Maisonneuve fracture - anatomy, diagnostics, operative approaches and technics of osteosynthesis
Kašper, Štěpán ; Tuček, Michal (advisor) ; Kachlík, David (referee) ; Hromádka, Rastislav (referee)
Although Maisonneuve fracture (MF) is a well-known type of ankle fracture-dislocation, there are a number of misconceptions that can lead to wrong choice of treatment. The concept of Maisonneuve fracture is also not understood by all authors in the same way and the recent CT studies confirmed that this is much more complex and variable injury. The dissertation was divided into two main parts: anatomical (experimental) and clinical. In the experimental part, the aim was to describe the appearance and shape and demonstrate the clinical significance of the crista malleoli lateralis (CML), which is located on the lateral side of the distal fibula and which previous anatomical and clinical studies neglected. Knowledge of this structure is essential to correct placement of syndesmal screws. The aim of the clinical part was the precise description of the pathoanatomy and associated injuries within the "classic" and less frequent forms of Maisonneuve fracture based on CT examination and intraoperative findings. Anatomical studies were performed on 352 dry bone specimens of adult fibula. Clinical studies were conducted on a set of 54 adult patients with "classic" MF occuring in the period 2012-2018 and 11 adult patients with "double" MF occuring in the period 2012-2020. In the anatomical part of the...
Modern therapy of chondral defects
Neckař, Pavel ; Havlas, Vojtěch (advisor) ; Zeman, Petr (referee) ; Tuček, Michal (referee)
The thesis describes the application of cultured bone marrow stem cells in the therapy of focal chondral defect of the knee joint. In the experimental part of the work, the goal was to quantitatively and qualitatively compare two sampling sites of bone marrow monocytic aspirates, the iliac crest bone and the proximal tibia, in order to determine a suitable cell source for advanced cell therapy. The sample analysis showed that the amount of monocytic cells and the yield of stem cells from the aspirate obtained were significantly higher in the bone marrow from the iliac crest. We did not find significant qualitative differences between the two sources of stem cells. In the clinical part of the work, I present a description of the surgical procedure and the results of a 1-year follow-up of patients after the implantation of cultured stem cells from the bone marrow, under the name BiCure® orthoMSCp (Bioinova, Prague, Czech Republic), fixed on a commercially available 3D scaffold Chondrotissue® (BioTissue AG, Geneva, Switzerland) using coagulated autologous platelet-rich plasma. The primary objective of the clinical study included the evaluation of the short-term and long-term safety of the applied medical product. The secondary objective of the work included the assessment of the effectiveness of the...
Scapular fractures associated with acromioclavicular and sternoclavicular luxation
Strnad, Tomáš ; Tuček, Michal (advisor) ; Fulín, Petr (referee) ; Pilný, Jaroslav (referee)
Fractures of the scapula used to be among rare injuries, but in the last two decades their number has increased. Fractures of the scapula can be isolated, but in 67-92% they are combined with injuries to other structures of the shoulder girdle. The dissertation was divided into two parts: experimental and clinical. In the experimental part, the aim was to describe the occurrence and variants of sulcus coracoglenoidalis (CGN) and sulcus arteriae circumflexae scapulae (SCF). These two little-known structures play an important role in the occurrence of some types of scapula fractures and in their surgical treatment. Combined scapular injuries with AC and SC dislocation are rare. Therefore, in the clinical part, we decided to identify and describe in detail combined injuries of the scapula with AC and SC dislocation, including a detailed description of fractures of coracoid process. Anatomical studies were performed on 330 dry bone specimens of adult scapulas. Clinical studies were conducted on a set of 519 scapula fractures in adult patients occurring in the period 2002-2020. In the anatomical part of the dissertation, two anatomical structures were described for the first time: CGN and SCF. Our studies pointed to considerable variability in the occurrence and degree of formation of both of these...
Paraarticular ossifications after total hip replacement. Modalities of preoperative and postoperative reduction interventions
Debre, Ján ; Dungl, Pavel (advisor) ; Tuček, Michal (referee) ; Dufek, Pavel (referee)
1 PURPOSE OF THE STUDY Heterotopic ossification is frequent and well known complication after primary total hip arthroplasty. Prophylaxis is crucial, ohterwise when ossification is matured, the only treatment option is surgical removal during revision hip surgery. Prophylaxis options are pre, peri- and postoperative modalities. Effectiveness of the profylaxis step sis the aim of this disertation. The goal of Study 1 was to prove the positive side effect of tranexamic acid application to reduce the heterotopic ossification ratio after elective total hip replacement. The goal of Study 2 was to prove the reduction in HO ratio with experimental modification of anterolateral approach with electrosurgery. The goal of Study 3 was to detect the knowledge among czech orthopaedic surgeons in ossification issues and compare the results of questionnaire with german results. 2 MATERIAL AND METHODS Study 1 Cohort of 401 total hip replacements were assessed prospectivelly. Stratification of particular degrees in Brooker scale, sex, laterality and fixation type were evaluated. The average follow up was 6,10 year (40m to 113m). Hips from 2012 are referential and hips from 2016 are administered to tranexamic acid procotol. Other secondary prophylactic modalities (farmacoprophylaxy or radiotherapy), tertiary...
Enhance the functionality of G Suite tools using App Script
Tuček, Michal ; Leipert, Jiří (advisor) ; Tocháček, Daniel (referee)
This work deals with cloud theory, focusing closely on the G Suite platform and its tools. In the next part, it presents the Google App Script, with which we can connect the individual G Suite tools and enrich their functionality. There will be introduction to the methodology of working with Google App Script with a few practical examples which should introduce the reader to this issue at the beginner level.
Scapular fractures: Clinical anatomy, radiodiagnostics, classification-it's importance for operative treatment
Tuček, Michal ; Bartoníček, Jan (advisor) ; Paša, Libor (referee) ; Skála-Rosenbaum, Jiří (referee)
Fractures of the scapula are not very frequent and account for less than 0.5 per cent of all fractures. In most cases they are caused by high-energy trauma and are associated with other injuries. Until recently, fractures of the scapula were rather neglected and treated mostly conservatively. Lately, hover the situation has changed. The number of patients with a scapula fracture is increasing, the interest in this issue is growing and discussion focuses more and more on operative treatment of these fractures. The dissertation thesis provides a comprehensive overview of diagnostics, classification and treatment of scapula fractures. In the anatomical section we have described weak areas in the construction of the scapula that predispose incidence of fracture lines. We have studied topographic relations of individual structures on the posterior aspect of the scapula from the viewpoint of the Judet approach and its modifications in individual types of fractures of the scapular body and neck. In the radiological section we have compared efficiency of individual imaging methods with the aim to evaluate the possibilities of individual radiographic methods, 3D CT reconstructions in particular, from the viewpoint of an exact determination of the type of the fracture, number of fragments and their...
