[cze][eng] Východiska: Téma situace a zkušeností těhotných žen v kontextu substituční léčby nebylo dosud zpracováno, autorka chtěla studií přispět k odtabuizování tohoto tématu a destigmatizaci těchto žen. Cíle práce: Zmapování situace těhotných žen v substituční terapii metadonem a upozornění na léčebná specifika. Metody: K získání dat bylo využito polostrukturovaného rozhovoru a analýza obsahu zdravotnické dokumentace, dále byla použita metoda pozorování, kvalifikovaného odhadu a klinické kazuistiky. Data byla zpracována prostřednictvím obsahové analýzy, která vychází ze zakotvené teorie. Soubor: Výzkumný soubor tvoří 7 pacientek, které porodily minimálně jedno dítě v průběhu léčby v Centru substituční léčby Kliniky adiktologie 1. LF UK a VFN v letech 2004 - 2014. Výsledky: Z analýzy vyplývá, že s těhotenstvím ženy redukují užívání nelegálních drog a tabáku. Přesto, že těhotenství nebývají plánovaná, přicházela v situaci, kdy byly respondentky stabilizované v léčbě i ve své sociální situaci. Případně bylo těhotenství motivem k rozhodnutí pro léčbu. Zkušenost těhotenství bývá podnětem pro snížení dávky nebo ambici odchodu do abstinence. Důvody bývají konfrontace s NAS dítěte, ale i zkušenost s odsuzujícím přístupem zdravotníků. Ve většině případů byla ženám zastavena laktace a znemožněno své děti kojit, ačkoliv...Background: The theme of the position and experiences of pregnant women in the context of methadone maintenance treatment has not yet been thoroughly examined, the aim was to contribute to break the taboo concerning this topic and help to destigmatize these women. Goals: To map the situation of pregnant women in MMT and to alert to therapeutic specificity of these women. Methods: Semi-structured interview and analysis of the medical documentation were used to obtain the data, observation method, qualified estimate and clinical case reports. Data were processed through content analysis. File: The research group consists of 7 patients who were treated in the Department of Adictology 1st Faculty of Medicine, Charles University and General University Hospital in Prague, who gave birth to at least one child between 2004 and 2014. Results: The analysis shows that during their pregnancy women reduce the use of illicit drugs and tobacco. Despite these pregnancies are usually not planned, it was observed that in these women the pregnancy occured in a situation when the respondents were stabilized in both the treatment and in their social situation.
