Celogenomové asociační studie, provedené v letech 2006-09, vedly k odhalení desítek genových lokusů, které predisponují jedince k obezitě. Nejsilnější signály byly zachyceny u polymorfismů v genu FTO (fat mass and obesity associated) a blízko genu MC4R (melanocortin-4 receptor). Studie předkládané v této práci referují o souvislostech polymorfismů vybraných genů s antropometrickými a metabolickými ukazateli. V souboru normoglykemických žen jsme nalezli asociaci polymorfismu blízko genu MC4R s hladinou růstového hormonu a leptinu, což vypovídá o jeho komplexní roli v hypothalamických regulacích. Byl zjišťován vliv variability genu FTO u štíhlých žen. Haplotypová kombinace čtyř rizikových variant u nich souvisela s mírným zvýšením BMI v rámci souboru. Vysvětlením jejich štíhlého fenotypu by mohl být nález vyšší hladiny růstového hormonu. Asociace vybraných obezitogenů jsme hledali i u adolescentů. Byla nalezena souvislost polymorfismů v genech MC4R a BDNF s metabolickým syndromem. Asociace polymorfismu v genu FTO s nadváhou a obezitou byla u adolescentů potvrzena, nově popsána byla souvislost varianty genu TMEM18 s podváhou. Varianta v genu PCSK1 korelovala s nižšími hladinami glukózy, a to zejména u chlapců. Druhá část této práce se zabývá účinky bariatrických operací,jejichž účinkem není jen snížení... Obesity is a frequent metabolic disease that causes many other health and socioeconomic complications. Obesity arises due to excessive energy intake and decrease in energy expenditure, which is a conseqence of contemporary lifestyle. Moreover, obesity has a strong genetic component. Common obesity is polygenic, multifactorial disease, in which individual genes interact with each other and with environmental factors. Genome-wide association studies, conducted between 2006-09, led to the discovery of dozens of gene loci that predispose individuals to obesity. The strongest signals were registered for polymorphisms in FTO (fat mass and obesity-associated) and near a gene MC4R (melanocortin 4 receptor). However, the contributions of these variations on the phenotype of obesity are very small, therefore, it is necessary to validate the results of such robust studies. It is very important to uncover the effects of genetic variants for understanding the molecular mechanisms of energy metabolism. The studies presented in this thesis refer about the impact of polymorphisms in selected genes on anthropometric and metabolic parameters of the patients of the Institute of Endocrinology and of healthy volunteers who underwent functional tests.
