Interpretation of electrocardiograms in the resuscitation department and in emergency pre hospital care

(v AJ) 1) introduction - introduction to the problem The diploma thesis on the interpretation of electrocardiograms by nurses and paramedics in the resuscitation department and in urgent pre-hospital care includes a theoretical summary of ECG pathologies, as well as addresses the issue of correct interpretation of electrocardiograms in relation to gender, age, workplace experience, and suggestions for improvement. 2) goal formulation, methodology The goal of the diploma thesis was to map the level of knowledge in interpreting electrocardiograms by nurses and paramedics in the resuscitation department and in urgent pre hospital care, depending on age, gender, length of experience, and job placement, with a focus on ECG interpretation. For the empirical part of the thesis, which deals with answering predetermined goals, a quantitative research method was chosen, conducted in the resuscitation department and emergency medical services. (v AJ) Introduction to the Issue: The diploma thesis deals with ethical and legal issues related to the provision of cardiopulmonary resuscitation, particularly the use of advance directive in intensive care units. Although advance directive have been part of legislation in medical practice for several years, they are not widely used. However, this document is crucial as it protects patient autonomy in situations where the patient cannot express their wishes and preferences. The use of this document in medical practice raises questions, especially regarding its content, accessibility, and adherence. Objective: The main goal of this thesis is to identify the ethical and legal aspects of cardiopulmonary resuscitation, specifically in relation to patients advance directive. The study aims to understand how non-medical healthcare personnel perceive these aspects in intensive care units. Role of nurse in organ donation in pediatric patiens

The topic of my diploma thesis is the issue of nursing care in pediatric patients before consumption of organs at the resuscitation department. I consider this topic socially very topical because the number of transplant candidates on transplantation lists is constantly increasing. Since child transplant candidates are the most vulnerable group, I have focused on child organ donors. The theoretical part criticizes the concept of death and threat, including examination methods that lead to brain death. I present the activities of the Coordination Center of the Transplant and Coordi- nated Coordinator in a donor program that seeks to proliferate the transplant program. An important component is legal legislation and ethical aspects, which are an integral part of the donor system in the Czech Republic. Study materials I searched in the databases NLK, SVKKL, Medvik, Embase, PubMed, Wiley Online Library, Medscape. The role of nurse from physician perspective in pharmaceutical care

Introduction to the problem: Pharmacotherapy is a very complex and specific topic. Many of healthcare professionals, including nurses, are involved in pharmacotherapy management. Although this field is ensured by various legal standards, some competences of nurses are not clearly defined. The primary aim of this research was to analyse the current role of the nurse in pharmacotherapy out of a physician's perspective. Methodology: The data as a part of the multicentric project was collected in the form of semi- structured in-depth interviews with selected physicians also called key-informants. The obtained data were analysed by open coding followed by categorisation. Finally, SWOT (strengths, weaknesses, opportunities, and threats) analysis was used to assess the opportunities and threats, which are related to determination of the ideal role of the nurse in drug administration. Results: Current practice shows that nurses are performing activities beyond their competences. Therefore, I consider legislative changes in this area appropriate. At many departments is not cooperation with the clinical pharmacist well established, moreover in some cases doctors are not interested in it. The perioperative nursing care for the patients during the gynecologic surgery in the lithotomy position

The topic of the work is nursing perioperative care of a patient who underwent a vaginal hysterectomy and vaginal plastic surgery for prolapse of the uterus in a lithotomy position at our workplace. Prevention of pressure injuries related to immobilization during the surgery is a fundamental nursing task of perioperative nurses. Small workplaces are not always adequately equipped. Lithotomy position is associated with a number of serious complications. Although the observed complications are rare and occur in surgical procedures longer than three hours, each operation can be complicated for many reasons and the operating time can be multiplied. Then, insufficient equipment and thus failure to provide adequate prevention can lead to, for example, pressure ulcers, peripheral neuropathy, compartment syndrome and damage to the patient. The main goal of the work was to use literature to present recommended evidence-based nursing procedures that ensure patient safety in the operating room when positioning for surgery and help to prevent pressure injuries. The care of the airways in the patients hospitalized at the unit care in view of patients

Introduction:The management of patients with a tracheostomy tube is still current. It brings stress situation not only for the patients but also staff who care for them. This issue occurs across all disciplines. The aim of this study was to determine how they feel patients at the care of the tracheotomy while hospitalization of the intensive care unit. Methodology: The research was carried out during 2015 and 2016 at the surgical intensive care unit. The group of respondents consisted of patients with established tracheostomy tube. 80 pieces were distributed questionnaires, 73 patients responded return reached 91.25%. Results: The investigation showed that the comprehensive care of a tracheostomy for patients burdensome and unpleasant. Everyone clearly stated as the most annoying and most exhausting bother them forced to cough. 90,41% of patients reported that their aspiration meets at regular intervals to be able to prepare this fact. All of the respondents perform respiratory physiotherapy and 67.12% felt that they benefit. 95.89% of respondents suffer from a sense of social isolation. 82.19% would welcome the presence of a psychologist at the ICU. Alternative methods in managing postoperative pain in living donor renal transplantant recipients

The main criteria for managing good postoperative care include the effective and appropriate choice of pain therapy. Experiencing pain is a purely individual feeling for each of us, so it is very important to have adequate and sufficient analgesia during this period. Modern management of post-operative pain therapy uses preparations of various pharmacological groups that potentiate and thereby more effectively reduce pain. By this mechanism, we can reduce the total dose of analgesics given, and more particularly, reduce the dose of opioids that have a higher risk of side effects. In this project i want to show how the analgesic catheter could be useful as alternative possibility in algorithm therapy of postoperative pain. Analgesic catheter enable continual local anesthetics administration, exactly 0,5% bupivacaine, which is operating in surgical wound. Main goal of the study is to find out and verify if analgesic catheter is method which provides continual analgesia strong enough to results in lower use of opioids. The specialty of Intensive Care deals with acute conditions from all medical branches within the entire age spectrum of adults. This field has its own distinctive characteristics, which determine the character of not only medical personnel communication with families of hospitalized patients, but the overall quality of nurses' cooperation with relatives. In Czech professional literature in the area of intensive care there is minimum publications on this issue up to date. The task of the current thesis is to map a view of paramedical staff (nurses) on the process of communication and cooperation with family members of patients hospitalized in the intensive care units. The empirical part of the research is processed as a qualitative research by the method of semistructured interviews with nurses from the Intensive Care workplaces. As the result of this work we found inadequate professional readiness of nurses in the fields of communication strategy and approach to the families of patients hospitalized in the intensive care environment. Early mobilization in critically ill patient

Introduction: The most of critical ill patients is immobilised for short or longer period of time. This depends on the character of their illness, medication or another organ support. As it has been proved during critical illness and forced immobilisation there is an significant reducing in muscle strength, exactly in 3 to 11 percent with every another day of immobilisation (46.) It is necessary to say that early mobilisation is not standardised approach in Czech medical environment. Aim of tthe work: The main target of this diploma is to figure out whether early mobilisation in critical ill patients means any significant difference in their functional status in comparison to those given standardised physiotherapy. Another target of this research is to set up safe and structured protocol for leading early mobilisation and to determine whether exercising with cyclo ergometer is linked to higher risk of side effects and to state the most probable reasons for delayed or not performed rehabilitation. ÚVOD: Syndrom vyhoření relativně závažný problém projevující psychosomatickým onemocněním, emocionální vyčerpaností jedince. Vzniká především pomáhajících profesí, jako důsledek chronického stresu, ztráty motivace svém povolání CÍLE Cílem diplomové práce bylo zjistit syndrom vyhoření ovlivňuje sexualitu všeobecných sester pracovištích neodkladné péče v kultních nemocnicích. Výzkumné šetření probíhalo pomocí dvou dotazníků. První standardizovaný dotazník zjišťuje míru syndromu vyhoření, jde o českou verzi Shironomovy škály (Shirom, Melamed, 2006 in Ptáček 2013) Druhý dotazník je zaměřen na xuální funkce ženy (Mellan in Kratochvíl, 1999 . Výzkumný vzorek tvořily všeobecné sestry ženy, pracující na odděleních neodkladné péče v nefakultní nemocnici. VÝSLEDKY vyhodnocení dat bylo použito 62 dotazníků. Byly stanoveny čtyři pracovní hypotézy H1: Předpokládáme, že rozvojem syndromu vyhoření bude klesat žen pracujících jako všeobecná sestra odděleních neodkladné péče v nefakultních nemocnicích pohlavní touha.
