The aim of the practical part of thesis was to identify and analyze the administration of drugs by nurses to patients in a healthcare facility. Methodology: In both the 1st (pilot) phase and the 2nd phase of data collection, the direct observation method was used. An observer (the study investigator) monitored morning, midday and evening oral drug administration during 30 working days in December 2023 and in January 2024 in four wards of one hospital. Introduction Medication errors are a very common problem that can arise at different levels and can lead to disruptions in the safety and effectiveness of the provided healthcare. The analysis of medication errors can help to improve the quality of patient care. Objective The aim of the practical part of this thesis was to describe and analyse the observed errors in the administration of medicines by a nurse in a healthcare facility. Methodology For the thesis, the method of direct observation by a trained team (pharmacist, nurse) was chosen and errors in the administration of drugs by the nurse to hospitalized patients were recorded. The aim of this thesis was to analyze the administration of drugs to patients hospitalized by nurses in one of four studied health facilities in the South Bohemia region in the surgical, internal medicine and aftercare wards. One is periprosthetic joint infection (PJI), whose risk can be decreased by antibiotic prophylaxis (AP). The aim of this effort was to describe trends in endoprosthetic surgeries of the knee and the hip, their complications and options of diagnostics in the theoretical part. New oral targeted drugs have been used in clinical practice to treat CLL. The aim of this thesis was to measure adherence to oral targeted drugs, analyze pharmacotherapy and identify drug-related problems (DRP) and their management. Methods: The cross-sectional pilot study ran from September 2022 to March 2023 in University Hospital Hradec Králové. Patients ≥ 18 years old, with CLL diagnosis treated with ibrutinib, acalabrutinib or venetoclax were included. The objective of this thesis was to analyze ICA focused on blood pressure (BP) measuring in given pharmacies and management of drug-related problems (DRP) associated with used pharmacotherapy. Methodology: The data was collected in community pharmacies in Poděbrady, Česká Lípa, Praha, Moravská Třebová and Kralovice in 2012-2016 period. Data colectlion consisted of 4 parts. First part was structured interview between CL and the researcher. Author: Veronika Duchoňová1 Tutor: Josef Malý1 1 Department of Social and Clinical Pharmacy, Faculty of Pharmacy in Hradec Králové, Charles University Introduction and aim: The aim of the thesis was to revise the medical documentation in the aftercare health facility, to classify the identified drug related problems (DRP) and to describe the chosen DRP by the SAZE principals. Methodology: The data collection for this thesis took place in ONLP in the time period from the 27th of August to the 30th of August 2019. The aim of the auditors was a revision of phamacotherapy of all 76 patients. A complete electronic and printed medical documentation was provided. The DRP were identified based on the information about a patient and described by the SAZE principals. The DRP were reported in the electronic database according to the modified PCNE classification V5.01. All obtained data was exported to the Microsoft Office Excel in the form of tables and processed by freqency analysis. Six case studies with the detailed description of the patient and the pharmacist's analysis by the SAZE principals were demonstrated. In clinical practice, direct oral anticoagulants (DOAC) are used as an alternative to warfarin to prevent thromboembolic events in patients with AF. The study aimed to analyse the statements of patients with FiP along with data from medical records focusing on aspects related to the use and management of DOAC, including devices and tools to improve adherence to treatment, patients' level of awareness of DOAC and other therapeutic agents used. Methods: The prospective study was conducted from 2020 to 2022 at the University Hospital Brno. Patients aged ≥ 18 years with AF on DOAC therapy (dabigatran, rivaroxaban and apixaban) were included in the study.
