The influence of exercise applied by bicycle ergometer on spirometric data measured on-line during exercise. Detection of exercise induced bronchoconstriction and dynamic hyperinflation

Bachelor Thesis "Influence of exercise on spirometric data measured on-line during exercise. Detection of exercise - induced asthma and dynamic hyperinflation" sumarizes knowledge about genesis, detection and prevalence of exercise-induced asthma. It further deals with the occurance of dynamic hyperinflation and its effect on lung mechanics. Practical part sets forward oportunity of observing an alteration in spirometric data and dynamic lung volumes during exercise and after its cessation in order to detect dynamic hyperinflation and exercise - induced asthma by showing practicle examples. It shows an alternative approach to exercise bronchial challenge testing. It further discusses the diference between reaction of spirometric data of non-asthmatic subjekt and asthmatic subjekt. The conclusion of this work is that the change in spirometric data, provided by modified bronchial exercise challenge, differs between asthmatic and non-asthmatic subject. The theoretic part describes the characteristics of AS as the main representative of spondylarthritides, including its diagnosis and testing methods. It also contains an overview of current approaches and strategies of a treatment. It focuses on biological treatment with TNF-α blockers. The practical part of the thesis illustrates the findings on the example of a patient with AS treated with biological therapy. It evaluates the subjective feelings in response to treatment and compares the results of treatment with standard therapy (i.e., without biological treatment) on another patient under otherwise similar conditions.
Respiratory physiotherapy - part of a comprehensive treatment of Duchenne muscular dystrophy patients

Duchenne muscular dystrophy is a progressive muscular disease. This serious disease attacks not only the musculoskeletal system, but also the respiratory system. Respiratory system is one of the most influenced systems. The risks of respiratory problems increase with deteriorating condition of the patient due to muscle weakness. This work is focused on treatment of respiratory system. The aim of treatment is early prevention and solving of complications. These complications include not only scoliosis and cardial problems, but mainly inefficient cough, night hypoventilation, irregular night breathing and failure of respiratory system even during the day. The aim of the respiratory physiotherapy is to keep the right functionality of lungs for the longest time and to choose the correct method of the rehabilitation following the patient's condition deteriorating. Interventions suitable for the patients with the DMD are techniques that leads to increasing lung volume, manual and mechanical techniques of assisted coughing, and noninvasive ventilation. NIV is one of the preferred methods of treating nowadays. Another method is continual invasive ventilation and ventilation with tracheostomy. The bachelor's thesis deals with the adenoid hypertrophy problems. It is divided into a theoretical and a practical section. The theoretical one consists of a general and a special part. The theoretical section summarizes anatomical knowledge related to upper airway, describes some of the respiration physiology aspects and it also deals with the most frequent upper airway pathologies. The special part goes into details about the adenoid hypertrophy problems. It describes symptomatology, diagnostics and therapy possibilities. It widely investigates physiotherapy possibilities, breathing re-education in particular. The practical part contains outputs of a questionnaire research which took place at the ORL children clinic of the 2nd Faculty of Medicine, Charles University. It gathers information about each aspect of the problems. Dysphagia failure of the upper esophageal sphincter in patients after stroke

Stroke is one of the most frequent causes of dysphagia, a serious symptom triggered by affected coordination of the swallowing muscles or limited sensation. This bachelor thesis focuses on providing a compendium of literature concerned with dysphagia and demonstrating how the symptom occurs after cerebrovascular accident. The first part discusses the swallowing process and stroke from anatomical, physiological and neurophysiological standpoint. Second part presents a closer view of the upper sphincter of esophagus, additional healt problems caused by dysphagia and possibilities of alimentation and brings up a set of the most frequently used diagnostic methods. From chapter 4 and 5 on, the thesis describes methods and technics that can be involved in the treatment of patient with swallowing difficulties and casuistry of a patient suffering from serious dysphagia caused by cerebrovascular accident.
The problems of Pediatric Thoracic Surgery from the view of respiratory Physiotherapy

The bachelor thesis is focused on pediatric surgery in thoracic area and its dilemma. The theoretical part contains knowledge about anatomy, physiology and kinesiology of developing organism. It is followed by overview of the most common diagnoses indicating thoracic surgery, describing each of those surgeries individually. The biggest issue of entire pediatric thoracic surgery is the risk of postoperative complications, which is also discussed in the thesis. This part is then followed by discussing means of physiotherapy. The informations are divided into several parts, each describing means of physiotherapy used in different time sequences. Practical part contains general means of examination and physiotherapic approach depending on patient's age. The last part is devoted to casuistry, which clearly demonstrates issues related to pediatric thoracic surgery from the physiotherapeutic point of view.
Monitoring of the Respiratory Functions in Patients after the Rib Cage Surgery according to Nuss

Objective: The goal of the thesis "Monitoring of the Respiratory Functions in patients after the Rib Cage Surgery according to Nuss" was to confront the present knowledge about respiratory functions and the strength of respiratory muscles of patients with inherited pectoral deformity pectus excavatum, to compare it with my own measuring and to find out about their development after the surgical correction according to Nuss. Methodology: There were 15 patients being monitored (13 men and 3 women), who underwent spirometric and bodypletysmographic examination and the occlusal mouth pressures were determined before the surgery. The same pulmonary function tests were run in the course of 4 to 10 months (7.1 months on average) on all the patients, the control determination of mouth pressures is available for 13 of them. Results: In the entry tests, compared to adequate values the patients in this file had increased residual lung capacities (RV 142%, p=0.001; ITGV 116.2%, p=0.004; RV%TLC 133.8%, p=0.002; ITGV% 116.1%, p=0.001) at the expense of statistically lower vital lung capacity (VC 81.9%, p=0.0004) and inspiratory reserve capacity (IC 84.2%, p=0.0003), while the total lung capacity remained unaltered (TLC 99.7%, p=0.877). Diploma thesis "Body schema in Patients with Chronic Obstructive Pulmonary Disease" is an experimental work which deals with body schema perception. As a part of the thesis there are particular tests of body schema included, which examine difference between control group and group of patients with chronic obstructive pulmonary disease. Research part of the work deals with current findings about body schema and with findings about chronic obstructive pulmonary disease. There is more detailed analyze of musculoskeletal questions in patients with chronic obstructive pulmonary disease included. In an experimental part of the thesis there are comments on results of the body schema tests. Affecting parameters of respiratory gas exchange by use of incentive spirometers

introduction: The theoretical part of the thesis briefly analyzes the anatomical, kinesiological and physiological aspects of breathing, pursues the issue of examinations of lung functions and also includes a chapter which applies to incentive spirometers. The experimental part of the work examines the impact of three-week training with trainer Coach2 on parameters of respiratory gas exchange at rest and during the exercise in healthy subjects. methods: Both output and control tests included spirometry and spiroergometry examination. The experimental group consisted of 10 subjects, control group consisted of 9 subjects. results: Our results show a significant influence especially on an increase in FVC, decreased respiratory rate at submaximal intensities of exercise and increased breathing rate at maximum load and also an increase in tidal volume at all load levels. Minute ventilation at maximum load increased by an average of 15.17 liters. The control group did not show any improvement. conclusion: It appears that training with trainer Coach2 might be effective in influencing certain ventilatory parameters at rest as well as during the exercise.
