Zlomeniny distálního radia jsou nejčastějšími zlomeninami lidského organismu. Konzervativní terapie u nestabilních typů zlomenin nedokáže zajistit zhojení v uspokojivém postavení. Osteosyntézy zamykatelnými dlahami jsou v současnosti chápány jako metoda volby při operační léčbě zlomenin distální části radia pro všechny typy zlomenin. Otázkou však zůstává uplatnění nitrodřeňových implantátů v těchto indikacích. Cílem studie je optimalizace indikací k jednotlivým osteosyntetickým metodám. Vyhodnocujeme jednoroční výsledky léčby zlomenin distálního radia. Soubor zahrnuje 49 zlomenin léčených konzervativně, 78 zlomenin ošetřených zamykatelnými dlahami s rigidním a s úhlově volitelným směrem šroubů a 35 zlomenin distálního radia stabilizovaných nitrodřeňovými implantáty Targon DR a X-screw. Klinický soubor dosahuje lepších rentgenových výsledků ve skupině nitrodřeňových osteosyntéz, naopak lepší funkční výsledky mají zlomeniny léčené dlahovými osteosyntézami. Dlahové osteosyntézy však byly indikovány u složitějších typů zlomenin. V matematickém modelu simulujeme zlomeniny distálního radia typu C1 a C2. Hodnotíme stabilitu dlahových a nitrodřěňových osteosyntéz při zátěži v tlaku ve směru osy předloktí, při dukčních pohybech v zápěstí, při volární a dorzální flexi v zápěstí a při pronosupinačních pohybech... Distal radius fractures are the most common types of break to the arm bones. Conservative treatment in unstable fracture types cannot guarantee healing of the bones in a correct anatomical position. Locking plate osteosynthesis is at present regarded as the method of choice in surgical treatment of all types of distal radius fractures. An issue to be solved concerns the role for intramedullary implants in these indications. The aim of our study was to find the optimal indication for each of the osteosynthesis method currently used in distal radius fractures. We evaluated the one-year results of treatment for distal radius fractures. The group evaluated comprised 49 conservatively treated fractures, 78 fractures treated by angle-stable locking plates with either rigiddirection or multidirectional screws and 35 fractures in which Targon DR intramedullary nails or X-screws were used. Some radiographic parameters were better in the patients treated by intramedullary osteosynthesis, while some of the functional outcomes were better in the fractures treated by plate osteosynthesis although this method was indicated in patients with significantly more complicated fractures.
