Neoangiogeneze asociovaná s tumory, neboli tvorba nových drobných cév v nádorové tkáni, představuje patologickou formu fyziologické angiogeneze. I přes svoji složitost a množství pro-angiogenních a anti-angiogenních faktorů v ní zapojených se neoangiogeneze stala slibným cílem v léčbě nádorů, a to hlavně z toho důvodu, že je pro růst nádorů a jejich diseminaci naprosto esenciální. Anti-angiogenní strategie jsou založeny na neutralizaci pro-angiogenních signálních molekul, blokaci vazebného místa jejich receptorů či inhibici receptorové tyrozinkinázové domény těchto receptorů. Pro anti-angiogenní strategii terapie nádorů je dále možno použít endogenní inhibitory angiogeneze, inhibici proliferace endotelových buněk, stabilizaci bazální membrány a v neposlední řadě cílenou disrupci samotné nádorové vaskulatury. Přestože v dnešní době již známe řadu anti-angiogenních molekul, pouze několik jich prošlo všemi fázemi klinického testování a bylo oficiálně schváleno pro léčbu nádorů. Mezi tato klinicky využívaná léčiva patří anti-VEGF-A protilátka bevacizumab (Avastin® ), solubilní VEGF receptor aflibercept (Zaltrap® ), protilátka cílená k VEGFR-2 receptoru ramucirumab (Cyramza® ) a nízkomolekulární inhibitory tyrozinkinázové domény VEGFR sunitinib (Sutent® ) a sorafenib (Nexavar® ). Neoangiogenesis associated with tumours is formation of new blood vessels from pre-existing quiescent vessels in surrounding tumour tissue and it results from pathological employment of normal angiogenesis. Neoangiogenesis became a promising target for cancer treatment in spite of its complexity and many pro-angiogenic and anti-angiogenic factors involved in this process. Anti-angiogenic strategies are based on neutralization of angiogenic ligands, their receptors or inhibition of signalling pathways employed by such receptors. Other potential strategies include upregulation or delivery of endogenous inhibitors, inhibition of endothelial cell proliferation, stabilization of basement membrane and direct disruption of tumour vasculature. Many anti-angiogenic agents have been identified in past several decades but only a few of them were approved for clinical use. Anti-VEGF-A monoclonal antibody bevacizumab (Avastin® ), soluble decoy VEGF receptor aflibercept (Zaltrap® ), monoclonal antibody directed against VEGFR-2 ramucirumab (Cyramza® ) and tyrosin kinase VEGFR inhibitors sunitinib (Sutent® ) and sorafenib (Nexavar® ) belong among approved agents.
