Obezita je jedním z hlavních rizikových faktorů pro rozvoj diabetu 2. typu, arteriální hypertenze a progresi aterosklerózy. Je doprovázena chronickým zánětlivým stavem tukové tkáně. Tuková tkáň je v současné době považována za aktivní sekreční a imunitní orgán. Zvýšením jejího objemu v důsledku dlouhodobé pozitivní energetické bilance dochází k nárůstu produkce adipokinů s prozánětlivým účinkem. Tyto hormony a cytokiny produkované adipocyty i makrofágy působí lokálně v tukové tkáni a současně vykazují systémové účinky. Mohou se tak různou měrou podílet na vzniku patofyziologických změn charakterizujících metabolický syndrom a vedoucích k rozvoji endoteliální dysfunkce a progresi aterosklerózy. Naopak nevelké množství tukové tkáně u štíhlých jedinců či výrazné snížení jejího objemu u obézních pacientů po redukci hmotnosti je spojeno s nárůstem produkce faktorů s příznivými metabolickými účinky a poklesem produkce prozánětlivých a inulinovou rezistenci indukujících faktorů. Endoteliální dysfunkce a zánět jsou důležitými markery aterogenního procesu a zhoršené prognózy u pacientů s metabolickým syndromem a diabetem 2. typu. Detekce endoteliální dysfunkce je užitečným nástrojem pro výzkum mechanismů aterosklerózy, screening z hlediska budoucího kardiovaskulárního rizika a sledování odpovědi na terapeutické... The metabolic effects of obesity have made this disease one of the most common risk factors for diabetes, hypertension, and atherosclerosis. Adipose tissue is now recognized as an active secretory and immune organ. Chronic inflammation is a common feature of the obesity, and inflammatory signals may originate within visceral adipose tissue as this fat depot expands in response to chronic positive energy balance. Both adipocytes and macrophages within fat secrete numerous hormones and cytokines that have local effects on WAT physiology but also systemic effects on other organs and may markedly contribute to the development of pathophysiological disorders associated with metabolic syndrome. On the contrary, leanness as well as significant weight reduction in obese patients increases production and circulating levels of metabolically beneficial factors and decreases production of proinflammatory and insulin resistance-inducing factors. Endothelial dysfunction and inflammation are important signs of vascular risk and worsened prognosis in patients with metabolic syndrome and type 2 diabetes. Measures of endothelial function remain invaluable for research into disease mechanism and response to new therapies.
