Application of explicite criteria of medications potentially inappropriate in the old age in the evaluation of quality of prescribing (III.)

Introduction: Propotion of older people is rapidly increasing in the world population. Elderly represents a specific, heterogenous population which is because of many reasons more vulnerable to some adverse drug events. Many explicit criteria of potentially inappropriate medication used in the elderly have been developed in order to improve the quality and safety of geriatric pharmacotherapy. These criteria represent basic tools, defining fundamental prescribing rules for the evaluation of appropriate choice and dosing of medications in the old age and for the evaluation of potentially inappropriate prescriptions in older patiens. The aim of the diploma thesis was to compare advantages and disadvantages of explicit criteria published until the end of 2015 year and to summarizea list of allpotentially inappropriate medications in the elderly (regardless of diagnoses and concomitantly used medications), applicable in the epidemiological studies of the EU COST Action 1402project and in subsequent studies. Analysis of pharmacotherapy in nursing homes

Geriatric patients of these days are typical of their multimorbidity and often concurrently use large amounts of drugs. However, their body reacts to certain drugs or groups of drugs differently than the body of younger ones due to all involutional changes that come with aging process. Polypharmacy and the use of medication that is potentially inappropriate in the elderly are a common cause of adverse drug reactions and put the patient's health at risk. Residents of nursing homes are more susceptible to multi-drug use and potential adverse drug reactions than community-dwelling seniors. This study is cross-sectional analysis of the pharmacotherapy in the nursing homes, focusing on the analysis of potentially inappropriate medications in the elderly (using STOPP criteria) and drug interactions (using databases AISLP and Micromedex). Title of Thesis: The Pharmacotherapy of Chronic Non-malignant Pain among Geriatric Patients Introduction: Epidemiological studies demonstrate that prevalence of chronic pain increases with higher age. Furthermore, pain among older patients is often underrecognized and/or undertreated. Although rational treatment of geriatric pain has been extensively studied during the past decade, many older people still suffer from uncontrolled or insufficiently treated pain. The aim of this study is to evaluate the treatment of non-malignant chronic pain by general practitioners (GPs) in geriatric patients. Further, we discuss the role of pharmacist in rational pain pharmacotherapy as well as complex pharmacotherapy management in these patients. Methods: Data were obtained by prospective assessment of 80 geriatric patients 65+ (mean age 75,0 years (range 65-94 yrs), 80 % females) suffering from chronic nonmalignant pain. Theatre Na zábradlí in the period from the leaving of Jan Grossman till the coming of Evald Schorm

This thesis deals with the Theatre Na zábradlí in the period since the leave of director Jan Grossman in 1968 until the arrival of director Evald Schorm in 1976. The work at first summarizes the social and political situation after the invasion of Warsaw Pact troops in August 1968 and put this situation into the context of culture. Then there is summarized the first ten years of the Theatre Na zábradlí and described the reason why the artistic director Jan Grossman had left.The main part is devoted to the Theatre in the years 1968-1976, and personalities which at that time worked in the theater. They were especially Jaroslav Gillar, Jaroslav Chundela and film directors - Jiří Menzel, Juraj Herz, Jiří Krejčík, Jaromil Jireš and Evald Schorm. The face of this scene in the difficult period of the Czech history is created through the contemporary reviews. The years 1976-1978 (time until the emigration of Jaroslav Chundela) are then described shortly to make complete documentation of this period.
The Analysis of Selfmedication in outpatients treated for arterial Hypertension

This thesis describes self-medication practices in a sample of 67 patients treated for arterial hypertension attending the Advisory centre of clinical pharmacology in a Teaching Hospital in Hradec Králové. We survey both common aspects of self-medication such as how often patients use over-the-counter (OTC) agents, for which health problems, which are the main groups of OTC, they use, how much they spend for OTC and where they get information about them, and more specific aspects, which are related with the fact, that these patients are at higher risk of self-medication because of their permanent antihypertensive farmacotherapy. The survey showed, that 82% of the patients with arterial hypertension use OTC - 29% of them often or constantly. However, behaviour of these patients in self-medication is not without risks. For example only 18% of patients using OTC inform their physician about all preparations. 18% of patients are not interested in safety of OTC, they use, and only 31% of them inform their pharmacist of diseases, they are treated for, and prescription medicines, they use. The survey implied, that the safe self-medication of hypertensive patients requires better education of this group of patients, which needs assistance both from physicians and from pharmacists.
Cultural Education

This thesis has the subtitle Conceptional cooperation between elementary schools and cultural institutions in Prague. It focuses not only on the theatrical field, but also on other types of institutions such as galleries, music ensembles and libraries in order to gain insight into the situation of programs for schools that contribute to the education of the youngest generation of visitors. It aims to map the situation of cultural education in Prague and to put it into the context of the Czech Republic and Europe. Further, the work examines motivation or vice versa barriers of the implementation of educational programs from the point of view of cultural institutions and schools respectively. This is done on the basis of background research, interviews and studies of the constitutional documents. The first part deals with the nuances of terminology related to the education of children in the field of arts and culture, and summarizes the conceptual documents, which work with the term cultural education. Work continues with the summary of the way-out of the current state of cultural education. There are described the most common types of programs for schools, the role of the lector in the institution and different possibilities for the implementation of such programs due to operating conditions. The second part is devoted to a comparison of outcomes from interviews with eight representatives of Prague's cultural institutions and five teachers from elementary schools. Finally the two examples of good practice from abroad are introduced as well as two examples of projects in Prague and their potential for future development.
Organizing of open-air opera performances and festivals

This thesis deals with the organizing of open-air opera festivals and performances. Attention will be given to such projects taking place in the amphitheaters or in the castle courtyards. The aim of the thesis is to pay attention to the specifics that go along with the organization of open-air opera projects. After a brief summary of the historical evolution the specifics of the theatre operation follows as well as an overview of the most important open-air opera events in the Czech Republic. The core of the work lies in the analysis of two events: The particular performances arranged by the agency Croce in Jindřichův Hradec and especially the festival Loketské kulturní léto (Loket Cultural Summer) in the region of Karlovy Vary. An attention is also paid to the history of the spaces. The work concludes with the evaluation of the position of Loket Cultural Summer between other similar projects as well as its position within the region. In conclusion, there is a proposed opera dramaturgy connected to both analyzed areas. The work also includes plenty of pictures.
