The Abstr act Title of thesis: The return of a person after lower limb amputation into society. Aim of work: Determine how the patient perceives care in terms of physiotherapy and psychology at various stages:  hase I. - prior to rehabilitation  hase II. - At the end of rehabilitation  hase III. - At intervals after the return back home Metho d: The task is concieved as a methodological study is concerned with the retrurn of a person into society after lower limb amputation. The processing is based on a method of observation of patients in a rehabilitation clinic. The method was further extended by evaluating feedback from patients. The method of auto-detection, which is based on assesment, communication with the patient and practical daily diagnosis of the progress of rehabilitation is combined with a method of questionnaire to verify the feedback from the patient. The aim of the feedback is to provide personal information on their own, individual - specific perspective of each individual patient "That would be reliable". Title: EN: Case study of physiotherapy treatment of a patient with Psoriatic Arthritis after Total hip arthroplasty. CZ: Případová studie fyzioterapie léčby pacienta s psoriatické artritidy po totální endoprotézy kyčelního kloubu. Thesis aim: In this thesis I will aim to discuss the rehabilitation after a total hip arthroplasty in a patient with psoriatic arthritis. The thesis is divided into two parts, one theoretical where the aim will be to explain some of the current research concerning psoriatic arthritis and total hip replacement. It will be focused on the anatomical, bio-mechanical and kinesiological properties of the lower extremities as well as the surgical procedure of total hip arthroplasty and following rehabilitation. I will also briefly look at the epidemiology and etiology of psoriatic arthritis and associated symptoms. The second part will focus on a 67 year old woman with the mentioned diagnosis in the post-operative state 2 weeks after surgery. The practical part aim to describe examination, therapeutical approach and conclusions after one week of examination and therapy. 4

Biochemical and molecular studies of cytochrome c oxidase and ATP synthase deficiencies

The mammalian organism fully depends on the oxidative phosphorylation system (OXPHOS) as the major energy (ATP) producer of the cell. Disturbances of OXPHOS may be caused by mutations in either mitochondrial DNA (mtDNA) or nuclear DNA. One part of the thesis is focused on the role of early and late assembled nuclearencoded structural subunits of cytochrome c oxidase (CcO) as well as Oxa1l, the human homologue of the yeast mitochondrial Oxa1 translocase, in the biogenesis and function of the human CcO complex using stable RNA interference of COX4, COX5A, COX6A1 and OXA1L, as well as expression of epitope-tagged Cox6a, Cox7a and Cox7b, in HEK (human embryonic kidney)-293 cells. Our results indicate that, whereas nuclear- encoded CcO subunits Cox4 and Cox5a are required for the assembly of the functional CcO complex, the Cox6a subunit is required for the overall stability of the holoenzyme. In OXA1L knockdown HEK-293 cells, intriguingly, CcO activity and holoenzyme content were unaffected, although the inactivation of OXA1 in yeast was shown to cause complete absence of CcO activity. The Carotid Stump Syndrome

Introduction: Internal Carotid Stump Syndrome may be one of the possible causes of ischemic stroke (iCMP), as well as retinal infarction (RI). Syndrome of the occluded internal carotid (ACI) (stump syndrome) is a nosological unit, which is characterized by an onset of ipsilateral iCMP or RI of thromboembolic ethiology in patients with occlusion of ACI via the externa carotid or arteria ophthalmica. In my thesis, I have concentrated upon the specification of the stump syndrome, its diagnostics and treatment; furthermore I have assessed the appropriateness of surgical approach in comparison with conservative approach. Material and methods: A total of 621 patients with occlusion were treated in two centres in the course of five years. In a group of 40 patients, the ACI occlusion was detected sonographically, the length of the occluded ACI being >5 mm, with normal vasoreactivity based upon SPECT CO2 and excluded cardiogenic cause of iCMP. The patients were divided in two groups - surgical and conservative. Patients were monitored in 6-month intervals for the total period of four years. Results: No RI or iCMP were detected in the surgical group; one patient died six months after surgery. We observed one case of amaurosis fugax in the conservative group. The topic of this thesis is about subcapital fractures, thier treatment and complications. The work is divided into two main sections. The first part deals with teoretical findings of the topic. This work consists of two parts, general and special. On the special one was focused more attention. In the general part we can learn basic information about anatomy, biomechanics and kinesiology of the knee. There is described the mechanism of injury of a soft knee, methods of examination and diagnosis, treatment of LCA rupture with conservative and surgical treatment and rehabilitation before and after a surgery. The special part gives a detailed description of the case of study of a patient after ACL rupture. The study has been made during the continuous professional experience, which I undertook at the end of January and February in the Regional Hospital Kladno.

Left ventricle remodeling in patients with primary aldosteronism and essential hypertension

Myocardial damage is one of the most serious consequences of arterial hypertension. Changes in the heart structure and function develop not only due to pressure overload itself, but many other hemodynamic and neurohumoral factors contribute to their formation. Our work has compared echocardiohraphic strucutural anf functional changes of the left ventricle, caused by essential hypertension and hypertension associated with primary aldosteronism (PA) as the most common reason for secondary hypertension. The first part of our work focused on the differences in left ventricle geometry in men with PA and essential hypertension after separating it's low-renin form (where, similarly to PA, the plasma volume expansion was considered to have the dominant effect on left ventricle remodelation). In men with low-renin forms of hypertension including PA, we observed greater both endsystolic and enddiastolic diameter of the left ventricle, lower relative wall thickness and more frequent eccentric type of hypertrophy when compared to essential hypertensives with normal renin levels. Whereas left ventricle cavity diameters were positively correlated to aldosterone levels, wall thicknesses were associated mainly with hypertension severity expressed as an average 24hour blood pressure and number of antihypertensives....

Title: Case Study of Physiotherapy Treatment of a Patient with Total Shoulder Arthroplasty Aim: The aim of this work is to characterize the problem of the total shoulder arthroplasty specially reverse type and to focus the physiotherapy procedure for the patients with this diagnosis. The work contains the detailed case report of the patient with reverse total shoulder arhtroplasty. Methods: The theorical part is made into the literary research. The special part contains the case study of the patient with reverse total shoulder arthroplasty and it proceeds from the practical work with this patient during the continuous practice in the clinic of rehabilitation Malvazinky, Medditera s.r.o. The entrance and the output of kineziological examination, short-term and long-term plan of the therapy, the method of the course of each therapy are included in this dissertation. Molecular evaluation of novel BCR/ABL kinase domain variants in patients with chronic myeloid leukemia

1 Abstract BCR/ABL is a constitutively active tyrosine kinase that has been shown to be at the heart of the development of chronic myeloid leukemia (CML) and about 30% of acute lymphoblastic leukemia (ALL). With the recent advent of tyrosine kinase inhibitors (TKIs), exemplified by Imatinib, Nilotinib, Dasatinib and Bosutinib, patients with Ph+ CML or ALL are candidates for the therapy with these agents. From the available TKIs, Imatinib is considered as front-line therapy for CML patients in chronic phase, while for Ph+ ALL patients, 2nd generation TKIs (nilotinib, dasatinib, bosutinib) might be considered as more effective therapeutic option. Since the treatment with TKIs is a long-term affair, a substantial proportion of patients acquire some sort of mutation in kinase domain of BCR- ABL, which could be a reason of treatment failure. To date, over ninety BCR/ABL kinase domain mutations have been identified, affecting over 50 amino acids. Recurrent BCR/ABL kinase domain mutations have already been in vitro tested to approximate for their in vivo behavior. Our goal is to invent in vitro technique that would allow testing TKI sensitivity of novel BCR/ABL kinase domain mutations, identified at very low MRD levels.
