SOUHRN Analyzovali jsme soubor 110 pacientů, kteří byli v pětiletém období (1999-2003) léčeni dekompresivní kraniotomií pro kraniocerebrální trauma. Hodnotili jsme význam jednotlivých faktorů na prognózu pacientů po jednom roce od úrazu. Mezi sledované faktory jsme zařadili věk, klinický stav a jeho horšení po přijetí, přidružená poranění, grafický nález, časový interval od úrazu do operace, operační nález, nitrolebeční tlak a pooperační komplikace. Kromě vlivu jednotlivých faktorů na prognózu nemocných jsme hledali i korelace jednotlivých sledovaných parametrů mezi sebou navzájem. Výsledky: Zjistili jsme, že věk je nezávislý a vysoce významný faktor, který zhoršuje prognózu pacientů, kteří byli léčeni dekompresivní kraniotomií, v našem souboru na hladině významnosti p < 0,0001. Při hodnocení stavu vědomí při přijetí (GCS) jsme významnou korelaci s prognózou nemocných (GOS) zjistili ve skupině pacientů, kteří při přijetí nebyli farmakologicky tlumeni (na hladině významnosti p = 0,0141). U 18 ze 110 pacientů došlo po přijetí ke zhoršení stavu vědomí, což vedlo k indikaci dekompresivní kraniotomie. Nezjistili jsme, že by zhoršení (pokles) GCS měl vliv na prognózu nemocných. Při současném hodnocení šíře a reaktivity zornic jsme v celém souboru (n=110) zjistili vysokou korelaci k prognóze nemocných (GOS) a to... The study presents analysis of group of 110 patients treated for craniocerebral trauma by means of decompressive craniotomy between January 1999 and December 2003. The influence of different factors to the outcome of patient one year after injury was evaluated. Among the factors in view we included age, clinical status and its worsenig after admission, accompanying injuries, time period between injury and operation, finding during operation, intracranial pressure and postoparative complications. In addition to evaluation of particular factors to the outcome of patients we followed the correlation between various factors. Results: perimesencephalic cisterns and thickness of subdural hematoma were evaluated. In the subgroup of patients with subdural hematoma (n=63) it was found, that worsening of the outcome correlated with the thickness of subdural hematoma (p = 0.,025) and with the shift in the CT (p = 0.0001). Correlation between shift or width of cinsterns to the outcome of all patinets (n = 110) was not found. The perioperative measurement of dural gap before the duroplasty is the speciality of this work. In all patients (n = 110) the correlation between dural gap and the outcome was not found.
