[cze][eng] Souhrn 0BIdentifikace rizikových faktorů a optimalizace dispenzární péče u nemocných po resekci kolorektálního karcinomu a jejich rodin Úvod: Nemocní po resekci kolorektálního karcinom jsou ohroženi recidivou původního tumoru a metachronními neoplaziemi. Jejich příbuzní prvního stupně mají rovněž vyšší riziko kolorektálního karcinomu, na kterém se podílí hereditární a sdílené nehereditární faktory. Cíle: Hlavním cílem endoskopické části práce byl průkaz přínosu chromokolonoskopie s roztokem metylénové modři pro zlepšení záchytu neoplazií v rámci dispenzárního endoskopického sledování u nemocných s kolorektálním karcinomem. V epidemiologické části práce byly posuzovány rizikové faktory a současná dispenzarizace nemocných po resekci kolorektálního karcinomu a jejich příbuzných prvního stupně. Metodika a výsledky: V endoskopické části práce byla u 62 nemocných po resekci kolorektálního karcinomu po běžné kolonoskopii bílým světlem provedena chromokolonoskopie. K výkonu byl použit 0,1% roztok metylénové modři, metodu bylo možné provést u 94% (58/62) nemocných. Byla zhodnocena endoskopická a histopatologická charakteristika detekovaných lézí u každé metody. Chromokolonoskopie vedla k statisticky vysoce významnému zvýšení (p < 0,001) celkového počtu nalezených lézí, počtu adenomů s nízkým stupněm dysplázie,...The identification of the risk factors and optimization of the follow-up of colorectal cancer patients and their families Introduction: Patients after colorectal cancer resection are at risk of a recurrence of neoplasia at the site of anastomosis as well as metachronic neoplasias. First-degree relatives of colorectal cancer patients also have a higher risk of the disease that is caused both by hereditary and non-hereditary common factors. Objective: The main goal of the endoscopic part of the project was to evaluate the benefit of chromocolonoscopy with a solution of methylene blue for detection of neoplastic lesions in patients after colorectal cancer resection. The epidemiological part of project was focused on the evaluation of risk factors and current follow-up of patients with colorectal cancer and their families. Methods and results:In the endoscopic part of the work we performed regular white-light colonoscopy followed by chromocolonoscopy in 62 patients after colorectal cancer resection. We used 0.1% solution of methylene blue; total chromocolonoscopy was possible in 94% (58/62) of the patients.
