Possibilities of influencing blood pressure in spinal cord injury patients during verticalization

Orthostatic hypotension is a common problem for individuals with spinal cord lesions, especially in the acute period after spinal cord injury. The first part of this study deals with the theoretical analysis of the pathophysiology of cardiovascular control after spinal cord injury, the definition of orthostatic hypotension and its inclusion within the classification of the syncopal conditions. An important part of the theoretical part is the management of orthostatic hypotension, which brings non-pharmacological therapeutic approaches, and marginally pharmacology, to decrease the severity of this condition. The experimental part focuses on testing of a group of probands with spinal cord injury (11 quadriplegics) on two different types of tilt tables according to the same protocol. The first tilt table is a standard; the second one is Erigo, tilt table with integrated robotic mechanism, which moves the legs to simulate walking. The parameters, such as blood pressure, heart rate, oxygen saturation, and the occurrence and severity of pre-synkopal symptoms, were compared. Options of respiration physiotherapy in patients at a spinal unit

Bachleor thesis "Possibilities of the Respiratory Physiotherapy in Spinal Cord Unit Patients" deals with respiratory problems of the patient with spinal cord injury. It debates problems of the patient in general, introduces the classification of spinal cord injuries and also informs about a clinical manifestation of the injuries. It includes a comparsion of respiratory kinesiology in healthy man to a man with spinal cord injury. It also informs about common respiratory complications. The work mentions the assessment of the patient. It also shows the most frequent techniques of respiratory physiotherapy which either prevent or suppress respiratory complications. The swimming effect on the body condition at patients with transversal spinal cord lesions

The theory part contains information regarding spinal cord injuries, possible complications, therapy, influence of swimming and water environment on the physical constitution, muscle functions and respiratory functions. Information about the methods of water movement therapy for physically handicapped are a part of the theoretical part as well. The aim of the practical part was to evaluate the influence of regular swimming on the physical condition and respiratory functions of spinal lesion impaired patients.
Lifestyle of persons with disability of motor funtions - personal statements.

Any disability can bring disadvantages in many aspects of human lives. This often affects physical, mental and social areas of life. It depends on many circumstances and factors to what extent and at what intensity disability influences the way of life. The research part of my diploma thesis confirms this fact. Not only the type of disability and its prognosis, the personality and his/her ability to handle difficult situations in life, but also the quality of social relationships and the strength of the social safety net are important factors. Irreplaceable role in caring for people with a disability has comprehensive rehabilitation, which helps not only to improve the health condition, to restore job skills and self-sufficiency, but mainly supports a successful return to the home environment as well as to the regular everyday life.The aim of my thesis was to learn about individual life stories of people struggling with a disability of physical functions, whether due to an illness, injury or another disability, resulting in movement disorders, and to find out how the disability affects their lives. I tried to analyze specific aspects of life and social consequences of disability (coping with disabilities,value orientation, interpersonal relationships, future plans, self-realization). The base of my work is the holistic, i.e. bio-psycho-socio-spiritual approach, I respect the individuality of each individual, his/her attitude to life with a handicap (disability).
Possibilities of application fitness in physioteraphy in patients with spinal lesion

The number of persons who suffered spinal cord injuries, as well of physical therapists who are interested in exercising in a fitness centre and in other fitness activities. This fact results in integration of these activities into physical therapy programmes and therapeutic stays intended for persons with spinal cord lesion. Incorporation of fitness activities into physical therapy programmes is common particulary in the USA. The aim of this thesis was to study the influence and contribution of fitness activities for muscle disbalances in the area of the torso and for the posture with patients suffering from spinal cord lesion and futher to identify the possibilities of incorporating fitness activities into physical therapy programmes for wheelchair bound persons. With respect to the aim of thesis defined in this manner, I have summarized the fact on spinal cord lesions in relation to torso muscle disbalances with spinal cord lesion in the theoretical part of the theses. Subsequently, I have tried to collest the existing facts on fitness, including the definition of fitness, the importance of physical activities and muscle strengthening for wheelchair bound persons. I have also focused on basic principles, terms and exercises and I have analysed its incorporation into the exercise programmes for wheelchair bound persons. In the practical part of the thesis, I have used the qualitative casuistic-based research method. The research was carried out in the Rehafit Physical Therapy Centre in Prague. Two test persons were selected: a man and a woman with diagnosed spinal cord lesion. The woman has been diagnosed with Th11-12 paraplegia, the man has been diagnosed with C4-5 tetraplegia. Both patients with spinal cord lesion are of similar age and suffered the spinal cord injury approximately at the same time. With the observed patients, there has been an influence of muscle disbalance detected in the area of torso, inprovement of posture and stability of seated positions, increase of muscle power, as well as improvement of overall physical conditions. The results of the research show that incorporation of fitness activities into physical therapy programmes in the period following the period of termination of the intensive physical tehrapy has a undeniable imporatnce for patients with spinal cord lesion, particularly for affecting the posture and muscle disbalnces.
Activization of the paraplegick and kvadruplegick patient

The activation of a paraplegic, quadruplegic patient is an in important part of the nursing care for the patient handicapped in this way. By a good and timely activation we achieve a better co-operation of the patient during the rehabilitation, practicing self-sufficiency; the patient communicates better with us and he co-operates. The activation is manifold; as for the hospital activation, the most known elements are books, television, radio, talks, co-operation with the family, internet. Other possibilities of activation are spinal units, rehabilitation center and e.g. the ambulant Paraple center providing the services in the following aid to the handicapped patients.
Health and social problems of people after spinal cord injury

The aim of this work is to find out whether the people after spinal cord injury are motivated enough to get work and whether emplozers accept their demand of part-time working hours because of their health condition. The method of this work was research, the form of questionnaire. The research was carried out in Prague, The Union of Paraplegics in the Center Paraple. 82 out of 95 asked respondents took part in the survey. The thesis consists of two parts. The first part is theoretical. It presents brief anatomics description of spine and central nerve system, the change of health condition after spinal cord injury as well as the psychosocial trauma of people wirh spinal cord lesion. Next, the teoretical part deals with komplex rehabilitation-medical, social, pedagogical and working one. The thesis focuses especially on the working rehabilitation, referring to one elementary human rights-the right for work. It deals with the problem of the handicapped people unemployment and its characteristics. Next the chapter of working rehabilitation deals with the legislation of handicapped people unemployment, especially the new unemployment law n. 435/2004 Sb. Nowadays we can no longer ignore international documents concerning the employment of handicapped people, mentioned in the final chapter of the theoretical part. The second part of thesis is practical. It includes the resultsof both, the survey and the questioning by phone. The data are processed percentualy and graphically. every graph has its own description. Then the results are discussed. People after spinal cord injury are motivated to work. Yet, there are many obstacles to the employment of these people, which may result in their desillusion and demotivation to work. One of such obstacles is e.g. the fact that potential employers do not accept their demand of shorter working hours. The final part of the thesis includes the conclusion, the list of sources and supplement.
