Using of an Automated External Defibrillator by Non-Medical Components of the Integrated Rescue System in the Liberec Region

This bachelor's thesis is focused on the using of an automated external defibrillator by non-medical components of the Integrated Rescue System in the Liberec Region within the framework of performing urgent cardiopulmonary resuscitation. The aim of this work is to evaluate the frequency, time course and results of interventions in cases of patients with sudden cardiac arrest, in which an automatic external defibrillator was used by the non-medical component of the Integrated Rescue System. The work is divided into two main parts. The first part is a systematized theoretical overview of basic information on the issue. The second part is focused on practical aspects and is created from data obtained from the basic components of the Integrated Rescue System. The data were examined by descriptive statistics methods. Based on the obtained outputs, a systematic overview of the outcomes of first responders to sudden circulatory arrest was created. This overview mainly includes data outputs focused on the frequency of activation of first responders, the time of their arrival at the scene, the success of resuscitation in the pre-hospital phase and the outcome of the patient. The information obtained was compared with the conclusions of foreign studies focused on the same area. The period for which the data were collected included the year 2017 after May 2021. At the end of the work, an evaluation of the key results is made and suggestions for further research related issues are given.
Training of cardiopulmonary resuscitation in hospital - preparation of the simulation scenarios

Presented thesis deals with the topic of training of cardiopulmonary resuscitation at hospital and preparation of the simulation scenarios. It contains theoretical and empirical part. The objective of the thesis was to find out about the process of the training for nurses at hospital. To analyze their readiness for providing cardiopulmonary resuscitation based on training they absolve. And then to map missing elements within cardiopulmonary resuscitation training. Three research questions were focused on these objectives. In which way current training of cardiopulmonary resuscitation at hospital takes place? To what extent are nurses ready to provide cardiopulmonary resuscitation to a patient at their workplace? Which shortcoming contain trainings of cardiopulmonary resuscitation at hospital to this point? For gathering the necessary information, qualitative research was chosen. The technique of semi-structured interviews and observation was chosen. The research sample was made up of 11 probands, meaning nurses working at chirurgical and intern standard department and chirurgical and internal intensive care unit at the same hospital. The condition of the same workplace was chosen so all nurses will have absolved the same trainings. The results of the research shown that training does not involve simulation training, but nurses would welcome this option. Training takes place at prepared mannequins, not within a simulation scenario and authentic situation at a department. Also, mostly on the basis of absolved trainings they feel prepared to provide CPR at their department but mostly because they know that they are not alone at the department. Among the most common shortcomings of the training they rank aids to CPR - real mannequins, real situation but also training room. The thesis will be beneficial to development of CPR training for nurses but also for students getting ready for the profession of general nurse. And that mostly because of simulation scenarios creation which are output of the thesis and is based on research in which we tried to find out opinions and needs of nurses connected with training.
Children febrile convulsions in pre-hospital care

The bachelor's thesis deals with the children febrile convulsions in pre-hospital emergency care. The theoretical part of the thesis describes the matters of childhood, its specifics and division. It also focuses on the clinical picture, complications, prevention and treatment of febrile convulsions in children in prehospital emergency care. There were two goals set in the practical part. The first goal was to map and compare the procedure of paramedics in febrile convulsions in children in the emergency ambulance crew of the South Bohemian Region and the Vysocina Region. The second goal was to map the knowledge of paramedics in febrile convulsions in children in the emergency ambulance crew of the South Bohemian Region and the Vysocina Region. Two research questions were also identified: How are paramedics in the emergency ambulance crew of the South Bohemian Region and Vysocina providing pre-hospital care to children with febrile convulsions and what is the knowledge of paramedics about febrile convulsions in children? To obtain the necessary data, the method of qualitative research was used through the semi-structured interviews with 10 paramedics of the Medical Rescue Service of the South Bohemian Region at the regional center in Ceske Budejovice and Medical Rescue Services of the Vysocina Region in Trebic. Respondents were answering sets of 16 questions and the outcome was categorized into 13 categories, which succeeded in meeting both goals and at the same time answered both research questions.The research found that the interviewed paramedics do have sufficient knowledge about the causes of febrile convulsions and what their basic therapy is. Unfortunately, they don't know a division of children age well enough. All respondents agreed that they were afraid of the diagnosis of Febrile convulsions precisely because of the pediatric patients and that the knowledge in the field of pediatrics is not their strongside. There was no significant difference between the two regions in the answers concerning the procedures of febrile convulsions in children in pre-hospital emergency care.
Ability of laical public providing first aid in acute condition from point of view of paramedics

This bachelor's thesis examines the issues of providing non-professional first aid in acute condition from paramedics'point of view. The bachelor's thesis is divided into a theoretical part and research part. At first, the theoretical part describes the emergency medical service, its rescue forces and types of ambulance crew in order to explain the connection between the first aid and pre-hospital emergency care. This part analyses the first aid not only from the theoretical and practical point of view, but also from a social psychological one. Then, the theoretical part also deals with anatomy, physiology, pathophysiology and the first aid of six selected acute conditions. The second part is focused on the analysis of data obtained from a research survey. The research survey was conducted by a qualitative survey using as main methods a semi-structured interview and direct observation during university practice. The semi-structured interview includes 33 questions and is answered by eight paramedics that serves in the South Bohemian Region. The aim of this thesis is to find out what the range of capabilities of general public is in providing the non-professional first aid, how effective the general public is in providing the non-professional first aid and whether people are willing to help. The obtained information led to answering all research questions and fulfilling the aim of the bachelor's thesis. The results of the research evaluate non-professional public as willing enough and with sufficient abilities to provide the first aid but on the other hand the effectiveness of performed actions is considerably insufficient. The main contribution of this bachelor's thesis is the rise of awareness about the issues of the first aid provided by laical public, which could contribute to the improvement of the first aid educational method and subsequently to the improvement of providing the non-professional first aid by general public in cases of acute conditions.
Cardiopulmonary resuscitation from the point of view of emergency medical assistance group

This thesis thoroughly explores the issues that come with cardiopulmonary resuscitation, which in pre-hospital care is handled by emergency medical assistance groups and of which paramedic staff are an inseparable part. These paramedics have a necessity for sufficient, in-depth knowledge and experience. Without the early and critical assistance that paramedics are able to provide, a patient could suffer from irreversible changes in health and are even at a risk of death. This bachelor's thesis is divided into a theoretical section and a research section, further exploring this incredibly important subject. The theoretical section explores the history of cardiopulmonary resuscitation. It also digs into the basic life support provided mainly by lay rescuers, in accordance with the guidelines of the European Resuscitation Council. Furthermore, this section also contains information about advanced life support, once again in accordance with the guidelines of the European Resuscitation Council, which is aimed primarily at health care professionals and particularly those who carry the title of paramedic. The research section on the other hand, is based on qualitative research based on interviews consisting of 13 questions. These questions are targeted towards a research group consisting of a group of 12 paramedics employed in the South Bohemian region. This section includes an analysis of the obtained data and their evaluation. The aim of this work is to map the awareness of paramedics about the latest suggested methods for cardiopulmonary resuscitation according to the guidelines of the European Resuscitation Council and to find out how paramedics view and handle cases of sudden circulatory arrest.
Assessment during the course and effects of cardiopulmonary resuscitation

The objective of this bachelor thesis is the assesment during the course and the effect of cardiopulmonary resuscitation in Hospital Kolin, a.s. The theoretical part contains chapters describing cardiopulmonary resuscitation according to the current recommendations, adult education, competencies of the general nurse and lifetime education in the organization in general. The empiric part is focused on the didactic evaluation of the course as well as participants' evaluation, evaluation of skills and knowledge of the participants as the effectiveness indicator. Lastly, in the final part there are suggestions for changes which could optimize the course. The target group was the participants of the CPR course - doctors and general nurses of Hospital Kolin, a.s.
Knowledge of the younger schoolchildren about life emergency staets

The bachelor's thesis concerns the research of the knowledge of the younger school children about life-saving situations. A group of fourteen children visiting the second school class in the Prague West district was used as a sample. The goal of my thesis was to gain a picture about the knowledge of the eight years old children about urgent first aid as well as to give the children a quide how to act in the unfamiliar crisis situations. The thesis is divided into two parts. Theoretical part is divided into separate chapters. The beginning is devoted to an introduction into the topic of the urgent first aid, to present the components and function of the integrated rescue system and there are as well some statistics mentioned here. The second chapter deals with general procedures recomended when providing first aid. In the last chapter I describe particular selcted life-threatening situations and methods of first aid provision. In the empirical part the methodology and organization of the research is decribed. I have used a qualitative focus group method in connection with practical training of some urgent first aid techniques. Nursing Process for a Patient with Post-resuscitation Therapeutic Hypothermia after Acute Myocardial Infarction

The bachelor's thesis deals with acute cardiology, namely with casuistry of a patient after acute myocardial infarction with successful cardiopulmonary resuscitation in the field, artificial pulmonary ventilation and therapeutic hypothermia. The thesis wants to present the highly specialised and demanding nursing care of resuscitated patients from the nurse's point of view with the use of the most modern procedures and technology. The theoretical part of the thesis is divided to several chapters describing anatomy and physiology of the cardiac muscle, acute myocardial infarction, cardiopulmonary resuscitation and therapeutic hypothermia. The main focus of this part of the thesis is on the diagnosis of the acute coronary syndrom itself. The theoretical part describes in more detail therapeutic hypothermia and nursing care of patients after cardiopulmonary resuscitation. Very important chapter is also introduction of a patient on whom is concentrated the nursing care in the practical part of the thesis, and presentation of his hospitalization. Possibilities of organ protection after global ischemia during cardiac arrest.

Successful cardiopulmonary resuscitation is the first step to rescue life during cardiac arrest. High mortality even after successful restoration of spontaneous circulation is substantially caused by patophysiological process associated with ischemia-reperfusion injury and it is widely called post-cardiac arrest syndrome (PCAS). There are many patophysiological mechanisms involved in the development and progress of this syndrom; the key role seems to play oxidative stress, triggering the activation cascade of systemic inflammatory reaction. In our study we have tested different possibilities of influencing the post-cardiac arrest syndrom. In the first experimental study we have compared the effect of mild therapeutic hypothermia with controlled normothermia on PCAS in a porcine model of cardiac arrest. In the second study we have compared in the same model the protective effects of mild therapeutic hypothermia, administration of nitric oxide and ischemic postconditioning. Results of the first experiment have revealed that mild therapeutic hypotermia is superior in the resuscitability, maintenance of blood pressure, oxidative stress suppression and organ damage protection than controlled normothermia.
