Possible relation between instrument playing and occurrence of musculoskeletal disorders among guitarists and pianists

Playing a musical instrument requires hours of exposure to a fixed position and a performance of greatly repeated movements. This excessive physical load in combination with other factors can often result in playing-related musculoskeletal disorders (PRMD). This thesis specifies the definition of PRMD, clarifies the idea of musician's medicine as a fairly new specialization, titles the most commonly occurring medical conditions and risk factors in musicians with an emphasis on guitarists (classical and electric guitar players) and pianists. It also offers recommendations on how to play these instruments with respect to the ergonomics. The aim of practical part of the thesis was to find the possible relation between PRMD and instrument playing among guitarists and pianists in the last 12 months. It was composed of three parts: questionnaire of musculoskeletal disorders, clinical examination and video-analysis of instrument playing. The study surveyed 401 guitarists and pianists without specified instrumental level. 19 of these participated in the clinical examination. Effect of deep foot massage on postural stability in patients with spinocerebellar ataxia

Introduction: Spinocerebellar ataxia (SCA) is currently defined as a group of diseases with progressive cerebellar syndrome, which manifests distinctively by ataxic stance and gait. In patients with SCA, deterioration of postural control occurs due to changes of pathophysiological degenerative nervous system. Control of postural stability is significantly influenced by somatosensory afferentation of soft tissue of foot. The work objective is to determine the effect of deep foot massage on postural stability in the group of patients and to compare obtained results with experiments without deep foot massage. Methods: Seven patients of the Clinic Rehabilitation at the Faculty Hospital, Prague Motol (average age 51.9 ± 13.9 years, 5 men, and 2 women) with SCA and clinical signs of cerebellar ataxia were participated in this study. All patients were examined using posturography before and after application of deep foot massage. Measurements were conducted on a firm surface, foam pad with open and closed eyes. The effekct of one training of postural stability in patiens with spinocerebellar ataxia

We followed the immediate effect of sensorimotor training on postural stability in patients with spinocerebellar ataxia (SCA). We have tried to explain the possible mechanisms of adaptation to change COP, motor learning and neuroplasticity in the field of neurodegenerative changes in the cerebellum. Research Methods: A total of 10 patients with spinocerebellar ataxia (6 men, 4 women) aged 34-71 years, with an average disease duration of 11.6 years participated in the measurement of postural stability. First, we measured the patients with SCA before and after a 15-minute sensorimotor-training exercise, then this group underwent another day of the same measurement without sensorimotor training. As a second control group of 10 healthy subjects served a similar age, who also underwent sensorimotor training. Then we compared the results of posturographic measurement. Results: In the group of SCA patients, sensorimotor training was found significantly lower values in the monitored parameters LENGTH (t stat = 3,537731 > t krit = 2,262157), VELOCITY (t stat = 3,537731 > t krit = 2,262157) and energy to sustain COP (t stat = 2,715085 > t krit = 2,262157), measured on a foam pad with eyes closed. Slackline as a therapeutic possibility to influence postural stability

Objectives: To summarize knowledge about a control of posture, a balance training and a testing on balance platform. I investigate in experimental part of my thesis if slackline training will affect stabilography and differences in quantitative parameters will be found in a training group or a control group. Hypothesis: A specific slackline training positively affects stability which was measured in posturographic selected tests done by the training group. Methods: We test postural stability in control (10 persons) and training (10 persons) group in selected tests of stability. We used a balance platform for testing. The data were statistically analysed. Results: 4 weeks of intense slackline training result in improvement of postural control. That is support by the data of stabilometric examination. The results were just below statistical significance (p > 0.05). Conclusion: The results testify that slacklining can improve postural control in healthy subjects. The results were on the boundary of statistical significance in our intervention group.
Swimming as an indicator of normal and altered development of the rat

Swimming is an inborn reflex in rats. It represents a complex motor behaviour, which changes significantly throughout the postnatal developmental period. In the current study, development of swimming behaviour was studied using a group of rat pups aged between P5 - P21. A protocol for assessment of swimming in the rat was developed based on literature studies and the results of our own observation on rat swimming. The experiments were conducted using a tank 50x15x30cm, filled with water at 24řC, in which each pup is left for 1 minute or less, if it sinks to the bottom. Swimming was assessed using the following developmental markers: sinking, floating, nose position and swimming movements of the limbs. To study the effect of GABAB receptors ligands on the development of swimming behaviour, three groups of pups aged between P12 - P32 were injected intraperitoneally either physiological solution or 3 mg/kg GABAB receptors antagonist CGP 46381 or 3 mg/kg GABAB receptors agonist baclofen, daily between P7 and P11. Effect of whole body vibration therapy on postural stability in Charcote-Marie-Tooth.

Objective: Charcot-Marie-Tooth disease (CMT) is the most common hereditary motor and sensory neuropathy. Genetic mutation causes neurological disorder of peripheral nervous system. Diseases most common signs and symptoms are distal muscle weakness, foot deformities and somatosensory defect. One of the most notable consequences of the disease is balance disorder. The goal of this study was to ascertain impact of one-off application of vibration therapy on postural stability in CMT and referential group. Method: There were 16 patients diagnosed with CMT that participated in the research, 15 of which finished all procedures (average age 44.3 years, age ranges from 25 to 57 years, 12 woman and 3 men). Referential group was composed of healthy people of similar sex and age. All probands underwent measuring on Balance Master before vibration therapy, immediately after and 30 minutes later. The therapy take place in standing position in the Power Plate, therapy lasted for 5 minutes. Results: A significant improvement of Endpoint Excursion, Maximum Excursion, Movement Velocity and Sway velocity parameters was established after applying one-off vibration therapy in group of patients diagnosed with CMT. Evaluation of posturography after Vojta therapy and active video game in children with cerebral palsy

Aim: The aim of this thesis is to evaluate and compare the effect of therapy Vojta method (VRL) and active videogames Nintendo Wii at children with mild celebral palsy. Methodology: The research sample included eight patients with cerebral palsy. Patients were divided into two groups. In the first phase of the research, one group underwent therapy VRL, one group received treatment Nintendo Wii. In the second phase, the group exchanged (each patient underwent both types of therapy). There were posturography measurements parameters at the beginning and at the end of each phase on the Balance Master instrument. Results: The effect of therapy Vojta method was observed in tests Limits Of Stability, Rhytmic Weight Shift and Walk Across, but the resulting values were not statistically significant. After treatment with the use of Nintendo Wii the significant change was detected in test Rhytmic Weight Shift. The difference in the effect of therapy was observed in Test Limits of Stability and Rhytmic Weight Shift. Conclusion: It appears that VRL therapy has an effect on the speed component of motor performance, while therapy with Nintendo Wii affects the lateral shifting of the weight in patients with cerebral palsy.
Segmental evaluation of standing posture using accelerometers

Master's thesis "Segmental evaluation of standing posture using accelerometers" is focused on assessment of relationships between body segments during quiet standing with varied sensory afferentation according to CTSIB test. Theoretical part reviews prevailing theoretical work regarding posture, stability, equilibrium and balance and discusses the possibilities of posture assessment with accent on accelerometry and jerk based metrics. The objective of experimental part is to discover influence of sensory afferentation on following aspects: acceleration and jerk of body segments (head, thorax, sacrum and shins); linear correlation of acceleration between body segments; linear correlation of jerk between body segments; jerk magnitude ratio of individual segments. The results demonstrate increasing jerk of sacrum and shins with more demanding postural condition. Jerk linear correlations vary from 0,05 to 0,3 which contradicts the simple inverted pendulum hypothesis of body movement during quiet standing. Higher correlations are registered in the standing on the foam surface compared to firm surface. This relation is accentuated while standing with closed eyes. Active Wii video game system as a form of balance training - pilot study

Introduction: The theoretical part of the thesis summarizes findings from foreign literature on virtual reality and its use in rehabilitation, as well as the possibility of the benefits of active video games as part of balance training, specifically gaming console Nintendo Wii. The practical part is a pilot study to evaluate the effect of balance training in patients with cerebral palsy using this gaming console. Methods: The study included five patients with cerebral palsy, three in the form of spastic hemiparesis, two in the form of spastic triparesis. Patients were aged 8-18 years. Therapy took place on the Nintendo Wii, selecting five games from cd/dvd Wii Fit Plus. Patients underwent a total of 8 treatments with a frequency of once a week. One therapy lasted 20-30 minutes, according to success in individual games. The effect of therapy was evaluated using the Balance Master, specifically through tests Weight / Bearing squat (weight distribution of the lower limbs), mCTSIB (modified test balance sensory interaction) Limits of Stability (CoG sway to the borders of support base), Rhytmic Weight Shift (fast active transfer of CoG), Walk Across, Tandem walk and Step/Quick Turn (walking with a turn).
