Urinary Incontinence After Delivery
JANDOVÁ, Markéta
The bachelor thesis is focused on the topic of urinary incontinence in women after childbirth. The thesis is composed of a theoretical and practical part. The theoretical part of the bachelor thesis describes the anatomy and physiology of the urinary tract and micturition. Furthermore, attention is paid to urinary incontinence, its classification, risk factors, diagnosis, and incontinence aids. Finally, the treatment of urinary incontinence is briefly summarized. One goal was set for the practical part of the bachelor thesis. To determine the frequency of urinary incontinence in women who have had their last childbirth 2-15 years ago. Five hypotheses were formulated for this goal. H1: Women who suffered from obesity during childbirth (body mass index equal to or greater than 30.0) do not suffer from urinary incontinence after childbirth more often than women who did not suffer from obesity during childbirth. H2: Women who gave birth to a child with a birth weight over 3,500 g suffer from urinary incontinence more often than women who gave birth to a child with a birth weight under 3,500 g. H3: Women after vacuum extraction or forceps delivery do not suffer from urinary incontinence more often than women who did not give birth using these methods. H4: Women who had birth injuries do not suffer from urinary incontinence more often than women who did not have any injuries during childbirth. H5: Women who gave birth more than 8 years ago were less educated about urinary incontinence by the midwife than women who gave birth less than eight years ago. The goal was achieved through a quantitative research survey using their own online questionnaires. The questionnaire contained 21 questions. Sixteen questions were closed, three semi-closed questions where it was possible to mark more answers including "other:" and two open questions. Statistical processing of all questionnaires took place in Microsoft Excel. The research sample consisted of 393 women who gave birth to their last child more than two years ago and no more than fifteen years ago. Of the total of 393 respondents, 217 women reported experiencing urinary leakage and 176 women reported no urinary leakage. The research survey showed that more than half of the respondents, i.e. women 2-15 years after childbirth, experience urinary leakage. It was also found that women are very poorly educated about this issue by the midwife. Of the total of five hypotheses, only hypotheses H1, H3, and H4 were confirmed. Hypotheses H2 and H5 were not confirmed. No dependence was demonstrated for any of the hypotheses. It was found that the level of information was the same for women who gave birth less than eight years ago and for women who gave birth more than eight years ago, and it was very low.
The role of midwife in care of women during a delivery
ŠATALÍKOVÁ, Nikola
This bachelor thesis deals with the role of a midwife in caring for a woman during childbirth as well as women's knowledge of the midwife's duties. The aim of the bachelor thesis was to find out how women perceive the role of a midwife. The bachelor thesis is divided into a theoretical part and a practical part. The theoretical part describes the definition of a midwife and gives a brief description of the development of this profession in a historical context. Basic information is presented about childbirth and the individual stages of labor. Subsequently, each of the chapters describes the care that is provided to the mothers by the midwife, from the reception to the birth itself to the period after birth. Furthermore, the theoretical part of the bachelor's thesis is divided into subchapters, which provide information on individual stages of labor, including information on the midwife's duties including accompanying the expectant mother to childbirth, the alleviation of labor pains as well as attending to birth injuries. The practical part consists of methodology, results and discussion. The practical part of the bachelor's thesis was carried out by means of a qualitative research survey using semi-structured interviews. The research group consisted of eleven women who underwent vaginal birth and were asked to assess the care that the midwife provided during labor. Interviews with informants were conducted with their prior consent, electronically recorded, literally transcribed and subsequently analyzed. The interviews took place from January to March 2020. To maintain the anonymity of the women, the informants were marked as Ž1 to Ž11. For the analysis of the obtained data, 2 categories and 6 subcategories were compiled. According to the above-mentioned goals of the bachelor's thesis, two research questions were determined for the research part. The first research question looked at how women perceive the role of a midwife during childbirth. Research has shown that most women perceive a midwife as a person with great responsibility and experience who accompanies a woman in childbirth. Most of the informants also stated that they perceive the midwifery profession as a mission. The research shows that the most important roles of a midwife during childbirth are the roles of a communicator, an educator and, finally, serving in a supporting role of an expectant mother during childbirth. The second research question examined the opinion of women regarding the competencies of midwives. Research has shown that women are well informed about the competencies of a midwife during pregnancy, childbirth and the puerperium. Based on the research survey, it was found that most women were aware of the profession of midwife before giving birth. None of the women report any significant negative experience with a midwife. On the contrary, most women were pleasantly surprised by the willingness and diligence of the midwife. The informants stated that they felt safe in the presence of the midwife and fully trusted her. The knowledge gained during the compilation of the bachelor's thesis may help women who do not have enough information about what care a midwife can provide and what actually falls within her competencies. The result of the bachelor's thesis is an information leaflet, which describes basic competencies of a midwife and the care provided by the midwife to mothers in individual stages of delivery.

Options of physiotherapeutic treatment of these problems are discussed as well. The practical part of the thesis is supplemented with a case study of a pregnant patient, who was preparing to the delivery during pregnancy with the goal of preventing perineal injury. The case study is completed with anamnestic data collection and analysis of kinesiology of the patient in pregnancy and after the delivery. A therapeutic plan is a part of the case study as well. The thesis is supplemented with the results of executed questionnaire survey of 49 patients after vaginal delivery.
Multiple Sclerosis and Pregnancy
Hanulíková, Petra ; Rob, Lukáš (advisor) ; Binder, Tomáš (referee) ; Krofta, Ladislav (referee)
Introduction: Multiple sclerosis (MS) is an autoimmune disorder of the CNS that typically affects young women of childbearing age. Due to the international published data, safety for pregnant women with MS can be assumed. However, no study has been published in the Czech Republic to address the effect of MS on pregnancy and perinatal outcomes. Objective: Analysis of the clinical course of patients with MS during and after pregnancy, and perinatal outcomes in comparison with healthy pregnant women. Methods: A single centre prospective observational study in the period 2006-2015 was conducted. Complete data from 68 patients with MS were analyzed (85 deliveries) and were compared with a control cohort of 68 age- and parity - matched healthy pregnancies. Results: The comparison between relapse rate and EDSS before, during and after delivery showed no statistically significant difference (relapse in 7.4% and 9.5%, EDSS 1.27 and 1.49). Perinatal outcomes were comparable in both cohorts. The weight of newborns differed by 159 g, (p = 0.295), complications in pregnancy were represented in 16.2% in the group with MS and in 27.9% in controls (p = 0.295), caesarean section was performed in 16.2% in patients with MS and in 23.5% of controls (p = 0.629), 79.4% of patients with MS were breast-feeding. In the MS...
Maternal and fetal microchimerism.
Hynková, Marie ; Tlapáková, Tereza (advisor) ; Pačes, Jan (referee)
Microchimerism is defined as the presence of small amount of genetically distinct cells in one individual. It most often arises during pregnancy and breastfeeding. It can also arise from blood transfusion or organ transplantation. During pregnancy there occurs a bidirectional migration of cells through the placenta between the mother and the fetus. Cells which are transferred during pregnancy and breastfeeding can persist in the offspring until adulthood. During breastfeeding, a big number of immune cells is transferred to the offspring via mother's milk. These cells protect the offspring against pathogens and are involved in modulation of its immune system. Fetal cells persist in mother organism even decades after giving birth and can have long-lasting effect on mother's health condition. These fetal cells can help to regenerate mother's damaged tissues, but they can also contribute to the development of serious autoimmune diseases.
