The Approach of Czech Health System to Psychosomatic Medicine in Comparison with International Discourses
Wolfová, Magdaléna ; Háva, Petr (advisor) ; Kotrusová, Miriam (referee)
The research problem in this thesis is the quality of treatment of Czech patients suffering from somatoform disorders. The goal is to evaluate the state of scientific discourse of Czech specialists in Psychosomatics, compared with the international situation as a potential starting point of further institutional change, including its legitimacy (interviews with citizens). Methods were used: (1) critical discourse analysis and (2) semi-structured interview. Findings: The existence of barriers between representatives of the various professional disciplines prevents the development of Czech scientific discourse of Psychosomatics, combined with the applied reform changes focused on individualism and competition. Czech Republic unsufficiently utilizes the knowledge support from international organizations (WHO, OECD, EU) in the field of mental health policy. The formation of Czech health policy is not compliant with human rights related to health and is more focused on the question of financing than on patients' health. Citizens perceive deficits in psychosomatic approach and try to substitute with complementary and alternative forms of medicine.

Přibylová, Kristýna ; Háva, Petr (advisor) ; Mašková, Pavla (referee)
This work deals with effectiveness of coeliac disease management. It is a model case of chronic disease management. The analytical framework of this work is the multilevel Innovative Care for Chronic Conditions Framework. Crucial is the interaction between health care team that represents health care system, community partners and patients with their families. In the case of coeliac disease it is necessary to include also the role of representatives of market with gluten free products. The interaction of these participants takes place in wider political context, whose state influences this interaction fundamentaly and reflects so in final results - state of patients and quality of their live. Using the evaluation of the quality of life tool SF-36, it has been ascertained, that the quality of life of coealiac patients is lower in comparison with European standard of healthy population and that the causes of the problem have to be searched and systematically removed at all levels of the system of chronic health care. The findings validate the theoretic presumption, that the chronic health care is still considerably underestimated and that it does not reply to the needs of chronic ill patients, which reflects just on general quality of life.

Šimandlová, Nikola ; Mašková, Pavla (advisor) ; Tušková, Eva (referee)
This thesis is concerned with the Czech doctor's campaign "Thanks, we are leaving" on the background of the health care system in the Czech Republic. The campaign started in 2010 by the Czech doctors trying to focus on the working conditions, salary conditions, educational system and some failures of the health care system with the aim to improve it. The campaign resulted in February 2011 in a compromise between doctors and Ministry of Health. This thesis focuses especially on media and on the interest group LOK (Medical union trade club) which set the agenda. The perception of the campaign is ambiguous both for the public and for the doctors themselves. The individual milestones of campaign are explained by the theory of punctuated equilibrium from the authors Bryan D. Jones and Frank R. Baumgartner. Using many helpful methods such as content analysis of media messages, semi- structured interviews with particular actors, analysis of secondary sources, stakeholder analysis or analysis of selected events in health policy I explained the core events and actors who participated in this campaign. The theoretical concepts used in this thesis are: public policy in its multidisciplinary meaning, health policy and health care system, punctuated equilibrium theory, theories concerned with interest groups...

Šanc, Filip ; Novotný, Josef (advisor) ; Matoušek, Roman (referee)
This diploma thesis explores the emergence of the welfare state in developing countries, in particular shown on the example of Botswana, Ghana and India. The inquiry is focused on the period beginning in 1990, when the neoliberal paradigm was dominating, untill 2010. The recent years are in token of the shift from the neoliberalism to the post-neoliberalism characterized by a number of concepts, which are taking into account. The common feature of these concepts is the diversion from the narrow focus on GDP, as the only indicator of the growth, to the social dimension of the development. This shift is also being distinguished as a transition from the basic-needs concept to the rights-based approach. Therefore, the thesis explores, if these shifts are remarkable in the analyzed countries, eventually, if there are any divergences as compared to the theoretical concepts. To achieve this goal, a broader analysis of the welfare state was used, which involves social, health and education policy. Based on this analysis, the diploma thesis tries to classify the analyzed countries into the welfare state typology; eventually, in case such classification is impossible, it describes the weaknesses of this welfare state the typology. The theoretical part is devoted to the wider context of the problem of trust in the health care system. This section is trying to describe the changes in society which has been changing relationship of a doctor and patient. Most significant changes that affect this relationship are represented by the development of information society, as well as commercial and system pressure on the doctor. It also highlighted the importance there is the of human rights related to health and patient rights. The empirical part focuses on the research results and their discussion. The research method was applied in the form of qualitative interviews, which were then rewritten and clarified schematically using tables. Research results did not confirm the assumption that citizens lose trust in medicine as a science (lege artis). Problems of the health system respondents associate more with its wrong settings, which can be blamed for the Ministry of Health. Citizens were fairly unsatisfied with the current political situation and this is surely reflected the results of the decreased trust in state institutions.
