Uvod a cíle: Cévní mozkové příhody (CMP) jsou ve vyspělých zemích třetí nejčastější příčinou smrti (Chyba! Nenalezen zdroj odkazů.) a nejčastější příčinou invalidity obyvatelstva ve věku nad 60 let. Pro volbu optimálního léčebného postupu má zásadní význam rychlé zhodnocení rozsahu nevratně postižené mozkové tkáně a mozkové tkáně potencionálně zachranitelné. Cílem této práce je ověřit možnosti předpovědi jádra mozkové ischémie u pacientů s akutní CMP pomocí automatického zpracování zdrojových dat výpočetní tomografické angiografie (CTA-SI) a nekontrastního vyšetření výpočetní tomografi (NCCT) vytvořením perfused blood volume (PBV) map celého mozku u pacientů s akutním ischemickým iktem. Zjistit korelaci takto vytvořených map s rozsahem výsledného infarktu. Dalším cílem práce je určit optimální kvantitativní kritickou hodnotu PBV pro identifikaci jádra ischémie v akutním stadiu ischemického iktu. Soubor a metoda: Pomocí prototypového software byla CTA-SI a NCCT data zpracována k vytvoření PBV map u 37 pacientů s akutním ischemickým iktem, u kterých byla angiograficky prokázána rekanalizace uzavřené tepny intravenózní trombolýzou. Tyto mapy byly automaticky porovnány s rozsahem dokončeného infarktu na kontrolním NCCT. U pacientů s dokončeným infarktem byla provedena pixel-by-pixel analýza dokončeného... Background and Purpose: Ischemic stroke is the 3rd most common cause of death and most common cause of permanent disability in developed countries. Rapid diagnostic work-up with reliable assessment of infarcted brain tissue and potentionally salvageable brain tissue is critical for acute stroke management. The first aim of this study is to asses delineation of infarct core in acute stroke patients using whole brain perfused blood volume (PBV) maps. These maps are calculated by automatic processing from computed tomography angiography source images (CTA-SI) and non-contrast computed tomography (NCCT) data. The second aim is to determine optimal quantitative threshold of PBV for infarct core identification in acute stroke setting. Material and methods: PBV maps were constructed using prototype software from NCCT and CTA-SI data in 37 acute ischemic stroke patients with angiographically proved recanalization after intravenous thrombolytic treatment. These PBV maps were automatically compared with final infarct extension on follow-up NCCT. The anatomic pixel-by-pixel correlation was assessed using Matthew's correlation coefficient (MCC) for infarct core delineation using different critical values of PBV.
