The application of Vojta Method within complete rehabilitation system in the ARPIDA Centre in České Budějovice.

The diploma thesis at hand aims to find out to what extent Vojta method can stimulate child?s ability to receive and actively participate in other means of the comprehensive rehabilitation system. The diploma thesis observes application of Vojta method within the comprehensive system of rehabilitation provided in ARPIDA in České Budějovice. The diploma thesis is divided into two parts. The theoretical part deals with the current state of the issue in question, provides characteristics of individual means of the comprehensive system of rehabilitation, and acquaints the reader with the technique named reflex locomotion, which is referred to as the Vojta method. The practical part comprises of two chapters. The first chapter provides the reader with a description of the selected facility. The second chapter introduces case reports of the clients completed with analyses of interviews held with parents and other members of the multidisciplinary team. Using the mentioned materials, the author produced six case reports of selected clients of ARPIDA centre. Five out of six observed clients were diagnosed with infant cerebral palsy in different forms with combined defects. For the purposes of the diploma thesis, the author used the method of qualitative research. The practical part is based on analyses of medical records and personal documentations of selected clients of ARPIDA centre; author?s own direct observation; analyses of interviews held with parents and other members of the multidisciplinary team; and observations made by the author while performing her duties as a physiotherapist. Finally, it was concluded that application of Vojta method is necessary within the comprehensive system of rehabilitation which strives to reduce the effects of the disability and improve the quality of life in subjects with the said disability. The author?s own research was designed to verify the findings learned from the literature reviewed for the purposes of the theoretical part. The conclusion of the practical part contains a summary of results, assessment of accomplished goals; and research outcomes and recommendations. The thesis may be found useful by both experts and students who have professional interests in the issue; yet, it should also be made available to the members of the multidisciplinary team as a feedback.


Rehabilitation of patients with asthma bronchiale

The topic of my thesis is the recovery of patients suffering from bronchial asthma. Asthma Bronchial (next asthma) is one of the most frequented civilisation diseases the prevalence of which has been increasing dramatically in the last 20 years. It is a chronic inflammatory disease of respiratory tract, which may appear at any age. The four major symptoms of asthma are whistling breath, shortness of breath, cough and tightness in the chest. Physiotherapy enables considerably a complex treatment of patients suffering from asthma. The complex treatment program takes into consideration the breathing and moving abilities of the patient, primarily the current state of his or her deep stabilisation system. The theoretical part of my thesis is focussed on a general description of asthma and its possible treatments, in terms of physiotherapy. The main aim of my thesis is to set up and practise a physiotherapeutic program in order to mitigate the asthma symptoms. The particular set-ups are focused on the respiratory and motion physiotherapy respecting their individual needs. The practical part of my thesis is based on a quantitative survey made by a semi-structured interview focussed on revealing the patient history, direct observation (examining the patient and realising the rehabilitation program). The survey was made within a group of three patients of a different sex and age undergoing the treatment in Luhačovice Spa, as well as one patient of a pre-school age treated in her home environment. The premises of a successful treatment following the rehabilitation scheme reflect an individual attitude to patients and their active participation in the recommended exercising and physiotherapeutic activities in their free time. My thesis can be used as a brief asthma bronchial outline for the public and for study purposes of future physiotherapists.

Role fibroblastového aktivačního proteinu v růstu a invazivitě gliomových buněk

High grade astrocytomas are very progressive brain tumors. Glioblastoma multiforme is the most frequent and the most malignant type with very infiltrative phenotype of the tumor cells. Fibroblast activation protein FAP is a predominantly membrane bound prolyl peptidase bearing exo- and endopeptidase hydrolytic activities. FAP is known to play a role in wound healing, cell migration and invasion and its expression is linked to the pathogenesis of several malignancies. mRNA expression of FAP is upregulated in 48% of glioblastomas according to The Cancer Genome Atlas microarray data. The involvement of FAP in the pathogenesis of astrocytic tumors is largely unknown. The aims of this work are to analyse the expression of FAP in primary cell cultures derived from high grade gliomas and to analyse the influence of FAP on the growth, migration and invasion of glioma cells. Our ELISA and western blot results showed heterogenous expression of FAP in the studied glioma primary cell cultures and cell lines. Both enzymatic activities characteristic of FAP were detected in the primary glioma cell culture P11 with high expression of FAP. Title: Effect of prophylactic and functional knee brace and knee sleeve on plantar pressure distribution during stance phase of gait in healthy individuals Summary: Knee bracing is a very commonly used method by both doctors and patients. Brace producers and also most of the studies focus the effect of the brace directly on the knee joint, but do not study, how it influences other regions of human body. This work studied the influence of knee bracing on plantar pressure distribution changes and also changes of the progression of the centre of pressure (COP). This study rewiewed the literature for the theoretical background in biomechanics and kinesiology of gait, description of structural, functional and methodological factors influencing the dynamic interaction of the foot contacting the ground and a review of knee brace questions. The experimental part concerns a study of the gait of 6 healthy subjects using a Footscan platform and the effects of three types of knee braces on the dynamic interaction of the foot contacting the ground. Influence of the surrounding tissue on the stresses in brain arterial aneurysms

This thesis is focused on stress in brain aneurysms. It consists of three parts. First part is aimed for gaining information about the topic from scientific resources. Next part consists of analyses of geometry of cerebral aneurysms on the computed wall stress. Analyses are performed on four basic geometrical models and results are being discussed. The risky areas in each investigated shape have been identified as well as the comparisons of stress between those shapes have been performed and the most dangerous shape among investigated shapes has been determined. Third part investigates the influence of surrounding tissue on the brain aneurysm. Conclusion of this thesis is that brain gray tissue has positive yet negligible effect on the computed wall stress.

The brain infarct core delineation using computed tomography angiography source images in acute stroke patients

Background and Purpose: Ischemic stroke is the 3rd most common cause of death and most common cause of permanent disability in developed countries. Rapid diagnostic work-up with reliable assessment of infarcted brain tissue and potentionally salvageable brain tissue is critical for acute stroke management. The first aim of this study is to asses delineation of infarct core in acute stroke patients using whole brain perfused blood volume (PBV) maps. These maps are calculated by automatic processing from computed tomography angiography source images (CTA-SI) and non-contrast computed tomography (NCCT) data. The second aim is to determine optimal quantitative threshold of PBV for infarct core identification in acute stroke setting. Material and methods: PBV maps were constructed using prototype software from NCCT and CTA-SI data in 37 acute ischemic stroke patients with angiographically proved recanalization after intravenous thrombolytic treatment. These PBV maps were automatically compared with final infarct extension on follow-up NCCT. The anatomic pixel-by-pixel correlation was assessed using Matthew's correlation coefficient (MCC) for infarct core delineation using different critical values of PBV. Options of physiotherapy treatment in patients after stroke.

The topic of this thesis is Options of physiotherapy treatment in patients after stroke. The objective of the thesis is to survey physiotherapy-nursing care for patients after stroke and to describe current possibilities and quality of physiotherapy care for patients after stroke The thesis deals with possibilities of physiotherapy treatment and cooperation between nursing and physiotherapy staff. One of the targets is description of physiotherapy care for patients after stroke which is documented by patients{\crq} medical records. The other target is physiotherapy care monitoring supported by the quantitative research using the questionnaire method. The aim of this thesis was to develop an appropriate methodology to be used as an instrument for physiotherapy care for patients who had sustained acute stroke. Another objective was to determine the level of cooperation and awareness of staff members involved in the physiotherapy-nursing process. To meet the objectives the method of quantitative research was used. The methods of participated observation, semi-standardized interview and documents evaluation were included. Complementary data were obtained by the quantitative research using the questionnaire method. The research was conducted in patients after stroke who were hospitalized at the neurological department in Ceske Budejovice hospital. Additional quantitative research, using the questionnaire method, was conducted at the neurological and internal departments in hospitals in Ceske Budejovice, Cesky Krumlov and Pribram. On the basis of the research results, methodology of physiotherapy care for patients after stroke in acute condition was developed and the lack of awareness and cooperation of physiotherapy and nursing staff was found. The thesis will be used as a study and information material for all members of the medical staff involved in treatment of patients after stroke, but also for general public. Information can be forwarded through lectures, leaflets, etc.
