[cze][eng] Cushingův syndrom je vzácné onemocnění způsobené nadměrnou sekrecí kortizolu, které je spojené s významně zvýšenou morbiditou a mortalitou. Kromě somatických příznaků též výrazně zhoršuje kvalitu života osob. Cílem práce bylo zjistit, jak Cushingův syndrom ovlivňuje kvalitu života v České republice. Bylo vyšetřeno 24 osob s aktivním Cushingovým syndromem (18 žen, 6 mužů) ve věku 39,6±13,9 let a 75 zdravých jedinců kontrolní skupiny (64 žen, 11 mužů) ve věku 39,4±14,12 let. Osoby byly vyšetřeny pomocí specifických dotazníků kvality života (Cushing QoL) Odpovědi byly statisticky analyzovány pomocí Mannova-Whitneyho testu. Údaje z dotazníkového šetření byly doplněny pomocí narativních a strukturovaných rozhovorů. Při analýze těchto dat jsme postupovali pomocí metody tzv. "Zakotvené teorie". Výsledky prokázaly zhoršenou kvalitu života jedinců s Cushingovým syndromem a to jak v oblasti somatických, tak v oblasti psychosociálních dopadů. Kvalitu života osob s Cushingovým syndromem nejvíce ovlivňuje změna tělesného vzhledu, porušená sebekoncepce, porušení kognitivních funkcí, porucha emocí, změny sociálních rolí, únava, bolest, znalosti o nemoci a léčbě. Tyto faktory se navzájem prolínají. Hodnocení kvality života osob s Cushingovým syndromem má význam pro identifikaci a následné odstraňování překážek,...Cushing's syndrome is a rare disease caused by increased cortisol secretion, which is associated with significantly increased morbidity and mortality. Besides somatic signs and symptoms it is characterized also with impaired quality of life. The aim of this study was to find out how Cushing's syndrome influences the quality of life in Czech Republic. 24 subjects with active Cushing's syndrome (18 women, 6 men) aged 39,6±13,9 yrs. And 75 healthy control subjects (64 women, 11 men) aged 39,4±14,12 were examined using disease specific quality of life questionnaires (Cushig QoL). Answers were statistically analysed using Mann- Whitney test. Questionnaire was combined with narrative and structured interviews. We used so called "A grounded theory" to analyse the data. The results of our study demonstrated impaired quality of life in both, somatic, psychological and social consequences fields. The quality of life of people with Cushing's syndrome was affected mainly by changes on their body, disturbed self-conception, disorder of cognitive functions, emotional disorders, changes of social rolest, fatigue, pain, knowledge about this disease and its medical treatment.
