Abstarct Analysis of the Therapy in Patients with Juvenile Idiopathic Arthritis Author: Martina Procházková Tutor: PharmDr. Josef Malý, Ph.D. Department of Social and Clinical Pharmacy, Faculty of Pharmacy in Hradec Králové, Charles University Introduction and aims: Methotrexate (MTX) is an effective and safe drug used for the treatment of juvenile idiopathic arthritis (JIA) and is well-known like a golden standart in the treatment of this serious illness. The aim of this study was analysis of therapy in patients with JIA focusing on MTX and faktors affecting its intolerance. Methods: The study was undertaken from January 2017 to February 2018 at the Department of Paediatrics and Adolescent Medicine, 1st Faculty of Medicine, Charles University in Prague. Patients younger than 18 years old, who have become ill with JIA for the first time or repeatedly between years 2015-2017 and were treated by MTX were included in this study. The prevalence of MTX intolerance was measured by "Methotrexate Intolerance Severity Score" questionaire every 3 months during regular rheumatological examinations. Bryophytes distribution modelling

The aim of this bachelor thesis is to summarize recent knowledge about Species Distribution Modelling in botany, focusing on bryophytes. Species Distribution Modelling is used to explain the relationship between species occurrences and environmental conditions of their habitats. This method has unused potential in bryophytes compared to vascular plants. The distribution of bryophytes is influenced by their dispersal and ecological characteristics. The most important factor is a close association between bryophytes and microclimatic conditions of their habitats. This association is studied, but is not yet incorporated in the modelling process along with using data in an appropriate scale. Currently there is an increased interest in bryophytes distribution modelling. In Europe, there are some studies using this method for bryophytes mainly in Iberian Peninsula, Italy and northern Europe. There are approximately 25 articles focused on bryophytes distribution modelling worldwide. This method can be used for ecological niche modelling, in biogeography and for prediction of distribution in future climate. Application of Raman spectrometry for analysis of selected mixtures

PharmDr. Hana Sklenářová, Ph.D. Title of diploma thesis: Application of Raman spectrometry for analysis of selected mixtures The identification and determination of methanol and ethanol using Raman spectroscopy was tested. For such tests two types of calibration standards were prepared. The volumes of analytes used were adjusted to get the final concentration of the alcohols corresponding to 40 volume %. In the first type of calibration varied amounts of methanol and ethanol were applied, in the second different amount of methanol was used. Measurement was carried out using portable dispersive Raman spectrometer with laser with an excitation wavelength of 785 nm. The effect of the measurement conditions to linearity of the calibration obtained in the evaluation of the signal intensity and the area of the intense band in the spectrum were tested in detail. Measurement conditions included the integration time (1000 ms, 2500 ms, 5000 ms and 7500 ms), the laser intensity (100 and 75%) and temperature (laboratory and 4-8 řC). The role of inspection in forming the policy of increasing the quality of social services in the Czech Republic.

This thesis deals with the role of inspection in shaping the policy of increasing the quality of social services in the Czech Republic from the perspective of the provider, the founder and policy makers. The policy of improving the quality intentions and direction, which should lead to the identification of existing service levels, remedy identified problems and improve performance at different levels. The first part deals with general and specific quality concept in the market sector and in social services. The evaluation systems and quality management known from the commercial sector are presented and subsequently applied to the evaluation of the quality of social services. The practical part of my attempt is to answer the question, how inspection affects improving of policy-making quality of social services. The main method used is a case studies based on semi-structured interviews. In conclusion, the outputs describing the role of inspection from the perspective of individual actors are presented. Falls as an Indicator of Quality of Care in Social Organization

The diploma thesis deals with the issue of falls among the elderly placed in residential social care facilities. The author mainly focuses on the issue whether the falls can be viewed as a quality indicator of the offered care. The author deals with the issue of falls both on the general level (legislation, standards of quality) and also on the level of a specific residential social care provider represented by home for the elderly Sue Ryder in Prague. Further, the author deals with the issue of falls in inpatient healthcare facilities, as this issue is viewed as an important indicator in terms of comparing quality of care between social care and health care facilities. The key issue of the thesis are the ways to prevent falls of the elderly, so that their basic rights (especially the right to have their own choice), independence and self- sufficiency can be maintained, and also the intervention procedures in these cases. The aim of the thesis is to provide its readers with suggestions on how to improve quality of healthcare in terms of dealing with falls. These suggestions are based on the author's thorough examination of the relevant theoretical and legislative materials. Individualization in providing diets in homes for the elderly

Diploma thesis is focused on the individualization in providing diets in three homes for the elderly in the South Bohemian region. The main goal of the qualitative research was to uncover what the options of the homes in providing the diets are, where the boundaries and the limits of the individualized care are and what the clients' attitude toward individualization is. Results of the research show that the homes for the elderly can accommodate just to the basic requirements of the client, such as replacing unpopular dish or side dish for another one. Medical restrictions of the clients in preparation and serving their food are taken into account. The homes don't have the capacity for greater individualization. Another problem is the lack of staff. The homes are limited by legislation rather than by their financial budget. Clients' conservative and conformal attitudes are not empowering managers to expand some options or to change the system. The concept of the individualization is interpreted by the homes for the elderly in different ways. In practice, they rather just try to individualize the diets, than truly doing it. The role of cholesterol-7alpha-hydroxylase in cholesterolemia regulation

Cholesterol 7α-hydroxylase (CYP7A1) is an enzyme catalyzing the first step of conversion of cholesterol to bile acids. The enzyme activity is regulated to supply enough bile salts necessary for absorption of fats in the intestine. In some species it contributes to cholesterol elimination from the body when dietary cholesterol intake is high and, in such a way, protects against the development of hypercholesterolemia. CYP7A1 activity can be therapeutically affected by administration of bile acid sequestrants that increase the enzyme activity and thus lower cholesterolemia, and also by administration of bile salts. The enzyme deficiency in humans results in hypercholesterolemia. Several single nucleotide polymorphisms were identified in gene encoding CYP7A1 in humans. They form three large haplotype blocks. The most attention has been paid to the -203A>C polymorphism that has an impact on cholesterol and lipoprotein concentrations, on the response of cholesterolemia to dietary intervention and on the response to hypolipidemic drugs. The Introduction of the Process of an Individual Planning of Social Care in a Senior's House

The theme of the diploma thesis presented is the implementation of individual planning process in the elderly homes' social care. The process is set in the legislature of the social services of the Czech Republic, which has met several complex changes. The thesis contains a strategic plan how to implement such process in an institute. In the research part there has been described how successful the selected stuff were in the individual planning process implementing. Qualitative methods of research were used to observe how stuff got through the individual planning process and how their approach and behaviour developed. Furthermore the questionnaires were used to observe the impact on users' contentment before and after the individual planning process implementing. In the research the qualitative as well as quantitative research methods were applied. The research has brought in suggestions for creation of a methodology for individual planning process in an organization.
