Cariovascular Profile, Level of Its Control in Relation to Complications in Type 2 Diabetic Patients

1. A sample of 415 type 2 diabetic (DM2) subjects was examined. The control of cardiovascular risk factors, e.g. blood pressure, lipids and glucose metabolism, is unsatisfactory in this sample. There is a lack in nonpharmacological treatment and regimen - physical activity, smoking, low energy diet. The choice of antihypertensive treatment is satisfactory. Many patients have only the monotherapy. The antihypertensives are underdosed. We are not sure about the compliance. The treatment of dyslipidemia is not sufficient enough, there is a lack of the statin + fibrate combination therapy. The nonpharmacological regimen in needed for better glucose metabolism compensation. The pharmacotherapy should be indicated earlier (esp. metformin). 2. The prevalence of microvascular complications (mvc) was high in this sample (54%), especially the diabetic nefropathy (95%). The macrovascular compliactions (MVC) were present in 27% subjects, more in men, 55% of them had coronary heart disease. Subjects with MVC had more often mvc. The risk profile was worse in subjects with any vascular complications. The common risk factors in association with both MVC and mvc were the age over 60 years and hsCRP plasma level over 1 mg/L. Portfolio Theory

The aim of this work is to introduce Modern Portfolio Theory and its alternative Post-Modern Portfolio Theory. This work is subdivided in two parts. The first part describes the theoretical background of the Modern Portfolio Theory and the Post-Modern Portfolio Theory. A brief description of basic mathematical apparatus used in the both methods is overviewed putting an accent on different understanding and description of risk measurements. The second part of this work concerns implementation of the both portfolio theories in a case study which is focused on selection and analysis of behaviour of optimal portfolios built using the methods based on the both portfolio theories. The case study analyzes, taking into account various time series, the differences in composition of the optimal potrfolios and their performances during big turbulences in the capital markets in 2008. In conclusions the work compares the both theories and summarizes their advantages and disadvantages.

Exchange rate changes and their impact on financial management of international companies

This thesis deals with selected aspects of financial management of transnational corporations and describes the impact of foreign exchange risk on these companies. The theoretic part of this thesis explains terms like cash pooling, netting and concerns the issue of transfer prices. Furthermore the principles of foreign exchange risk, transaction, economic and translation exposure are defined with focus on financial derivatives that serve as instruments for managing and hedging the risk. The practical part depicts the case study, in which the theoretical knowledge has been applied to a chosen international company.

Prevention of Postoperative Complications in Gynecological Surgery

Prevention of posteoperative complications in gynecological surgery is very wide, very important and still very discussed theme. Postoperative complications along with complications that occur during the surgery increase both morbidity and mortality, prolong time of hospitalization and increase economic costs. These are reasons why should doctors all around the world try to prevent all these complications. In pre-operative procedure iit is necessary to know patient`s history throughly. All associated diseases and risks factors ca endanger patient during the surgery. We also have to treat and inactivate all infections focuses. Gynaecologist along with anaesthesiologist and internal medicine doctor prepare the patient before surgery. Anesthesia in gynecological surgery has it`s own difficulties and restrictions that the anaesthologist must be aware of. Every surgery is done in aseptic and antiseptic conditions. Throughough desinfection of operative field, keep the sterile conditions and uses methods of physiological surgery is essential. The most freqeunt complications are infection, bleeding and tromboemolism. The best way to prevent infection is the antibiotic prophylaxis. It`s especially useful and effective for surgeries with high risk of infection (hysterectomy, cesarean section, induced abortion)....

Biomechanics of Whiplash Injury and its Influence on Cervical Spine Structures

Title Biomechanics of whiplash injury and its influence on cervical spine structures Objectives: Objective of the work is charting of studies examining mechanism and course whiplash event, its influence on osseous and arthral textures, intervertebral discus, ligaments, neural textures, cerebro-spinal fluid and vertebral artery. In summary form mention possibilities of prevention in terms ofconstruction car. Method: Diploma work is processed in the form of commented literary background research. Results: Confrontation of individua! works brought definite results, respecting kinematics cervical spine at rear impact. Most ofin vivo and in vitro studies confirmed clear two-phase answer. Immediately after stroke (50 - 110 msec) cervical spine is forming S-form deformation with flection UCS and extension LCS, whereas it is extension with abnormal JAR. Whole spine is loaded by compression at this time. The second period is C-form deformation with extension whole cervical spine. Risk of structures injury is the highest in S- stage. The most often it is localized in LCS, and it is in intervertebral joints including capsular ligaments, intervertebral discus and anterior longitudinale ligament. Implantation of cryopreserved mitral allograft into tricuspidal position in an experiment with sheep: Technical aspects of implantation and immediate results evaluated with the aid of epicardial echocardiography

Aortic, mitral and pulmonary valve replacement is routinely performed with reasonable clinical results. But the optimal substitute for the tricuspid valve remains controversial. Contemporary prostheses either mechanical or bioprosthetic are associated with a high risk of thrombosis and/or structural failure. Mitral allografts could have advantages over existing prostheses, especially in patients with infectious endocarditis. We describe our first experience with cryopreserved mitral allograft processing and implantation into the tricuspid position in a sheep experimental model. Material and Methods: We subjected 20 sheep (age 1 year, weight 23 - 36 kilograms) to general anesthesia and a left thoracotomy. An epicardial echocardiographic study was performed and the mitral valve was harvested. Donor animals were put to death by intravenous anesthetics injection. Allografts were immersed into cultivation medium E 199 with an antibiotic cocktail. After 24 hours storage at the temperature of 37.0oC the valves were kept at + 5 to + 7 oC over a period of 3-5 days. Then they were transferred into the cryoprotective solution (E 199 with 10% dimethylsulfoxide) and sealed into plastic bags using a double-layer technique. Finally, they were programmed cooled and stored in the liquid stage of liquid nitrogen (- 196 oC)...

Sperm acrosomal reactien in selected species of mammals

Mammalian sperm must undergo the process of capacitation - series of physiological and biochemical modifications prior fertilization. In last stage of capacitation sperm undergoes acrosome reaction (AR). During AR the cell membrane of the sperm fuses with the outer acrosomal membrane and the contents of acrosomal vesicle are released into extracellular space. Sperm which did not undergo AR or sperm missing acrosome at all are unable to fertilize. AR results into dramatical changes in the sperm head. Most of the proteins present in plasmatic and outer acrosomal membrane are reorganized or lost. There are also significant changes in cytoskeletal and intraacrosomal proteins are released to extracellular space uncovering new surface domains. Some sperms undergo AR even without presence of inductor of AR during capacitation in vitro. This event is called spontaneous (accelerated) AR. The latest research indicates that spontaneous AR is natural part of the process of fertilization. Field mice (Apodemus) show high level of promiscuity leading to significant risk of sperm competition. Diplomová práce se zabývá problematikou event marketingu. Kromě charakteristiky a vymezení pojmu event marketing pojednává o event marketingu v kontextu marketingového a komunikačního mixu. Dále je diskutována problematika měření úspěšnosti event marketingu. Stěžejní část práce tvoří šestá kapitola, jež se zabývá novými nástroji marketingové komunikace, analyzuje jejich pozitiva a negativa a provádí komparaci a jejich vymezení vůči event marketingu. Následuje část věnovaná deskripci trendů v event marketingu a jejich extrapolací v dlouhodobém časovém horizontu. Práce je doplněna případovou studií, jež analyzuje event marketingovou kampaň společnosti Volvo. Závěr je věnován shrnutí nejdůležitějších poznatků a možnostem jejich aplikovatelnosti v prostředí České Republiky.

It is also prepared itemized budget and construction details. The basis for this diploma thesis was design documentation.

The financial impact of monetary factors in the selected company

This dissertation evaluates development of exchange rates and its specific effects on STAP company a.s. The aim of this paper is to present the events that have had influence on the development of the exchange rate between Euro and Czech Crown and precautionary measures taken by STAP a.s. to prevent related risks. The first part comprises a summary of events that had a significant impact on the exchange rate development; the risks created by these events and methods devised to prevent these risks. The second part evaluates the specific financial derivatives used by STAP a.s. and their impact. Finally the recommendation is made for the future more effective usage of the financial instruments.
