgical solution of breast cancer on women return to their original profession. There were 68 suitable participants, who received surgical treatment, were aged 20-64, with finished treatment and who went back to their original employment. The results show that amongst manually working women, 60,9% return to their original employment, in administrative 78% return, which, in acceptance region of 0,05 (p = 0,142), is not a statisticaly significant difference. Furthermore, it was not found that age and lenght of treatment are connected (p=0,040). It appears that the time it takes to return to employment is significantly connected to the type of operation (acceptance region 0,05, p=0,046). This research shows how important it is for women after breast cancer treatment to receive timely rehabilitational intervention and to return to employment and how this affects their quality of life. Functional electrical stimulation of paretic upper extremity in patients after stroke

This study aims to evaluate the effects of functional electrical stimulation on motor and functional abilities of paretic upper limb in patients after stroke. Specifically detects the immediate effect of functional electrostimulation on the increase of the active range of motion, reduction of the hand spasticity degree and the improvement of functional performance. The research covered a total of 6 patients after first stroke in the basin aretria middle cerebral artery. The research sample had a homogeneous distribution of the paresis heaviness from light, moderate, to severe. The research was conducted in total of three weeks, every working day, 60 minutes a day, with 30 minutes of indicated electrostimulation and 30 minutes of testing of monitored parameters. Patients were evaluated using the Modified Frenchay scale, Modified Tardieu scale and the range of motion goniometer. Research showed in 60 observations that the functional electrical stimulation has immediate and significant effect on increasing the active range of dorsal flexion motion of the wrist (p-value 0.043).
