Using means of physical therapy to influence spasticity from the physiotherapist 's point of view

The theme of the bachelor thesis is using means of physical therapy to influence spasticity from the physiotherapeutist's point of view. Theoretical part describes upper motor neuron syndrome and spasticity as one of its symptoms. Further it states methods of evaluation and options of influence of spasticity including latest research methods. Separate chapter is dedicated to the physical therapy, its impact on human organism and means used to treat spasticity. In the practical part the influence of cryotherapy on spasticity of upper limb of two patients after brain injury is examined. Efficiency is measured by Tardieu scale and goniometry. Main contribution of the thesis lies in gathering and analyzing of present findings, mainly on the field of physical therapy. Kinect - utilising in the occupational therapy

The theme of this thesis is the use of Kinect in training of movements which are used in Activities of Daily Living (ADL) of patients with brain damage. Teoretical introduction is dedicated to analysing possible neurophysiological mechanisms which could be applied in rehabilitation with using of Kinect such as biofeedback, repetitive movements, complex paterns of movements. Training response was tested on the pilote sample of two patients: one with right-handed spastic hemiparesis after hemorragic stroke, the other with kvadruparesis, disability of low to medium-size of both superior limbs, more severe right, after craniocerebral trauma with multiple contuses of brain. Training was applied in series of six training days during several weeks. Development of results was expressed quantitatively using arithmetic averages achieved in particular days and variability was expressed by value their average absolute deviation. To compare development of training achievements and development of abilities fine and gross motor skills of patients was used Jebsen - Taylor Test. In the thesis are discused appropriate attributes of virtual reality to utilise in the occupational therapy and also possible problems and contraindication of some diagnoses simultaneously. Possibilities of physiotherapy in patients with cerebral palsy in the treatment of spasticity in the ankle

This thesis deals with the extensive subject cerebral palsy in children, focusing on spasticity in the ankle joint, and the possibilities of influencing it in a conservative and operative manner. The thesis further deals with the choice of methods and physiotherapy means with regard to the type of disease and the patient's cooperation. The picture of the disease differs for each patient, thus also the type of rehabilitation, the procedure and use of therapy. One limitation to achieving optimum results with this disease is often mental retardation of varying degrees, which prevents the patient not only from understanding the implementation of the therapy, but also the motivation for implementing it. It is therefore necessary to proceed individually, with patience and sufficient time given to the care for the patient.
Use of orthotic aids for children with cerebral palsy

Cerebral palsy (CP) is a complex of disorders in the development of posture and ability to move which cause limitation of activity. It is an unprogressive disease, which begins in the prenatal, perinatal, or postnatal phase of a neonate. Although the disability does not progress, complex rehabilitation may improve the clinical state. There is a difference between the spastic and the non-spastic type. The major aim of this thesis is to map the possibilities of orthotic care for children suffering from CP and to evaluate the effect of physiotherapy with the use of chosen aids. Three probands suffering from hemiparetic CP participated in the research. The aim is to answer these research questions: What are the possibilities of orthotic care for children with cerebral palsy? What changes will occur in the postural behaviour of probands due to the physiotherapy with the use of orthotic aids? The theoretical part is concerned with the issues of CP. The disease is described in a general way and divided according to the time of beginning and type. Furthermore, the diagnostics and therapy are explained. It is also concerned with spasticity and its symptoms, mechanisms, diagnostics and therapy. An overview of orthotic aids, and experimental methods like posturography and surface electromyography are covered as well. The practical part deals with a qualitative research which was completed by case interpretations of three schoolchildren suffering from hemiparetic CP. The thesis includes an initial and a final kinesiologic analysis and a description of applied physiotherapies. Furthermore, it includes the results of posturogtaphy and surface electromyography focused on the orthotic aids used by the probands. The research took place in Centrum Arbida in České Budějovice and in the physiotherapy centre at the Faculty of Health and Social Sciences of The University of South Bohemia in České Budějovice. This thesis may be used as a comprehensive overview of the given issues and possibilities of orthotic therapy for children suffering from CP.
Utilisaation of Physiotherapeutical Procedures and Methods in Patients with the Multiple Sclerosis

The title of thesis: Utilisaation of physiotherapeutical procedures and methods in patients with the multiple sclerosis. Abstract of thesis: This work is divided into theoretical and practical part. The content of the firt part is a study of theoretical works related to multiple sclerosis. The main chapters are devoted to history of multiple sclerosis, the definition of why and how the disease arises, the types of disease, clinical presentation, diagnosis, therapy, desce incidence and complications that can occur to people with this disease. Finally, I looked at various physiotherapeutic procedurs and methods related to this disease. In the practical part of the thesis , a qualitatice research metod - cause study - was used. The research was conduted at the Center for demyelination disorders (MS center) at the Charles Square. The test set consisted of two probands with a diagnosis od cerebro-spinal multiple sclerosis, specifically of the relapsing - remitting form of disease. These diagnoses were determined by the attending physician of the probands. During the physiotherapy I used elements of the Bobath koncept. Effect of hippotherapy at clients with sclerosis muiltiplex.

Hipotherapy is a complex therapy, broaden and recently very sought after rehabilitation method, which is being more and more recognized by experts for its broaden scene. It is a complex therapy thanks to involving medicine, psychology, pedagogy and also a social sphere. That's why is this method indicated at many different levels of disablement. It's being used with pacients with children's cerebral palsy, diverse level of cerebral dysfunction and multiple sclerosis. And it's the therapy's effect on multiple sclerosis that this thesis paper discusses. It also applies to brain and spinal traumas, scoliosis, back pain, orthopedic defects, cardiovascular disease, degenerative muscle disease, obesity, as well as it provides help at emotional and mental aspect, e.g. children with behavioral disorders and many others. At hipotherapy is being used a three dimensional (multidimensional) horse motion. Horse's spine moves up - down, forward - backward, right - left. Imitating a mechanisms of human walking, which leads to relaxation of spasticity. Impulses are transmitted onto back which brings interaction of impeller and back muscles, stand up pose, and posture interference. Spasticity and selective voluntary motor control assessment possibilities will be summarized. Classification systems of the gross motor functions (levels of locomotion) are also mentioned. The main objective is to elaborate selective motor control ability scale (IP) which is newly proposed in this study. The need for such instrument and its feasibility for clinical practice intend to be confirmed. The level of locomotion of 25 children with spastic cerebral palsy is compared with IP outputs, spasticity measurement outputs using Ashworth scale respectively. Evaluation of sensation in hands area in patients with multiple sclerosis

Title: Evaluation of sensation in hands area in patients with multiple sclerosis Objective: Summarize findings about multiple sclerosis and sensation in the hands area. Find suitable testing methods and use them to evaluate the quality of deep sensation in the hands area in patients with multiple sclerosis. Method: With help of selected tests to evaluate the quality of deep sensation in the fingers and palms area in selected probands with multiple sclerosis. Compare the results depending on the current spasticity and condition of the patient. Results: There were founded, that in people with multiple sclerosis the quality of vibrating and discriminating sensation and stereognosis in the hands area is reduced. The quality of statesthesia, kinesthesia, vibrating and discriminating sensation isn't directly linearly dependent on the current spasticity. The quality of stereognosis is directly linearly dependent on the current spasticity. Keywords: sensation, multiple sclerosis, spasticity
