Goals: to focus on the Quality of life in clients of the substitution program according the duration of treatment at the Department of Addictology, 1. LF UK a VFN in Prague. Background: The topic of dissertation was chosen on the basis of a 16-year clinical experience of the author with the target group of clients in methadone substitution. I realized based on long-term observation, that clients go through certain stages of satisfaction with their lives. This may correspond with their functioning in the program. The theoretical part describes the substitution treatment for opiate dependence and quality of life concept. Methods: A total selection method was chosen. Data are collected using a standardized questionnaire World Health Organization - WHOQOL-BREF (short version). File: The research sample consists of 39 patients (out of total of 65 clients of Substitution centre, Department of Addictology First Faculty of Medicine, Charles University and General Teaching Hospital in Prague. Results: There were no significant differences in the quality of life in relation to the duration of treatment. The highest quality of life had clients in the category 2-5 years in the program. Background: The theme of the position and experiences of pregnant women in the context of methadone maintenance treatment has not yet been thoroughly examined, the aim was to contribute to break the taboo concerning this topic and help to destigmatize these women. Goals: To map the situation of pregnant women in MMT and to alert to therapeutic specificity of these women. Methods: Semi-structured interview and analysis of the medical documentation were used to obtain the data, observation method, qualified estimate and clinical case reports. Data were processed through content analysis. File: The research group consists of 7 patients who were treated in the Department of Adictology 1st Faculty of Medicine, Charles University and General University Hospital in Prague, who gave birth to at least one child between 2004 and 2014. Results: The analysis shows that during their pregnancy women reduce the use of illicit drugs and tobacco. Despite these pregnancies are usually not planned, it was observed that in these women the pregnancy occured in a situation when the respondents were stabilized in both the treatment and in their social situation. The attitudes of medical doctors and other key professionals regarding opioid substitution therapy in Kazakhstan

Background A number of studies across countries have proven that the methadone maintenance program, if efficiently started and implemented, is really effective tool both for treatment of opioid dependence and harm reduction. The expansion of methadone substitution programmes in Kazakhstan may have a positive influence upon the health and social climate inside the country. The main target of this research is to survey the attitudes of medical doctors regarding opioid substitution treatment in Kazakhstan. Methods The study was composed of two surveys. Questionnaires were distributed (administered by electronic mail) to 17 centres for drug addiction treatment all over Kazakhstan. 19 medical doctors not yet providing opioid substitution treatment and 6 medical doctors enrolled in opioid substitution treatment responded to the questionnaire. Two therapists also participated this research and their responses are mentioned as well. Quantitative data were supplemented and enhanced by use of qualitative data obtained by analysis of original documents and by interviews with two key informants. Out of the data gained from the research it is obvious that despite some negatives of the substitution treatment it's clients considers it to be very positive especially because it allows them to get involve in the society and to live the "normal life". Suprisingly, in comparison with the orientation of intact population the clients almost completely ignore the influence of substitution on their state of health and the risk of transer of infection diseases. The effect of methadone on cardiac ischemic tolerance in rats

Opioids are considered as a dangerous addictive substances which are widely used in medicine for their strong analgetic effects. Opioids (such as morphine and methadon) may nevertheless play an important role in the resistance of the heart to ischemia by reducing the rate of cell damage. This protective effect is well understood about morphine but we don't know almost nothing about effects of methadone on the myocardium. The main aim of this thesis was to find out how chronic methadone treatment affects ischemic tolerance of rat hearts. For our experiments we used Wistar rats in two series. In the first series we administered morphine (10 mg/kg/day, i.m.) or methadone (2 mg/kg/day, i.m.) for 10 days. In the second experiment series we administered methadon for 28 days (2 mg/kg/day, i.m.). For analysis of the ischemic heart tolerance we used the isolated perfused heart method. Incidence and severity of ischemia and reperfusion arrhythmias were analyzed during the 50 min of ischemia and early reperfusion. Infarct size was analyzed histochemically, using tetrazolium salts and KMnO4 1 h after reperfusion and was determined by planimetric method. Case study of a client included in methadone substitution programme

of bachelor thesis Keywords: methadone- susbstitution therapy- client- substitution programme-causes The bachelor thesis deals with the effect of client participation in the complete substitution treatment programme with methadone, which aims to demonstrate a positive effect not only on the client itself, but also on its vicinity. The theoretical part deals with methadone, its effects and the progressive placing into the treatment of addicted clients, firstly abroad and then in the Czech Republic. It deals with forms of treatment working in the Czech Republic with a detailed description of a complex substitution therapy. At the end of the theoretical part are discussed the causes of origin of an addiction on the addictive substances. The practical part is a case study of two clients - married couple, what describes in detail their life before the treatment, during the treatment and at her one year after the treatment, when she has been included in the substitution programme for ten years and is ending with a voluntary departure from the program with the zero dose of methadone - the client successfully abstains. Subjective Perception of the Life Change of Persons in the Methadone Maintenance Treatment Program in České Budějovice.

The thesis focuses on the subjectively perceived changes in the lives of persons in the methadone maintenance treatment program, on their experiencing of the changes and subjective judgment of the treatment. The topic is treated according to the Bio-Psycho-Socio-Spiritual Model of Addiction. The theoretical part deals with the quality of life and the subjectively perceived well-being and defines six dimensions of well-being on which the practical part of the thesis is based. It also elaborates on the Bio-Psycho-Socio-Spiritual Model of Addiction in the context of the methadone maintenance treatment. Moreover, it describes the factors in life change and the motivation to change. The last theoretical chapter focuses on the opioid substitution treatment as such and more specifically on the methadone maintenance treatment. The practical part presents the results of the research. The aim of the thesis is to analyse how the persons in the methadone maintenance treatment program in České Budějovice perceive life changes. A qualitative research was chosen for that purpose. The data were obtained through the technique of narrative interview. The research group consisted of four respondents who were on the methadone maintenance treatment program. The results show that the subjectively perceived changes in the lives of the persons in the methadone maintenance treatment program relate to five out of six dimensions of well-being. No changes were recorded in the dimension concerning the sense of life, in other words, even after a several month methadone maintenance treatment these persons perceive no goal of their lives. Important consequences for work with these persons can be drawn from that, especially for therapists in opioid substitution centres, who should develop spirituality aspects of persons.
The Importance of Substitution Programmes in Drug Addiction Therapy System

The title of my thesis is: The Importance of Substitution Programmes in Drug Addiction Therapy System. Substitution therapy has its unique place especially in the treatment of opioid addictions. It helps individuals with stabilization and improvement of their social, somatic and mental state and it is important for the society, when public spending on the care and repression are reduced thanks to substitution treatment. In this bachelors thesis I deal with the issue of the effects of substitution therapy on the lives of people who use it for a long time. First, in the theoretical part, I define the issues concerning the addiction on non-alcohol drugs and then I deal with substitution therapy, its history, its current situation, its forms, types and components that substitution therapy includes. Attention in this section is paid to the effectiveness in terms of individual as well as social; therefore, I focus also on the disadvantages and difficulties that are associated with substitution therapy. As the objective of the thesis I chose to discover the importance of substitution programmes in drug addiction treatment. To achieve this objective, I defined two subobjectives, namely: Mapping the subjective evaluation of mental, somatic and social satisfaction of persons included in a substitution programme, and the comparison of mental, somatic and social situation prior and after inclusion of the client in a substitution programme. Given these objectives, I established three research questions that ask about the effects of substitution treatment on the social, somatic and mental aspects of the respondents. The research was conducted in a qualitative form, by the method of interviewing and the technique of semi-structured interviews. The research results have confirmed the improvements or stabilization of social, somatic and mental aspects of persons included in long-term substitution programmes. As the core finding I consider the fact that there is some abuse of substitution therapy, in which case its effectiveness in these three levels is less visible than in persons observing substitution therapy in the prescribed manner. The results of my thesis can serve for the needs for further research, to inform the general public and professionals, and they also have a practical use when working with clients in substitution treatment.
