The effect of increased neck muscle tension on tinnitus

This research bachelor thesis deals with the problem of cervicogenic somatosensory tinnitus with a focus on the effect of increased tension of the neck muscles. The theoretical part provides a brief anatomical description of the studied area in the context of the changes that occur there and lead to the development of tinnitus. It then elaborates on the diagnostic procedures and finally on the possibilities of intervention, which are within the competence of the physiotherapist. The practical part consists of case studies of two patients. The knowledge from the theoretical part was used to construct the examination and therapy that was then applied to these patients in order to verify the effect of manual therapy in tinnitus patients. A Visual Analogue Scale, the Tinnitus Handicap Inventory questionnaire and a goniometric examination assessing cervical spine range of motion were used for objectification. Patients' condition was assessed before and after the 10 week long therapy. The aim of the therapy was to relieve hypertonus in the neck region and subsequently reduce the intensity of tinnitus.
Correlation between inspiratory muscle strength and fitness in patients before lung transplantation

Introduction: Exercise capacity is frequently limited in lung transplantation candidates which can result in worse clinical outcomes of the transplantation itself. There can be several factors limiting exercise capacity, the purpose of this thesis is to determine the influence of inspiratory muscle strenght on reduced exercise capacity in these patients. The aim of this study is also to evaluate whether the Timed Up and Go Test is feasible for exercise capacity assessment in lung transplantation candidates as an alternative for the Six- Minute Walking Test. Methods: 14 patients with end-stage chronic lung disease considered for the lung transplantation waiting list inclusion performed Six-Minute Walking Test, Timed Up and Go Test and inspiratory muscle strenght evaluation using POWERbreathe® K5 (S-Index value). Correlations of the results of these tests were investigated. They were also evaluated separately for a group of patients with inspiratory muscle weakness (S-Index values lower than 80 % of predicted values). The bachelor thesis ,,CoughAssist in espiratory physiotherapy in patients with spinal cord lesions'' will be a research about CoughAssist, which is a specific mechanical insufflation- exsufflation device used in respiratory physiotherapy in patients with spinal cord lesions. It will discuss spinal patients with introduction of clinical manifestations and classification in spinal lesions. It will more specifically describe respiratory issues in spinal patients, the effect of lesion level on the patient's respiratory function, and respiratory complications associated with spinal lesions. Methods of assessing the patient's condition in terms of respiratory physiotherapy will be presented, as well as techniques used in respiratory physiotherapy in spinal patients. Furthermore, the thesis will discuss expectoration disorders, the cough pattern in spinal patients and the possibilities of treating these disorders within the framework of respiratory physiotherapy, especially with the CaughAssist mechanical insufflator-exsufflator. Sensorimotor training - part of comprehensive treatment of chronic obstructive pulmonary disease

The thesis deals with the possibility to influence the values of the lung function in patiens with chronic obstructive pulmonary disease (COPD) after exercise carried out according to the Methodology of sensorimotor training. The thesis contains a summary of theoretical knowledge about the pathophysiologic expresion of COPD, including their consequences not only for the respiratory system, but musculoskeletal system as well. Furthermore the thesis includes the information about the Methodology of sensorimotor training, that was used to influence the posture correction in our research. Methods: The spirometric examination of flow-volume curves monitor the changes prior to and after the exercise according to sensorimotor training in patiens with COPD (mean age 60, 733 ± 11,817). Results: The using of sensorimotor training can provide correction of the position of the whole body, of the chest, and change a chest mobility and in total to set up conditions for more economical muscle activation. This enable the changes in the values of the lung functions in the patiens with COPD, that we measured. Not all changes showed statistically significant change. Conclusion: Using sensorimotor training have an objective effect on the values of the lung function in patiens with COPD.
Dynamics of thorax in patients with COPD, its influence through POWERbreathe

This diploma thesis deals with chronic obstructive pulmonary disease (COPD) and its influence on the musculoskeletal system, lung transplantation and pulmonary rehabilitation. It discusses the role of POWERbreathe, a breathing training device, which is primarily used to strengthen the inspiratory muscles. The practical part was performed as a randomized control pilot study to assess the effect of POWERbreathe on the strength of the inspiratory muscles, the thorax dynamics and selected pulmonary function. The aim of the study is to determine the benefits of using the POWERbreathe for COPD patients during the pre-transplant period. In total, 18 patients with COPD IV. grade participated in the study (6 women and 12 men) with an average age of 59.8 ± 5.53 years. Probands were divided into two groups, only one of which used a POWERbreathe. Pulmonary functions (FEV1, VCin, FVC, PIF), muscle strength (SIndex), and chest dynamics (chest disturbances in the mezosternal and xiphosternal region with maximum inspiration and exhalation and respiratory amplitudes in these areas) were evaluated. The results did not show a significant inter-group difference in the strength of the inspiratory muscles. Possibilities of objective diagnosis of flat foot

The subject of this work is the flat foot - pes planus. Foot mechanical functions are described including features demonstrating flatfoot pathology. I summarize knowledge in embryology, anatomy and biomechanics, and focus on possibilities to evaluate functions of the foot with both classical and modern methods. The goal of this work is survey of diagnostic methods, mainly current procedures of objective evaluation and determination of the precize kind of the foottype pathology. I also include my original measurements on a patient and I compare the conclusions based on individual methods.
Physiotherapy procedures in patients after disc surgery in lumbar spine

This thesis focuses on problems of physiotherapy procedures after a surgery of intervertebral disc in the meaning of intensity, timing and therapeutic procedures. In the general part it gives comprehensive overview about functional anatomy, kinesiologi, biomechanics of the spine and about the function of intervertebral disk. It also deals with methods of investigation and surgical interventions. In the special part, the thesis focuses on the most appropriate choice of timing, intesity and mainly on special methods which are the most commonly used in patients after surgery of intervertebral disc. It gives the basic charakteristics of each special methods in relation to patients after surgery of intervertebral disc. The practical part of this thesis includes patient case report after surgery of intervertebral disc which describes the examination before neurosurgical intervention, surgery, hospitalization and outpatient rehabilitation.
Changes of a foot arch during pregnancy

There are many changes in a woman's body during pregnancy. This bachelor's thesis deals with the alterations of the arch of the foot and the mechanisms that can inflict it. Form of the thesis is review. Opening chapters sum the anathomy of the foot, the kinesiology, the pathologic kinesiology and important changes during the pregnancy. Main chapter deals with up to date knowledge about the alterations of the arch and related alterations of the pressures on the sole of the foot, as well as the alterations of size, volume and dimesions of the foot. Two case reports of pregnant women are part of the thesis.
