Souhrn Cíl: Vyhodnotit průnik ceftriaxonu, penicilinu a cefotaximu do likvoru přes zánětlivě změněnou hematoencefalickou bariéru. Najít korelaci průniku do likvoru v závislosti na laboratorních markerech zánětu. Materiál a metody: Hladiny β-laktamových antibiotik byly vyšetřeny u 24 pacientů. U všech nemocných byl hodnocen průnik antibiotik do likvoru (kvocient likvor/sérum Kl/s, likvor n = 24, sérum n = 24) a korelován s laboratorními markery zánětu (CRP, fibrinogen, počty neutrofilů v likvoru). Kultivačně pozitivní likvory (n=13) byly vyšetřeny na baktericidii. Vedle toho byly stanoveny (n = 28) sérové hladiny antibiotik před a po podání. Celkem bylo vyšetřeno 76 vzorků biologického materiálu. Výsledky: Průměrné hladiny ceftriaxonu v séru před podáním (31,2 mg/l ± SD 12,29) a po podání (300,0 mg/l ± SD 125,9) i penicilinu před podáním (14,3 mg/l ± SD 15,8) a po podání (110,38 mg/l ± SD 63,9) byly statisticky významné - (ceftriaxon p = 0,000156, penicilin p = 0,0216). Hodnoty testovaných parametrů zánětu byly celkově vyšší u pacientů s Kl/s ≥ 0,1 v porovnání s pacienty s Kl/s < 0,1: počty segmentů v likvoru (p=0,0112), CRP (p=0,00192), fibrinogen (p=0,0178) u ceftriaxonu. U penicilinu korelovaly hodnoty CRP a počty segmentů v likvoru s výší Kl/s. Žádný z markerů zánětu (nebo jejich kombinace) spolehlivě... Penetration of β-lactam antibiotics into cerebrospinal fluid and its relationship to inflammatory markers during invasive bacterial infection Aim: The penetration of β-lactam antibiotics (ceftriaxone, crystalic penicillin G - potassium salt and cefotaxime) into the cerebrospinal fluid (CSF) was estabilished and the correlation between the penetration and laboratory markers of inflammation was evaluated. Materials and methods: Levels of β-lactam antibiotics in serum and CSF were determined in 24 patients (76 samples). In all patients (n = 24) the CSF/serum ratio for antibiotics was calculated and it was correlated with laboratory markers of inflammation (C-reactive protein, fibrinogen, and CSF numbers of neutrophils). Besides, CSF was examined by a modified bactericidal test in patients (n = 13) with positive bacterial culture. In 14 patients (28 samples) levels of serum antibiotic concentration, both before and after its administration, were measured. Results: Serum levels of ceftriaxone before and after administration (31.2 mg/l ± SD 12.29 and 300.0 mg/l ± SD 125.9, respectively) differed with a statistical significance (p = 0.000156).
