Úvod: Achondroplázie je nejčastěji se vyskytující kostní dysplazií. Je způsobena mutací v genu FGFR3, který se účastní regulace proliferace a zrání chondrocytů na růstové ploténce. Aktivující mutace má pak za následek poruchu enchondrální osifikace a celou škálu příznaků: těžkou poruchu růstu s výrazným zkrácením končetin, makrocefalii s rizikem vzniku hydrocefalu, dysplazii střední části obličeje, spánkovou apnoe, zúžení páteřního kanálu, zvýšené riziko respiračních komplikací a zánětů dýchacích cest. Současný rychlý technologický vývoj vedl k lepšímu pochopení procesů na epifyzální růstové chrupavce a umožnil tak vývoj nových prostředků pro léčbu této poruchy (modifikovaný CNP, rozpustný FGFR3, meclozine). Přesto je současná léčba především symptomatická. U tohoto onemocnění je velmi důležité znát růstový vzorec pacientů, kteří nebyli ovlivnění žádnou růst ovlivňující léčbou (prolongační operace, růstový hormon). Cíle: Cílem práce je zhodnotit růst českých pacientů s achondroplázií, porovnat naše data s celosvětově používanými grafy dle Hortona et al. (1978) a současnými daty dle del Pino et al. (2018). Dalším cílem bylo ověřit přesnost multiplierové metody predikce dle Paley et al. (2005) u českých pacientů s achondroplázií. Materiál a metody: Náš soubor tvoří 79 pacientů (49 chlapců, 30dívek),... Introduction: Achondroplasia is the most common bone dysplasia. It is caused by mutations in the FGFR3 gene, which is involved in regulation of proliferation and maturation of chondrocytes on the growth plates. Activating mutation results in impaired endochondral ossification and a wide range of symptoms: severe growth disorder with limb shortening, macrocephaly with risk of hydrocephalus, mid-facial dysplasia, sleep apnea, narrowing of the spinal canal, increased risk of respiratory complications, and airway inflammation. The current rapid technological development has led to a better understanding of the processes of epiphyseal growth cartilage, thus enabling the development of new drugs for the treatment of this disorder (modified CNP, soluble FGFR3, meclozine). Nevertheless, current treatment is primarily symptomatic. It is very important to have a growth patterns of patients with achondroplasia who have not been affected by any growth promoting treatment (prolongation surgery, growth hormone). Objectives: The aim of this study is to evaluate the growth of Czech patients with achondroplasia, to compare our data with the world-wide used data by Horton et al.
