Title: Case study of physiotherapeutic care of a patient after ischemic stroke Objectives: To evaluate a case study of a patient after ischemic stroke and create a theoretical basis for the given area applying academic literature. Summary: This paper concerns the area of stroke and is divided into two parts. The general part involves the characteristics of stroke, its division, symptoms, risk factors, prognosis and diagnostic methods. Further, it talks about individual phases of the healing process, applied physiotherapeutic approaches and information about additional care after hospitalization. The special part is a detailed case study of a patient after ischemic stroke. This part was created based on a practise in the Military University Hospital in Prague during 6th - 31st January 2014, which allowed me to lead my own therapeutic units with a patient under professional surveillance. Title: Case report of physiotherapeutic care of patient with diagnosis ischemic stroke. Objectives: The main aim of this thesis was processing casuistic of ischemic stroke diagnosis patient, and obtain theoretical knowledge about this illness. Methods: Bachelor thesis is divided into general and special part. General part is focused in theoretical knowledge describing the anatomy of the vascular supply to the brain, epidemiology and neuroplasticity of the brain. In the second half of the general part is with greater detail processed the issue of ischemic cerebrovascular disease, its classification, clinical features and risk factors. The final section is dedicated to spasticity and rehabilitative care for the patient. A special section was created on the basis of the current practices since 20 January 2014 till 14 February 2014 at Vršovická zdravotní a.s.. Its content is a close up casuistics reports of patient after ischemic stroke with left-sided symptomatology. Results: The patient's condition improved during rehabilitation. There has been a positive effect on stability while standing, walking and overall self-sufficiency of the patient. Keywords: Stroke, ischemic, spasticity, rehabilitation care.

Title: Case study of physiotherapeutic care for a patient with the diagnosis of a tribal ischemic cerebrovascular accident. Objectives: This work aims at processing the casuistry of a patient with the diagnosis of a tribal ischemic cerebrovascular accident at summarizing the theoretical knowledge about cerebrovascular accidents and subsequent rehabilitation, at suggesting a therapy, observing its progress, and at evaluating the effect of therapeutic influence on the patient's condition. Methods: I acquired the practical sources for processing this bachelor's paper during my professional practice, which took place from 6th to 31st January, 2014, at the rehabilitation in-patients' ward of Vršovická zdravotní a.s. The paper is composed of two parts - the general and the special ones. The general part of the paper deals with the theoretical knowledge about cerebrovascular accidents, their classification, possible causes of their origin, and the anatomy of the cerebrovascular system. The main theme of the general part of my bachelor's paper is about rehabilitation after a cerebral ictus. Standardized examinations of spasticity in physiotherapy

Title: Standardized examinations of spasticity in physiotherapy Abstract: This bachelor's thesis is aimed at giving an overview of the most commonly used standardized examinations of spasticity in physiotherapy and their comparison. It deals mainly with the best known methods of examinations that are used in practice. The thesis is divided into two parts - theoretical and practical. In the theoretical part, muscle tone and its regulation and spasticity as one of the upper motor neurone syndrome symptoms are specified. Moreover, the methods of examination (Ashworth scale and its modifications, Tardieu scale and five-step clinical assessment of spastic paresis according to Gracies) are presented and described in detail there. The practical part contains two case reports of patients after a cerebrovascular accident. Examinations and evaluations of spasticity, including Modified Frenchay scale or walk tests, which are described in the theoretical part, are applied to patients. The main contribution of this thesis is to create a comprehensive comparison of standardized examinations of spasticity through studies and my personal experience. The Use of Physiotherapy in Spasticity Management

Title: The Use of Physiotherapy in Spasticity Management Objectives: The purpose of this thesis is to provide a summary of present findings on spasticity, with particular regard to its pathophysiological mechanisms and clinical manifestations, and to present an overview of medical and physiotherapeutic approaches applied in its treatment. Furthermore, it aims at making a research in physiotherapeutic methods and techniques used for reduction of spasticity. The thesis shall present relevant studies, provide their comparison and critically evaluate the effect of methods and techniques examined in them. Methods: This thesis is a descriptive analysis based on a literature review. Results: Based on the research findings, it may be concluded that physiotherapy has proven to be effective in reducing spasticity. However, it is not possible to determine whether the techniques to reduce spasticity are more effective than techniques without this primary purpose. The examined studies have shown that the choice of physiotherapeutic practices to reduce spasticity was not influenced by disease etiology. Moreover, the same techniques and methods were applied in pediatric and adult patients, regardless of diagnosis. Title: Therapy of spasticity with botulinum toxin and assessment of the effect by Tardieu Scale Abstract: This paper discusses paralysing effect of botulinum toxin which is used to treat spastic muscles in contemporary medicine. The paper focuses on spasticity in patients with disorder of the central nervous system caused most commonly by stroke. At first it deals with theoretical knowledge about physiology of movement, then describes active ingredient and explains the mechanism of its effect. It dedicates a lot of attention to follow-up rehabilitation as an integral part in the management of spasticity. This paper is based on two case histories. A practical part describes in detail patient's condition before and after application of botulinum toxin. A progress of the effect was assessed during periodic meetings. These instruments were used to assess: Tardieu Scale of spasticity and Frenchay arm test. In connection with the application of botulinum toxin is emphasized active co-operation of the patient. It is an essential precondition for positive results in the management of spasticity. Objective: Create the case report of a patient after ischemic stroke and elaboration of physiotherapy treatment schedule in the form of an after stroke patient case study. Methods: This work was done within uninterrupted practise at Central Military Hospital in Prague from 6. 1. - 31. 1. 2014 at the Department of Physical Medicine and Rehabilitation. The work is divided into two section. The first section includes basic characteristic of the illness - vascular anathomy, distribution of stroke, clinical presentation, diagnosis and treatment. CASE STUDY OF PHYSIOTHERAPY TREATMENT OF A PATIENT AFTER STROKE Objectives: Theoretical analysis of after stroke anamnesis and elaboration of physiotherapy procedures. Methods: This bachelor thesis was done with the patient's consent within practice at Regional hospital Kladno at the Rehabilitation Department from January 6th to January 31st 2014. The thesis is divided into two main parts - theoretical and special part. The theoretical part includes basic characteristics of the illness, ethiopathogenesis, incidence, diagnostics, treatment and options of physiotherapy methods. The special part contains the after stroke patient's case concept, processes and the effects of his therapy. Results: Patient's condition, daily activities, movements, spatial orientation, muscular strength and physical condition have improved after the therapy.
