The title of the bachelor's thesis: Case study of physiotherapy treatment of the patient after operation of lumbar spine for L4/L5 disc prolapse Summary: This bachelor thesis is divided in two parts, for general and practical part. The aim of general part is getting theoretical basics for diagnosis of disc prolapse L4/L5. The general part is contains of basics of the spine anatomy and intervertebral disc and kinesiology of the lumbar spine. This thesis continue with description intervertebral disc herniation, etiology, diagnosis and teratment of intervertebral disc herniation. The aim of practical part is to elaborate case of study of physiotherapy treatment of the patient after operation of lumbar spine for L4/L5 disc prolapse. The special part includes anamnesis of patiens, in-going kinesiology analysis, assesment of the physioterapeutic plan, description of the every sing therapeutic unit and outgoing kinesiology analysis. The conclusion of thesis is focused for long-term rehabilitation plan and whole effect of therapy. Title: Comparison of the quality of life of persons diagnosed with intervertebral disc herniation in relation to surgical and conservative treatment. Objectives: The aim of the diploma thesis was the research and evaluation of the quality of life in patients with intervertebral disc lesion with and without surgery, assuming that both monitored groups will be treated with individual rehabilitation treatment. Methods: This research was conducted using a standardized Short Form 36 (SF - 36) questionnaire on quality of life that is conditional to health. The study was conducted at Na Homolce Hospital in a rehabilitation clinic, where appropriate respondents (n = 117) were selected for this research. The group included 59 respondents who underwent surgery and 58 treated conservatively. The survey was attended by respondents aged 27-86. The final results were divided into both health, physical and psychological dimensions in both groups. Two-choice tests were used to compare. For all domains, the assumption of the normal distribution of the two compared selections was rejected, which precluded the use of the two-t-test. Title: Comparison of the quality of life of persons diagnosed with intervertebral disc herniation in relation to surgical and conservative treatment. Objectives: The aim of the diploma thesis was the research and evaluation of the quality of life in patients with intervertebral disc lesion with and without surgery, assuming that both monitored groups will be treated with individual rehabilitation treatment. Methods: This research was carried out using a standardized Short Form 36 (SF - 36) questionnaire on quality of life based on health. The Czech version used corresponds to MUDr. Petra, Ph.D. (2000). The study was conducted at the Na Homolce hospital at the rehabilitation clinic, where appropriate respondents (n = 170) were selected for this research. The final results were divided into the respective age groups in both groups. Subsequently, a statistically significant deviation between the respective files was verified in each age group and therefore it can be said that there are no significant differences between the analyzed files. Author: Bc. Veronika Valášková Title of the work: The effect of physiotherapeutic procedures on cervical spine surgery. Aim: The aim of my thesis is to give an overview of the physiotherapy treatment after surgery of herniated cervical intervertebral disc. Methods: This thesis is written as a review of literature sources. The thesis consists of three parts, the first of which highlights the issue of kinesiology, biomechanics, stability of the cervical spine, pathophysiology of a prolapsed spinal disc and imaging issues involved in the discovery of a the actual prolapse. The second part deals with the various surgical techniques and associated complications. The final theoretical part looks into physiotherapy treatment after the operation. I examine the chronology of physiotherapy after the operation, and ascertain if there is a necessity to apply a neck brace after the surgery. It is important to answer the question of when it is possible to return to work and when the patient can return to their original physical activities. Results: The results of this thesis shows that postoperative care in individual departments vary and is inadequately described within available resources. Author: Michala Paseleková Work title: Casuistry of physiotherapeutic treatment of a patient recovering from intervertebral disc prolapse surgery The aim of this thesis is to make a comprehensive view on the surgery of intervertebral disc L4/L5 prolapse, follow-up rehabilitation, and also to familiarize with the theoretical knowledge of this issue. The presented data were gained from the contiguous practical training experience in Faculty Hospital Královské Vinohrady in Prague at the Department of Rehabilitation Medicine, where I processed the casuistry of a patient with this disorder. The practice took place under the expert guidance of Eva Konečná and Mgr. Šimon Biskup during the period from 24.1.2011 to 18.2.2011. I used the processing of case study dealing with the surgery of intervertebral disc L4/L5 prolapse, and matters related to this issue. The result is a clarification of the operational solution of intervertebral disc prolapse and problems associated with this disorder, including follow-up rehabilitation treatment. Rehabilitation of patients with prolapsed intervertebral disc in lumbar spine during surgical therapy and conservative therapy

TThe title of the thesis is The rehabilitation of patients with the slip of intervertebral disc in the region of the lumbar spine in surgical and in conservative therapies. A lumbar disc herniation is one of the most frequent degenerative changes in the spine. It is also one of the most common diagnoses in neurology and neurosurgery indicated for surgical treatment. This is an alarming finding and therefore it must be pursued in depth. The first part is only theoretical, containing the anatomy and kinesiology of spine, the characteristics, division and diagnosis of the slip, and the description of the conservative and surgical therapies. The second part is devoted to the research, with two groups of respondents. In the first group there are three patients who underwent my five weeks? intensive rehabilitation plan. We met once every week and they did the exercises every day on their own. The first meeting was related to a complete examination and exercise education. The last meeting again included a thorough examination, and a long-term rehabilitation plan suggestion. The short-term rehab plan was mainly composed of pain relief components, searching for the relief positions. Also, we worked out the deep stabilization system, the abdominal muscles involvement, pelvic exercises, spasm loosening, sacroiliac joint relaxation, sensomotoric exercises, root stretching and kinesiotaping. Each patient was also familiarized with the secondary prevention and the contraindications for this illness. The second group are two patients who underwent a surgery at the neurosurgical department of the hospital in České Budějovice. Subsequently, they rehabilitated at the rehab department of the same hospital. This group received a complete care of the department ? the examination, the treatment, physical therapy application and file records. Their results were compared to the first group performance. The components of the therapy were: group fitness exercises, individual fitness exercises, group specific exercises (Flendkrais method elements, McKenzie, pelvic exercises), soft techniques for the scar, swimming pool fitness exercises. The aim was to describe the differences between the conservative and surgical therapies. The purpose of the practical part was to prepare and implement an intensive rehabilitation plan for particular patients, who were not indicated for the surgery, and compare the results with the patients after the surgery. A qualitative research was used for the purpose for the comparison. To gather the data, I used the following techniques: interview, anamnesis, entry and final kinesiological analysis, medical record consultations, photographing, and secondary data analysis. The field research is concluded with the result evaluation. The results show that the kinesilogical analysis was highly functional in two cases, and helped the patients. In one case it helped only partly, but it shall be stated that the patient did not exercise on his own, was not motivated and obviously wanted to stay on the sick-leave. Moreover, the results clarify that the surgery in most cases does not make do without the right rehabilitation. If the local stabilizers are not activated and the pathological movement patterns are not corrected, the relapse is quite probable, and the probability grows with every following surgery. The aims of the research were fulfilled. Key words: therapy, disc, herniation, rehabilitation.
