Information systems in medicine with a focus on scientific information in clinical practice and biomedical research

The dissertation deals with information systems in medicine with a focus on scientific information. Topics are treated from the perspective of a user, whether a scientist or a doctor in the clinical practice. Attention is therefore devoted to information sources, search tools, the ability to communicate and share scientific information, and current methods of analytical and synthetic processing of published medical literature, both at the level of value-added information services and at the level of automatic text processing. -- The topics mentioned above are divided into separate chapters. A large portion of the text devoted to new information sources is added to an overview of traditional biomedical databases. Further, current trends in biomedical information searching are described. Next, current possibilities of communication and sharing of scientific knowledge via the Internet and applications and services of Web 2.0 ar listed. Particular attention is paid to scientific information for clinical desicion-making support. -- The dissertation newly presents a comprehensive classification of biomedical information resources supplemented by illustrative examples. Theory and praxis of medical professionals' information behaviour

There were huge changes in spreading and use of scientific medical information in last fifteen years due to development of new information and communication technologies and changes in medicine as well. The research described in this thesis adresses possible changes in information behaviour of physicians and other healthcare workers both abroad and in the Czech Republic. Foreign studies were reviewed and summarized. A survey method was used for the Czech research, the survey was spread with the help of medical libraries and information centers both in print and online version. 228 valid answers were collected. Several factors were supposed to influence information behaviour (age, sex, affiliation, healthcare occupation, job description), but only the influence of affiliation and occupation was confirmed. There were significant differences between information behaviour of physicians and nurses and between research workers and hospital employees. It can be said that information behaviour of healthcare workers is influenced by the character of their occupation, that entails responsibility for life and that demands both a lot of information and lifelong education. Terminology Analysis of the International Classification of Nursing Practice

The presented work contains an analysis of the terminology system of the International Classification of Nursing Practice (ICNP) in the context of medical / health informatics. The author analyses the roots of difficult penetration of information technologies into health-related applications: it is the difference in the view of the nature of information between healthdisciplines and information technologies as well as the subjective nature of health as the subject of health disciplines. As a possible solution to overcome these differences that has its solid base in husserlian phenomenology and that is compliant with Peirce's semiotics, the author shows semantic matching of health-related data with algorithms that process these data. Semantic matching, however, must be based on a reliable and accurate description of meanings of concepts and relations - which brings the need for a sound terminology system. The work describes the origins, the history and the structure of the ICNP system of nursing terminology. The consequences of the use of multiaxial hierarchy for the first versions of ICNP are analyzed and the circumstances of the Czech translation are explained. Information behaviour of the medical profession in relation to the modern library and information services in health care

Introduction. Library information systems, with its application to the computerization of health systems, belong to the group systems serving the medical education and research and they contribute by their functions to the accessing to high quality electronic and traditional information resources in health field. Due to the development of information and communication technologies there are barriers to their use by professional users. Questionnaire survey, focused on selected target groups of health professionals specializing in preparation for the medical profession, verified study habits of respondents and their knowledge of the existence, organization and design of library information services in health care field. A separate part of the research represents a section dedicated to services of the National Medical Library in the light of new web technologies for personalization of searching through the system MEDVIK. Methods. Research method of user behaviour of health care worker was employed and used on a selected target group of health professionals specializing in preparation for the medical profession. Problematika elektronické zdravotní dokumentace a její rešení v projektu Elektronických zdravotních knížek v České republice

Cílem této práce je analýza možností vedení zdravotnické dokumentace v elektronické podobě a perspektiv v tomto oboru. Dále je popsán současný stav elektronizace zdravotnictví v České republice a technické možnosti uživatelů v ČR a v porovnání s tím je uveden i současný způsob vedení zdravotnické dokumentace. Z českých projektů je podrobně prozkoumán projekt Elektronických zdravotních knížek IZIP, jaké jsou jeho možnosti a funkce, jak je využíván českými lékaři a pacienty a co brání jeho plošnému rozšíření. Z evropských projektů je jako příklad uveden zdravotnický portál v Dánsku a e-karty v Německu. Dále jsou zmíněny mezinárodní evropské projekty, na kterých se podílí i Česká republika, jejichž cílem je zkvalitnění lékařské péče a výzkum v různých oblastí zdravotnictví.
