Role of the treatment verification system and verification generally in modern radiotherapy
TÁBORSKÁ, Hana
Radiotherapy is a complex process which involves the verification system, often underestimated by the user. The verification system can be used not only for the planning process of the patient{\crq}s exposure, but also for its setting and even for the setting of the irradiation device itself. Additionally, the verification system can set correctly the dose applied. The consistent verification both from the side of the radiological assistant and from the side of the software used is very important with respect to the patient safety, especially at present day, when new irradiation technologies are introduced and high curative doses are applied. Even a small mistake can cause fatal consequences. For this reason the verification system particularly operates independently without external interventions. It is the database based on the client/server information technology. In my bachelor thesis I should describe the classification, significance and necessity of the usage of the verification system and the verification procedures in the modern radiotherapy. The part of my bachelor thesis is also the statistical processing of offsets in the patient{\crq}s positions using the portal imaging and the accuracy of the dose applied using in-vivo dosimetry.

Introduction: Stress urinary incontinence is a common problem which affects a high percentage of women. Anti-incontinent surgery has played an important role in the treatment of stress urinary incontinence. Burch colposuspension was considered a gold standard in anti-incontinence surgery, with good long-term results. However, over the past decade this procedure has been largely replaced by the application of various types of TVTs (tensionfree vaginal tapes). In our study we have focused on comparing developments in the quality of life and the occurrence of complications in a long-term follow-up after TVT, TVT O and colposuspension. For subjective assessment of urinary incontinence we have chosen a validated specialized Incontinence Quality of Life questionnaire (I-QoL). The aim of our study was to monitor the trends with quality of life after TVT, TVT O and colposuspension procedures and to establish whether the occurrence of complications affects the quality of life (QoL) and compare of the quality of life among anti-incontinence procedures and longterm subjective and objective outcome. Methods: The study included 215 women who underwent a TVT, TVT O or Burch procedure between 1.6.2002 - 31.5.2005. Women were completely urogynecologic examined and urodynamic stress incontinence was documented....

Effect of pentoxifyllin on advanced dementia with predominated cardiovascular symptoms
Mészárosová, Jitka ; Ďoubal, Stanislav (advisor) ; Náhlovský, Jiří (referee) ; Mrňa, Boris (referee)
The high consumption of Pentoxifyllin in patients suffering from vascular dementia led to a concern and a clinical trial conducted in the nursing home in Česká Kamenice from 2001 to 2004. The retrospective survey was carried out on 176 patients and lasted 12 months. If we consider the currently valid criteria for the diagnosis of vascular dementia, the patients showed an advanced form of the disease. The experiment did not demonstrate any positive influence of treatment on either cardiovascular diseases or the mortality of afflicted patients. Despite the fact that we could expect the medicament to have a favourable influence on the symptoms of the advanced form of vascular dementia it was not proven so. It is absolutely essential for doctors to have good background knowledge of diseases and all kinds of particularities accompanying old age, especially when it comes to the early detection of dementia. High consumption of Pentoxifyllin led to a number of experiments and studies trying to prove its efficiency and hence justify prescripcions for the drug. An extended study carried out in 1996 demonstrated the favourable effects of Pentoxifyllin. Our experiment has also showed signs of improvement in pateints with weaker forms of dementia. Final decision on the suitability of the treatment with...

Prevention and treatment of pressure ulcers in nursing Home for elderly Milevsko
HRYCHOVÁ, Šárka
Background methods: Appearance of decubitus is a main indicator of therapeutic and nursing care quality and is monitored carefully. However, they supervence in despite of high-quality and professional care of nursing staff, because there are factors, which we cannot influence while decubitus emergence. The main purpose of research was to map the preventative and nursing care of clients with the risk of decubitus and with it, find out if the care is in high-quality and possibly debunk absences and deduce a treatment for work experience.Methodics: The research was done by quantitative method, by technique of secondary analysis of data. Analytic studies check if there is a relation among the watched features. The data was received from verified and enclosed nursing documentation of Home for senior citizens in Milevsko from years 2010 and 2011. Procured data was afterwards elaborated by descriptive statistics by Microsoft Excel and SPSS program. Validity of hypotheses was tested via Chí quadrate test. Research aggregate created nursing documentation of Home for senior citizens in Milevsko. This establishment has 36 beds and during the year 2010 and 2011, 85 clients were accomodated here. Consistent with the first research purpose "To map the high-risk factors, which increases the probability of decubitus emergence." was evaluated nursing documentation of 85 clients. Consistent with the second research purpose "To find out, the precautionary measures belittling the risk of decubitus emergence of vulnerability client group." was evaluated by nursing documentation of 54 clients. Consistent with the third research purpose "To map the nursing intervention of clients with decubitus." was evaluated by nursing documentation of 10 clients. Results: From the research, results arose that there was the risk of decubitus emergence at every client of Home for senior citizens in Milevsko according to directory evaluative Norton scale. The final results demonstrated a high percentage 63,4 % clients who were endangered by the decubitus emergence. After one month, a checking evaluation of the risk of decubitus emergence was made according to directory evaluative Norton scale. The results demonstrated the strong depression of the number of clients with the risk of decubitus emergence or at least the depression of risk. The last part of research was focused on watching the nursing intervention of the client group with decubitus and finding the fruitfulness of the decubitus treatment. The result demonstrated that the treatment was in most cases succesfull and that 80% of clients were healed in four month. Conclusion: The important finding for the nursing staff was the assertion that the provided nursing care in Home for senior citizens in Milevsko has a good standard. Simultaneously, some absences were found like deficient resource of nutritive accessoires, damp healing of wounds, and utilities against decubitus in standard goings. The solving of these absences is to increase accessibility and awareness of professional doctors for resource of the necessary amount of nutritive accessoires, damp healing of wounds, and not to let clients with deficient financial resources to be entirely dependent on out of date medical advancement. Also, not to corrupt treatment and precautionary measures. The next important step is to motivate family members and imply them to take care of their relatives. Factors afecting compliance and adherence to treatment in patients with HIV
Braun, Richard ; Šípek, Jiří (advisor) ; Procházka, Ivo (referee) ; Štech, Stanislav (referee)
The thesis examines factors that influence, directly or indirectly, the adherence and compliance to treatment among HIV/AIDS patients. HAART ensures long-term therapeutic effects and high quality of life, as long as the patients comply with the recommendations of their physicians and adhere to the prescribed medication regimens. In case of suboptimal adherence, the patients are under risk of developing resistance to the particular medication or even an entire class of ARV drugs. Still, there is a substantial number of patients who fail to adhere and to take their medication consistently, risking thereby not only drug resistance but a severe deterioration in their health status as well. The thesis uses qualitative methodology to explore both barriers and facilitators of adherence among HIV/AIDS patients in the Czech Republic. Such a research on Czech population has not been carried out as yet. The results are compared with the outcomes of foreign studies. Mills meta-analysis (2006) of 84 published research papers dealing with the subject of adherence among HIV/AIDS patients served as the basis for comparison. Key words: adherence, compliance, HAART, resistence, barriers to adherence, facilitators to adherence

In the thesis the author deals with the surgical technique of the genioglossus advancement and hyoid myotomy (GAHM) in the obstructive sleep apnoea syndrome (OSAS) therapy. He focuses in particular on methods used to widen the upper airway by surgical modification of facial bones. Following the definition of the research objective in Chapter 1 and the general introduction in Chapter 2, we can find in Chapter 3 the description and definition of OSAS as well as physiological, pathophysiological and epidemiological notes on sleep, ventilation in sleep and OSAS. In Chapter 4 the author recommends some new methods of examining the upper airway that can be applied in an ordinary dental surgery to predict the risk of development or occurrence of OSAS in some patients. Chapter 5 includes an overview of therapeutic options for OSAS and a detailed description of different GAHM techniques. In accordance with the research objectives and the methodology described in Chapter 6, the results in Chapter 7 show, for a large sample of examined patients, the dependence of obesity (as expressed by the BMI) on the severity of OSAS, along with some cephalometric values in which such dependence has not been published so far. Then the chapter documents a high success rate of GAHM treatment of OSAS in operated patients. This is so,...

Šerá, Božena ; Gajdová, Iveta ; Čermák, M. ; Gavril, B. ; Hnatiuc, E. ; Kováčik, D. ; Kříha, V. ; Sláma, J. ; Šerý, M. ; Špatenka, P.
Germination and early growth of buckwheat (Fagopyrum aesculentum) after low-temperature plasma discharge treatment in air gas generated in various apparatus were tested. Three pretreatment times were used: 180 s, 300 s and 600 s. Number of germinating achene, length and weight of sprouts were measured. The data was analysed with two-way ANOVA. A significant influence of the type of plasma apparatus, time of exposure and combination of both factors were found. The positive effect on germination and early growth was observed after application of GlidArc plasma. The opposite effect of PDBV apparatus, which is characterized by plasma with a high power density, was observed. Shorter pre-treatment times for this apparatus must be therefore used.
