Use of Nurses' Competencies in Preventive Care of a Patient with Ischemic Heart Disease

Ischemic heart disease belongs to the most frequent diseases in developed countries. I have chosen this topic not only because I am interested in this issue but also because I work at emergency ward of internal medicine and majority of patients there suffer from this disease. There are two parts in my thesis {--} the theoretical and the research part. The theoretical part deals with heart anatomy and physiology, with epidemiology and aetiology, with risk factors, with classification, clinical picture, diagnostics, with treatment and prevention of ischemic heart disease. Further, the thesis survey educational process, roles, mission, function and competences of a nurse. The first objective of the thesis was to find out the awareness of nurses´ competences in the area of secondary preventive care in patients with ischemic heart disease. The second objective was to find out, whether nurses are able to apply their competences in the area of secondary preventive care in patients with ischemic heart disease. The third objective was to find out involvement of management (head nurses, departmental nurses, shift nurses) in the secondary preventive care in patients with ischemic heart disease. The fourth objective of the thesis was to find out whether nurses use their knowledge of secondary prevention in patients with ischemic heart disease. Four hypotheses were set for the above mentioned objectives. H1: Nurses are aware of their competences in the area of secondary preventive care in patients with IHD. H2: Nurses are able to apply their competences in the area of secondary preventive care in patients with IHD. H3: Shift nurses, motivated by head nurses, are more involved in the secondary prevention in patients with IHD. H4: Nurses use their knowledge in the area of secondary prevention in patients with IHD. A questionnaire was made up to achieve the objectives and to prove or to disprove the hypotheses of this research. The questionnaire contained 24 questions {--} closed and semi-closed. The research sample was formed from nurses from South Bohemian and central-Bohemian regions. 377 questionnaires were handed out. 242 questionnaires returned and 230 of them were applicable to the research. The first, second and fourth hypotheses were proved. Shift nurses are more involved in secondary prevention in patients with IHD, however, not on the basis of motivation by head nurses, thus, the fourth hypothesis was not proved. I also revealed that nurses do not have sufficient knowledge in the area of secondary preventive care in patients with IHD. On the grounds of these results we created mental maps on ischemic heart disease and its prevention. These maps might enhance nurses´ knowledge and improve the life of patients with ischemic heart disease.
Issue Standards for Treatment of External Childrens Genital from Birth to 3 Years

Hygienic care is one of the most critical pillars of care for child´s health. Learning the right hygienic habits in childhood and keeping them throughout the lifetime is of vital importance for human health. Unfortunately, information about the hygiene of external genital of children is often inconsistent, both among the parents and healthcare workers. The thesis deals with the issue of treatment of external genital of children from birth to three years of age, while the main emphasis is put on education of mothers. The objective of the thesis was to determine, whether the nurses know their competences in treatment of the external genital in newborns, babies and toddlers and whether the nurses are able to apply the competences in full. The third objective of the thesis was to map the awareness of parents of how to treat external genital of children from birth to three years of age. Three hypotheses were established. The first hypothesis: Nurses know their competences in treatment of external genitals of newborns, babies and toddlers. The second hypothesis: Nurses, who know their competences in treatment of external genital of newborns, babies and toddlers, are unable to fully apply them. The third hypothesis: Parents of newborns, babies and toddlers do not have sufficient information about the proper hygienic treatment of external genital of their children. The quantitative research used two anonymous questionnaires, while one questionnaire was designed for nurses working with children from birth to 3 years of age. The other questionnaire was created and intended for mothers of children in the age category from birth to three years. The questionnaire research was performed on the territory of the South Bohemian region. The first hypothesis: ``Nurses know their competences in treatment of external genital of newborns, babies and toddlers.{\crq}q has been confirmed. The second hypothesis: ``Nurses, who know their competences in treatment of external genital of newborns, babies and toddlers in treatment of external genital of newborns, babies and toddlers, are unable to fully apply them.{\crq}q has been rebutted. The third hypothesis: ``Parents of newborns, babies and toddlers do not have sufficient information about the proper hygienic treatment of external genital of their children.{\crq}q has been confirmed. Based on those findings we have decided to develop an educational standard for nurses about the intimate hygiene of girls and boys from birth to three years of age and an educational material for treatment of external genital for parents of young children. We believe that the materials will improve the awareness of this issue and result in a better quality of care for children.
Nurses´ co-operation with other members of medical staff as an integral part of high-quality treating care

There are many miscellaneous people co-operation at health services. Diversity is not only in professional knowledge but in nature of work and attitude to work of every individual as well. There is different work content in every field but the work is hard and professional everywhere. It wouldt be possible to provide high-quality care service without nurses and other team members co-operation.
Nurse status today and in the future

Nowadays the status of a medical nurse represents a fairly actual topic. It is a profession where many changes had to be undertaken during a few years beginning with scholars education up to a graduation of a nurse and their responsibilities. A nurse not only plays the role of a caretaker and a helper but is also an individually thinking professional who has received a proper education and has his or her competence bound by the jurisdiction. The theoretical part focuses on the status of a nurse from the sociological viewpoint, on its development from a historical perspective, on changes taking place in the present and stretching to the future and on the trends we might expect in this profession. The practical part contains information obtained by the research which focused on the perception of the status of a nurse by patients, nurses and physicians. The aim of this graduation research was to question nurses in the Czech Republic about their contentment with their current status and explore their future visions, furthermore to obtain opinions on the topic from both patients and physicians. The H1 hypothesis was: nurses are not content with their current social status, was proved correct. The H2 hypothesis was: the ideas of a nurse about his or her future status in the society are positive was proved correct. The H3 hypothesis was: the assessment of the status of a nurse by a patient is negative, wasn´t correct. The H4 hypothesis: physicians do not follow the current status of a nurse, correct. The H5 hypothesis was: physicians do not display any interest in following the status of a nurse in the future was correct.
Language education in the nursery school

The diploma work considers the positron and importance of language pedagogy in present preschool educational system, follows its impact on the content in separate fields of preschool educational program, language goals deduced from acquired knowledge, types of activities in progress in nursery school. Based on the relationship of deduced goals to different language levels, sets language competence margins to preschool age children as well as methods used for their development. Practical section investigates the level of language education (with respect to language levels) of preschool age children in a ``small-type{\crqq} nursery school class.
The development of communicative competence of the preschool-aged children in context of curricular reform of Czech education

This bachelor thesis introduces the problems of the communicative competence development in the Frame work educational programme for pre-school education (below RVP PV) and describes how to include this issue in the school educational programme of the kindergarten, where I work. The main objective is to create an experimental programme to support pre-school children communication skills development followed by subsequent verification of its effectivity. The theoretical part of the thesis deals with the general introduction into communication and communicative competence development issues in RVP PV. It explains the concept of communication, defines the social and pedagogical communication and outlines the methods of communication. There are also mentioned those areas of RVP PV, which are directly connected with the communicative competence. The practical part concentrates on concrete possibilities to develop the communicative skills development of pre-school children. It results from the realization of an experimental programme itself and evaluates its effectivity in practice. In the next part there is assembled knowledge gained from pedagogical workers about methods and possibilities of communication development in kindergartens. It concentrates on knowledge of factors which can influence the communication development.
Methods and techniques of active teaching in Czech language on the middle school

The aim of this diploma thesis is to find out the influence of methods and techniques of active teaching on the level of essay-writing in the basic school. The theoretical part describes chosen methods and techniques of active teaching within the context of the educational process. An experiment was carried out in the practical part and it verified the influence of these methods on the development of essay-writing potential of the pupils. The popularity of the chosen methods and techniques of active teaching was being researched by a questionnaire survey. The methods of active teaching were discovered to facilitate the success rate of the essay-writing and a statistically significant increase in the number of sentences and syntactic pairs was found in the experimental group.
Team work and its evaluation on the example of software testing

This diploma thesis focuses on team work during a process of software testing and is divided in three main parts. The first part of this thesis presents main issues, goals and philosophy of software testing. This part is also aimed at core competency of software tester. The second part is aimed at definition of team, teamwork and its advantages and disadvantages. There are discussed ways how it can be measured and evaluated. The third part contains an analysis of real software development project. Significant barriers of team efficiency related to team communication and information flows within and between teams are identified and discussed. Finally there are shown some ways how they can be faced.
Project and programme manager's certfication as a guarantee of quality and competence

This diploma thesis deals with project management certifications. It focuses on project and program managers and explores their relationship with the certifications and possible links to quality management. The main objective is to make a cross-section area of certificates in the Czech market, create a a summary of available certifications and perform research, which chats certifications of companies carving out on bussines within the Czech ICT market. The thesis is divided into several parts. The first part is based on the theoretical foundations and was set up as an introduction to project management. Futher are particularly detailed project management competencies like an object of knowledge and skills, that are required to perform manager's work. The second part contains a list of available certificates, including requirements for obtaining. Contribution of this part is just the list created, which concentrates all the important details about particular certificates, including assignment of certificates for the recommended work positions. The last section describes the research implementing, which focuses on finfing out the plans of project and program managery for obtaining new certificates. Research deals with the analysis and synthesis of responses received and creates new knowledge, that reveal possible future trends. Research is unique in dealing with the Czech ICT market, which has not been studied on this topic yet. Diploma thesis presents a comprehensive view at the certifications, but also focuses on a narrow area of the ICT market, where possibilities of certificate usage are observed and confrontation of different views is offered.
Analysis of the training process in the international business

The thesis examines the issue of training and development from a theoretical point of view, then from a practical point of view in two selected international companies (operating in the fields of banking and logistics). The thesis also discusses a problems of European structural funds, especially the request for education projects of an Operational program of the CR "Lidske zdroje a zamestnanost". The theoretical part describes the phases of the educational process: the identification of training needs, education planning, implementation phase and is greatly accentuated phase measurement and monitoring of the effectiveness of training. This work examines new trends in education and development of employees in 2010. The practical part of the bank monitors and evaluates the process of systematic training and use of competency models, practical part in the logistics proposes educational activities under conditions of low input costs - notably through training internal trainers.
