Psychic and physical factors in a nurse profession at home care

Homecare is given to people in their natural social environment. It is aimed mainly toward sustaining and supporting health, restoring health and improving possibility of self-care, giving palliative care to incurably ill patients and enabling peaceful dying and death. For the nurses to be able to give quality nursing care they need to be in psychic and physical balance. It is important for the nurses to be aware of the consequences of such imbalance and also of the way it influences the quality of their work especially when handling difficult situations (Misconiová; 1995; Paragraph nr. 96/2004 Col.; Komačeková, 2009). First aim was to map psychic and physical factors influencing the work of a homecare nurse. Second aim was to find differences in psychic and physical factors in the profession of nurses working in homecare and in hospital. This study is based on qualitative research using questioning through in-depth interview. Research was conducted in the beginning of February 2014. Methods of direct transcription and open coding were used for data analysis. Two questionnaires were added to the original research examining Psychic stress and Physical activity according to Baecke, evaluating the amount of psychic and physical stress at work respectively. The research group consisted of 6 nurses aged between 33-55 working for homecare agencies in the south bohemian region. Written agreements were obtained from the nurses prior to them being interviewed or be given the questionnaires. Five categories were identified from the analysed interviews: psychic stress, physical stress, handling of stress, consequences of stress and the differences found regarding psychic and physical factors observed in homecare versus hospital environments. Results obtained from the questionnaire estimating Psychic stress show average value at 3.5 which can be regarded as borderline between decreased and increased risk. The analysis of Physical stress questionnaire gave the average of 3.1 corresponding to medium load. Regarding the first aim of this work the results of the questionnaires and interview are showing greater psychic stress felt by the nurses in homecare environment as compared to physical load. The second aim comparing homecare and hospital environments shows less psychic than physical stress in homecare than in hospital environments. Practical output of this bachelor thesis will be a written recommendation for nurses working in homecare aimed toward improvement of their psychic and physical health in connection to their working experience.

Children attachment to adult care providers in children homes - analysis of conditions and possibilities

The thesis tries to find the answer to the basic question: "What are the possible ways to strengthen attachment between children at the age from 9 to 18 months and their caregivers at the facilities of institutional care, nurses in particular?" The work also aspires to map the external conditions influencing formation of the attachment. In order to explain the topic, some theoretical findings of developmental psychology, attachment theory and information about the system of institutional care in the Czech Republic have been presented here. The answers concerning the strongest factors of attachment behavior and different attachment of one child to various caregivers and vice versa were being sought with the help of the method of an involved observation of the interaction child-caregiver during specific activities of feeding and bathing. Pieces of information about external influences of the attachment behavior and the possibilities to nurture it were being gathered by the method of semi-structural interview with the caregivers and the directors of the visited institutions. in English My thesis is focused on integration of diagnostic and terapeutic methods of clinical psychology into the concept of contemporary cardiology. Modern medicine consider not only somatic aspects of patient's ilness, but also the aspects of personalitie's diferencies and needs. Psychologist applies the knowledge of psychological theories and psychophysiological mechanisms into the field of clinical diagnostic and treatment thinking about the individual patient. (A) Psychologic treatment of in-patients involves: (1) Crisis intervention (KI), i.e. treatment of the patients with urgent psychological difficulties. Cisis intervention presents the most indicated type of intervention in in-patiet's psychological treatment. (2) Pre-operative psychological care before recent cardiothoracic surgical intervention reduces patient's distress in pre-operational and post-operational period. (3) Individual approach to problematic personalities can significantly improve cooperation with medical staff. Ethnic identity of a gypsy child in an adoptive family

The graduation thesis treats of the theme of ethnic identity of a Gypsy child in an adoptive family. The theoretical part of the thesis outlines the history of Czech Romanies and the framework of adoptive and foster care in the Czech Republic. The main chapter of the theoretical part discuss the question of identity, particularly ethnic identity and its formation. The empirical part discuss a research on formation of ethnic identity of an Gypsy Child in an adoptive family, realized by a questionnaire examination of adoptive and foster parents. The research consider the problems of the diversity of gypsy children, their origin or the importance of a contact with the original Gypsy community. One of the referenced questions is also the influence of racism upon the formation of ethnic identity.

The Czech Republic as a signatory of the Convention on the Rights of the Child has vested the practice of state administration in the field of care for minor children to municipalities with extended powers, where the protection of the rights and legitimate interests of minor children is safeguarded by state authorities of social and legal protection of children (ASLPC hereinafter). These authorities are incorporated to the extent laid down by the Act on Social and Legal Protection of Children (Act No 359/1999 Coll., as amended). The position and roles of ASLPC and their competence and jurisdiction are also regulated by the same act. The position of workers of these authorities and its legal embodiment is equally important. Regarding professionality, there are high demands on these workers. In the Czech Republic, significant changes in legislature have been made over the past three years. These amendments have led to the strengthening of the rights of minor children and to the establishing of new tools for their protection. In connection with the adoption of new substantive regulation of family law, which is now exhaustively dealt with by the Act No 89/2012 Coll., the Civil Code, new procedural rules have been established regarding protection of rights of minor children. Such procedural legislation includes (apart from the long-existent Act No. 99/1963, the Code of Civil Procedure) the Act No 292/2013 Coll., on Special Civil Proceedings. The decision-making regarding minor children has been vested mainly by the state into the competence of courts. The courts appoint the locally relevant ASLPC as a guardian who then represents the interests of minor children. The purpose, interwoven with and derived from the first one, was to explore the opinions of selected social workers of ASLPC and of district court judges regarding the defined problems occurring during the work of ASLPCs and courts while protecting the interests of minor children. These particularly include: the use of the tool of precautionary measure; problems of different territorial jurisdiction of courts and ASLPCs; processing of complaints regarding bias in various phases of the proceedings; professional training of workers of ASLPC It is evident from the proposals of both judges and ASLPC workers that it is necessary to unify territorial jurisdiction. The courts are suggesting the priority criterion of the address where the child mainly resides. ASLPC workers suggest the official permanent residence as the prime criterion. Bias causes problems in all phases of proceedings. It is obvious that courts take differing approaches towards the solution of this problem (some courts solve the problem of bias complaints filed by the parties while other courts do not) and for ASLPC workers it is difficult to assess how to act when such complaint is raised against them. The training of ASLPC workers - workers are obliged to educate themselves and their employer is obliged to finance such training. Not all ASLPCs are able to provide training in the required extent - due to financial reasons (the average costs of education of one employee pose from 9,169 to 13,400 CZK annually and they cover approximately 6 days of training). The heavy work-load caused by insufficient numbers of ASLPC employees prevents them from completing such compulsory training. As part of the examination of the "participation of a collision guardian in court proceedings" it has been found out that irregular participation of the worker in the proceedings (absence from participation in appeals proceedings); incomplete reports (directed to courts) from surveys in families. Based on comprehensive examination of the problems of territorial jurisdiction, it was recommended that requests are independently recorded and financial compensation is provided for ASLPCs who are requested to represent minors in front of courts, to draft proposals for precautionary measures.

Upbringing and reaching of persons with autistic spectrum disorders

The diploma thesis "Upbringing and teaching of persons with Autistic Spectrum Disorders" deals not only with diagnostics of individual autistic disorders, but also with pedagogical and educational approaches, integrating of autistic persons into the normal society, possibility to use their abilities etc. The Autistic Spectrum Disorders include Autism, Atypical Autism, Asperger syndrome, Rett syndrome and childhood disintegrative disorder. All these are among pervasive developmental disorders that typically appear within the first three years of life. the early examination is very important, otherwise the child can be misdiagnosed as mentally retard. Scientific studies showed, that many individuals with autism are not retarded, but in fact, they are unable to give their answers accurately for the determination of their iq in some intelligence tests. Autistic children and adults typically have difficulties in verbal and non-verbal communication, social interactions, and leisure or play activities. Relevant behavioral or cognitive intervention can help children gain get self-care, social, and communication skills. Only few people with autism live independently after reaching adulthood, however, but some of them become successful and they are relatively highfunctioning. Injuries of children at home

The theoretical part of this thesis is oriented at an actual issue of accidents involving children in home environment, their prevention and a correct provision of first aid by a non-professional at these types of selected accidents. The practical part of this thesis was led by a qualitative form of research, a method of a semicontrolled interview. The interview given was led by chosen caring people, who gave first aid to children with a home environment injury. The whole process of the interview was recorded at a digital record book and then rewritten to a text form, with the approval of the respondents. The respondents were chosen on the basis of a survey. The final information from the interviews were analyzed and subsequently summarized into results. Six respondents were registered into the research part. Six basic questions were given to the respondents. The questions were chosen to find out information about an injured child, an injury character, a procedure of caring person while giving first aid, or other professional care and permanent effects. While preparing the research part, I was expecting that most of the respondents would manage to give first aid, however, according to the results, half of the parents didn't know how to deal with giving first aid. Non- professional first aid, provided at the right time and done correctly, is one of the basic and essential steps to reverse the injured person back into the normal quality of life. This is why, all people should be able to give first aid without special equipment and tools. The aim of my thesis was to monitor the process of the caring person while giving first aid for selected accidents involving children in home environment. I gave myself one fundamental research question: What was the process of the caring person while giving first aid for accidents involving children in home environment? At the final evaluation and comparison of my results, I achieved the results, in most examined facts, which are presented in other different studies dealing with similar issues- children's injuries and giving first aid for accidents involving children.
