Vaccination in children
Truhlářová, Petra ; Jílek, Petr (advisor) ; Vejsová, Marcela (referee)
Author: Petra Truhlarova Name: Vaccination in childhood Faculty of Pharmacy in Hradec Králové Field of study: medical laboratory technician Background: One of the most important goals of my thesis is to point out a current lack of information on vaccination of our population. This fact stems from unawareness of a significant progress achieved in the field of immunology in recent decades. Main findings: My bachelor thesis deals with vaccination in an early childhood. I tried to create an overview of compulsory as well as voluntary vaccination in certain periods of childhood and I also mention immunological aspects of vaccination. I briefly characterize selected contagious diseases and I present options of their prevention by utilizing vaccination. Furthermore, I present some child practitioners' opinions of vaccination as such. The thesis provides a general overview and comparison of history of vaccination in the Czech Republic and in the world in general. Conclusions: The greatest benefit of vaccination lies in a creation of a vaccination scheduler for compulsory and above-standard vaccination, further focus on the opponents of vaccination, an effort to explain the principles of vaccination, the issue of the immune system response to vaccination, a combined vaccine application etc.

Další část pak vymezuje základní terminologii a kontext vztažený k pohlavní identitě a...

Impact of the Gastrointestinal System on Metabolic Control in Children with Type 1 Diabetes Mellitus - Gut Peptide Hormones
Huml, Michal ; Sýkora, Josef (advisor) ; Dědek, Petr (referee) ; Rušavý, Zdeněk (referee)
Our objective was to evaluate plasma levels of gut hormones and hormones associated with glucose metabolism in children with type 1 diabetes mellitus (T1DM), and to correlate plasma concentrations of gut hormones with blood biochemistry, markers of metabolic control and anthropometric parameters. A multiplex assay kit (LINCOplex® ) was used for the determina- tion of postprandial plasma levels of specific gut peptide hormones. Amylin, glucose- dependent insulinotropic polypeptide (GIP), active glucagon-like peptide 1 (GLP-1), active ghrelin, insulin, leptin, pancreatic polypeptide (PP), and polypeptide YY (PYY) were assessed prospectively in 55 subjects including 19 T1DM children (mean age: 13.4 years) and 21 healthy reference controls (mean age: 13. 4 years) and 15 patients with functional abdominal pain (FAP) (mean age: 10.5 years). In total, 440 plasma hormones samples were assessed in 55 patients. Entered data were examined using a non-parametric Wilcoxon's test. Further- more, statistically significant correlations were assessed by stepwise regression analysis. Our study demonstrated that the determination of specific postprandial gut hormones with the multiplex assay kit (LINCOplex® ) was highly efficient. Not only was a small amount of plasma sample (25µl) required for analysis, but also a...

The study of molecular and biochemical bases of cytochrome c oxidase deficiency
Veselá, Kateřina ; Zeman, Jiří (advisor) ; Farghali, Hassan (referee) ; Drahota, Zdeněk (referee)
This thesis has been worked out in The laboratory for study of mitochondrial disorders (Department of Pediatrics, 1st faculty of Medicine, Charles Univezity in Praha), which serves as the diagnostic center for patients from Czech and Slovak Republics. During the last years, more than 40 children with isolated COX deficiency were diagnosed in our lab. But molecular background except 12 patients with mutations in SURF1 gene was remaining unknown. Due to the lack of adequate treatment for these patients, the genetic counseling and the possibility of prenatal diagnostics have high importance for the families. The possible dual origin of the defect with different hereditary aspects makes the genetic counseling in the affected families complicated and prenatal diagnostics based only on biochemical analyses very problematical if even possible. This work had been arisen basically from the necessity to find the molecular background of isolated COX deficiency in our patients. In addition of simple characterization of molecular background and optimalization of methods for routine diagnostics, we were able to study the impact of several mutations in nuclear genes for COX assembly factors on the biochemical, structural and histochemical level in affected tissues.

The aim of our study was to evaluate the severity of hepatic and kidney damage retrospectively in the years between 2000 and 2004 with a focus on their reversibility, and to analyse the prognostic factors following Amanita phalloides (Am. ph., mainly hepatotoxic agent) and ethylene glycol (EG, nephrotoxic liquid) based on the calls made to the Czech Toxicological Information Centre and analyses made by toxicological laboratories. Analysis relied on data from medical records. A variety of clinical and laboratory parameters were collected. Fisher's test, Student's t-test and the calculation of linear correlation coefficients were used for statistical analysis. Twenty-four of 206 case-patients with EG intoxication were children. Fifty of 158 adult patients who survived, developed signs of nephrotoxicity. In 19 patients renal function normalized up to one month following the intoxication. Total 21 patients were followed-up as out-patients. Renal function completely recovered in 13 patients during 6 months and in three patients during 8-24 moths following the intoxication. The serum creatinine level was only mild elevated (119 |imol/l) in other two patients. However, their follow-up finished. In three patients renal damage persisted, serum creatinine level 138-200 (imol/1, in 19-26 months after discharge from...

Diagnosis and treatment of the injuries to the permanent teeth
Hecová, Hana ; Kilian, Jan (advisor) ; Nováková, Květoslava (referee) ; Gojišová, Eva (referee)
Injuries of the permanent teeth represent extensive and serious problems of dentistry. With the exception of the hard dental tissues, gingiva, periodoncium, dental pulp and alveolar bone are offen affected. The healing of injured teeth depends upon different factors, such as the stage of root development, the presence of bacterial infection in oral cavity and the possible penetration of bacteria or their toxins through the dentinal wall. These factors as well as incorrect treatment can cause posttraumatic complication, which appear immediately after dental trauma or up to several years lato. In this study the evaluation of 384 patients with injuries of the permanent teeth was performed. All these patients were treated in the Dentistry Department of the Faculty Hospital in Pilsen. The aim of the clinical part of the study was the analysis of the epidemiologic data and of the occurrence of posttraumatic consequences in every group of dental injuries. The investigated dates were compared with the dates in the literature. The relation between patient' age, stage of root development, treatment of dental trauma and occurrences of posttraumatic complications has been investigated. The main findings of this study agree with the data in the literature. Dental injuries occur more frequently in children at the...

MUDr. JEHLIČKA SUMMARY This study has focused on a new method for the assessment of endothelial dysfunction (ED) using the Deceleration Index (DI). A total of 242 ultrasonographic examinations were performed in 92 children (FH 32, DM1 30, healthy controls 3O). The average DI values were 21.8%(±9.4)in healthy children, 19.9%(±9)in FH and 17.6%(±14)in the DM1 group. There were statistically insignificant differences between the study groups. This was most probably due to the high biological variability of the method employed. The coefficient of variation of DI was 1.02 (0.68 -1.53). Therefore, the clinical usefulness of DI is limited with regards to the individual assessment of ED and thus for the prediction of cardiovascular risk. The association of ADMA and oxLDL with ED and their superior sensitivity compared to DI and FMD has been confirmed in our study. Folic acid supplementation did not lead to a statistically signifiant improvement of DI and FMD. Similarly, plasma levels of ADMA and oxLDL remained unchanged despite a decrease of homocysteine and hsCRP in these children. This finding demonstrates a complicated relationship between ADMA, homocysteine and folic acid. The protective effect of folic acid on ED in high risk children was not confirmed in this study.
