Physiotherapy in relation to functional infertility of women

Infertility can be defined as a couple's inability to conceive during one year of unprotected intercourse, at least twice a week, or it can be defined as a woman's inability to deliver a viable fetus. In the Czech Republic, 20-25% of couples are involuntarily childless. Infertility as a disease can be treated by assisted reproductive techniques and many other methods, including physiotherapy treatment. The aim of this thesis was to describe some possibilities of physiotherapy treatment in relation to the functional infertility of women, then to describe the effect on the overall condition of the musculoskeletal system, pelvic floor and fertility of the woman. Based on the stated objectives, fertility disorders and their causes have been described in the theoretical part, as well as physiotherapy methods suitable for the treatment of functional female infertility. Several methods have been described; the method of Ludmila Mojžíšová has been described in more detail. In the practical part of the work, qualitative research was used, namely case studies in the form of case reports. The research took place at the Rehabilitace Borovany s.r.o. facility near České Budějovice. Women with diagnosed functional infertility were selected for the research. The aim of this research was to observe changes in the state of the entire locomotor system, especially the pelvic floor. These patients had an overall improvement in locomotor system, an improvement in posture. The condition of the deep stabilization system muscles, especially the pelvic floor muscles, improved further, and the respiratory stereotype was corrected. All the patients felt subjectively better. One of these patients reported a pregnancy during physiotherapy treatment. This bachelor thesis is an asset for learning about functional female sterility and its treatment methods. It can also be used in the clinical practice of physiotherapists interested in this issue.
Physiotherapy in women with dysmenorrhea

This bachelor thesis deals with the reduction or even elimination of pain or any other discomfort connected with menstruation while using physiotherapeutic methods. The aim of this thesis is to describe frequent functional disorders of the locomotor system for women suffering from dysmenorrhea. The theoretical part also includes information about anatomy and physiology of deep stabilizing system and female reproductive system. The second aim of this thesis - to create a workout plan for the patients - was achieved based on the functional disorders in every patient during initial kinesiology analysis. Women performed at home the suggested exercise for five months. An appointment was arranged with each patient once in two weeks. The patients regularly got new exercises from the workout plan and were checked out during these appointments. At the end of the research a final kinesiology analysis was performed, and it was compared with the information gathered during the initial kinesiology analysis. These differences are mentioned later in discussion, together with comparison of all information acquired during the research including the sources used. All women subjectively noticed lower intensity of menstrual pain at the end of the research. Their assessments differ. This thesis can serve physiotherapists but also the public, mainly women suffering regularly with menstrual pain. It can be an option of solution of their problem in a different point of view than pharmacological.
Effect of Physiotherapy on Pelvic Floor Dysfunction

This bachelor thesis deals with a problem of pelvic floor dysfunction and the effect of a physiotherapeutic intervention on them. The pelvic floor dysfunction is a very broad range of disorders which may accompany a woman all her life or they appear in relation to women's life cycle. Whether it is a painful menstruation, sexual intercourse, urinary incontinence or other difficulties, these disorders have always a great influence on the woman's quality of life. Given the fact that it is also a very multifactorial issue, a complex therapy is needed. Each time we must consider the individuality of a female patient, the state of her body and primarily of her mind. The thesis is divided into two parts. In the theoretical part, anatomy and kinesiology of the pelvic area is described, as well as the vertebrovisceral relations, selected pelvic floor disorders, characteristic functional disorders of the musculoskeletal system, and also the present physiotherapeutic methods. The practical part is concerned with own study, in which seven female patients participated. The effect of therapy of musculoskeletal system and its influence on the above-mentioned dysfunction was tested on them. An initial kinesiological analysis was performed on each patient, which showed her current problems and conditions of her musculoskeletal system. The functional disorders, which were found, were recorded to a table, since the patient's initial and final state could be better appraised that way. All patients underwent mainly a manual therapy and were educated on correct breathing and a corrected posture. Each patient was trained and received a booklet with a set of exercises, which activate the pelvic floor muscles, to do at home. The goal of the thesis is to summarize the issue, describe various physiotherapeutic methods and their effect on pelvic floor dysfunction. This thesis can be used for education of the professional and also lay public about possibilities of the pelvic floor physiotherapy, which is nowadays in great demand, but not so widely used like other physiotherapeutic techniques of different illnesses.
Effect of physical exercise during pregnancy to diafragma pelvis

1 Abstract Title: Effect of physical exercise during pregnancy to diafragma pelvis Objectives: The aim of this thesis was to point out the influence of the appropriately chosen regular exercise during pregnancy on the state of the pelvic floor, especially after childbirth. Methods: Six selected probands, first-time mothers, aged 29-34 completed a four- month exercise plan that was specifically prepared for pregnant women. During the exercise, they communicated their feelings, and these were reported in the case reports. The pelvic floor was evaluated by an ultrasound examination after the childbirth. The ultrasound examination was performed by professional staff at the ÚPMD at 6 weeks, 6 months, and 12 months postpartum. The mothers provided me with their documentations from the individual examinations. Further, the interviews were conducted with individual probands, where anamnesis was taken using questions, and subjective feelings were ascertained not only during exercise, but also during and after the childbirth. The questions were aimed at experiences associated with the pelvic floor. Results: All probands completed the exercise program in its entirety. The influence of selected factors on the method of childbirth and perineal lacerations

This bachelor's thesis describes if selected factors influence the method of childbirth and injuries during childbirth. These factors are the mother's BMI before pregnancy, the number of births to date, mother's age and the weight of the fetus. The data in the practical part are collected from the NIS MEDEA, Department of Obstetrics and Gynecology 1st Faculty of Medicine of Charles University and General University Hospital in Prague, Apolinářská 18, Prague 2, Czech Republic. The theoretical part of the work focuses on the general anatomy of the female pelvis, and describes the types of perineal lacerations and surgical vaginal births and their risks. The comparison of the effect of methods by Kegel and Mojžíšová in patients with stress incontinence

Urinary incontinence is defined as the involuntary escape of urine that is objectively demonstrable and causes social and hygienic problems. It is a common problem, that is in its results also important medical, social and economical defect. The object of the research of this work was a group of woman patients with the stress form of urinary incontinence, that was instruated by Kegel's or Mojžíšová's methods in home exercises. The goal of this thesis was to influence contractility, power and tense of pelvic floor muscles and to evaluate the effect of the chosen methods. Objectively the results of the work don't demonstrate change of assessed parameters, but it has came to subjective improvement of urinary incontinence's symptoms and differences in kineziological investigation. Title: The effects of physiotherapy for urinary incontinence in pregnancy and especially after childbirth Abstract: The topic of my bachelor thesis is the complexity of the effects of physiotherapy for urinary incontinence in pregnancy and especially after childbirth. The thesis includes qualitative research. In the theoretical part, the anatomy of the female pelvis, incontinence problems and examination overview, the course of pregnancy and childbirth with possible negative consequences on the pelvic floor are described. Additionally the paper describes the mechanism of developing stress urinary incontinence in women and theoretically demonstrates physiotherapy for the stress incontinence. The practical part contains physiotherapeutic methodology with introduced exercise examples, which impact pelvic area. Furthermore a group of exercises suitable for women during pregnancy and after childbirth orientated on the incontinence prevention is shown. The thesis includes two casuistics, kinesiological analysis, physiotherapy description, short and long term plan and exercise results. Title: Comparison of rehabilitation approaches for female urinary incontinence at different departments in the CR. Objectives: The aim of this thesis was to determine the percentage of physiotherapists who have offices in their encounter with incontinent patients and percentage distinguish whether they are sent to a specialist or incontinence is known only by them. Determine which method of pelvic examination with stress incontinence (diagnosed and undiagnosed) is performed most often. Physiotherapy in prevention and therapy of perineal injuries and episiotomies

This bachelor's thesis summarizes the information of pelvic floor function and perineal injuries that are caused by the delivery. In the theoretical part of this thesis are described anatomical structures in the pelvic area and their changes during the pregnancy and the puerperium. The main part of the thesis is focused on possibilities of preventing the perineal injuries. There are described problems that are caused by perineal lesions and dysfunction of the pelvic floor. Options of physiotherapeutic treatment of these problems are discussed as well. The practical part of the thesis is supplemented with a case study of a pregnant patient, who was preparing to the delivery during pregnancy with the goal of preventing perineal injury. The case study is completed with anamnestic data collection and analysis of kinesiology of the patient in pregnancy and after the delivery. A therapeutic plan is a part of the case study as well. The thesis is supplemented with the results of executed questionnaire survey of 49 patients after vaginal delivery.
