Funkční poruchy hybnosti (FPH) známé také pod pojmy konverzní, psychogenní nebo disociativní motorické poruchy jsou komplexní a klinicky heterogenní jednotkou zahrnující řadu motorických fenotypů a přidružených non-motorických symptomů. I přes potenciální reverzibilitu mají často chronický průběh a nepříznivou prognózu. FPH byly odbornou společností léta opomíjeny a řazeny do jakési šedé zóny na neuropsychiatrickém pomezí. Navzdory významným pokrokům v posledních dvou desetiletích, zůstává tato problematika v mnoha ohledech nedostatečně objasněna a pochopena. Cílem zde prezentovaných studií bylo vyplnit některé mezery v poznání neurofyziologických a klinických korelátů u FPH. První studie se zaměřila na vyšetření robustního neurofyziologického fenoménu prepulzní inhibice (PPI). Nalezení alterací v PPI u pacientů s FPH současně s předchozími nálezy snížené PPI u jiných funkčních syndromů podpořilo současný neurobiologický model, který navrhuje, že se vzniku všech typů funkčních symptomů účastní jednotné neurální mechanismy. Narušená PPI dále zdůrazňuje odlišení funkčních poruch od faktitivní poruchy nebo simulace. Druhá studie měla dva hlavní cíle: ozřejmit vztahy mezi motorickými, non-motorickými symptomy a kvalitou života a také prokázat existenci eventuálních klinických subtypů uvnitř této... Functional Movement Disorders (FMD), also known as conversion, psychogenic, or dissociative motor disorders, are complex and clinically heterogeneous group of diseases. In addition to motor symptoms, FMD are manifested by a number of comorbid non-motor symptoms. Despite the potential reversibility, these disorders still have an unfavourable prognosis and are associated with a low quality of life. Regardless of the growing interest of both clinical and research communities over the last two decades, this issue remains insufficiently understood in many aspects. The aim of the presented studies was to fill some gaps in the knowledge of neurophysiological and clinical correlates in FMD. The first of the presented studies explored the robust neurophysiological phenomenon of prepulse inhibition (PPI) in FMD. The alteration in PPI, along with previous findings of a reduced PPI in other functional syndromes, supported a possible unified pathophysiology across functional neurological and somatic syndromes with implications for diagnostic classification and development of novel biomarkers and treatments.
