Comparative analysis of physiological, degenerative and postoperative lumbar spine using computational modeling

This diploma thesis deals with the comparative analysis of the physiological, degenerative and postoperative state of the lumbar spine. The problem was solved by computational modeling using the finite element method. For this solution, it was necessary to create appropriate computational models. The spine physiological state was compared with cases of the lumbar spine with different levels of spondylolisthesis, cases of the lumbar spine with different sacral angles, and cases of lumbar spine with applied fixators. Based on the performed analyses, it was found that spine degenerative state has a great influence on the distribution of stress in the vertebral discs and the vertebra itself. Compared to the physiological state, with increasing level of the spondylolisthesis the stress in the vertebral discs increases, while the stress in the vertebral section decreases. Furthermore, the spondylolisthesis affects the force flow. The higher the level of the spondylolisthesis, the more the force flow is shifted onto the posterior articular ligaments. The best results in terms of the spine physiological state were achieved with the applied fixator model.

Irena Novotná Title: A Case Study of Physiotherapy on a Patient with Vertebral Algic Syndrome of the Lower Spine Objectives: The aim of this bachelor's thesis is to provide a comprehensive summary of theoretical information related to lower back pain. The work includes a list of the most suitable diagnostic and therapeutic methods to this issue. Lastly, the thesis provides a case study of physiotherapeutic care for a patient with lower back pain. Methods: Throughout the duration of the case study, I applied physiotherapeutic methods and procedures that I had acquired during my three-year study of physiotherapy at the Faculty of Physical Education and Sport in Charles University. Results: The effectiveness of physiotherapeutic care was assessed by comparing the patient's initial and final kinesiological assessment, which are summarized in the chapter titled "Evaluation of Therapy Effect". The therapy had a favorable impact on the patient's overall condition. Conclusion: This bachelor's thesis is based upon continuous professional practice. Possibilities of physiotherapy in pregnant women with lumbar spine pain with regard to functional and structural parameters of the pelvis

This bachelor's thesis deals with the possibilities of using physiotherapy methods during pregnancy to relieve musculoskeletal pain, especially in the lumbar spine, while monitoring the functional and structural parameters of the pelvis. The work is divided into theoretical and practical parts. In the theoretical part, the aim is to give an overview and to write down information of a basic nature, first of all about the anatomical structures that are most closely related to this issue. The anatomy and kinesiology of the spine is described at the beginning, followed by the examination possibilities. The chapter on the pelvis is similarly structured, with more detailed functional and structural parameters of the pelvis. Furthermore, the theoretical section describes basic information on the course of pregnancy. A wide section then describes the changes on different systems during pregnancy, such as the respiratory, cardiovascular, gastrointestinal, urethropoietic and reproductive systems. As an integral part, there is also a description of changes in the spine and pelvis. Finally, appropriate or inappropriate physical activities and physiotherapy methods that can be performed during pregnancy are summarized. The aim of the practical part is to observe and evaluate the effect of physiotherapy intervention for low back pain during pregnancy. The research population included three probands in the second trimester of pregnancy who were suffering from low back pain. I implemented 6 therapy sessions with each proband once every 1-2 weeks. At the beginning, I did an initial kinesiological analysis with anamnesis taking. I presented each proband a series of exercises to influence the musculoskeletal system. At each session, we summarized the other changes since the previous session. At the end of the therapy sessions, we reviewed the results and performed an outcome kinesiological analysis. The work can be used by physiotherapists in practice as inspiration for therapeutic procedures and informative educational material.
The influence of rounded shoulder posture on the intensity of low back pain

Title: The influence of rounded shoulder posture on the intensity of low back pain. Objective: The objective of this paper is to determine whether there is a connection between low back pain and a forward shoulder posture. Methods: 23 probands divided into two groups according to pre-established criteria participated in this study. The control group consisted of 10 probands without a forward shoulder posture and without low back pain. The second experimental group consisted of 13 probands with a forward shoulder posture and with low back pain lasting at least three months. The experimental group took part in a four-week therapeutic programme aimed at correcting the forward position of the shoulder joints. This programme consisted of therapy used by Dr. Rašev - the application of a hot towel roll and elastic resistance exercises with Thera-Band. The forward shoulder posture was assessed visually and with a digital caliper. The numerical pain rating scale (NRS) was used to determine whether the low back pain changed when the forward shoulder posture was corrected. The title of the bachelor's thesis: Case study of physiotherapy treatment of the patient after operation of lumbar spine for L4/L5 disc prolapse Summary: This bachelor thesis is divided in two parts, for general and practical part. The aim of general part is getting theoretical basics for diagnosis of disc prolapse L4/L5. The general part is contains of basics of the spine anatomy and intervertebral disc and kinesiology of the lumbar spine. This thesis continue with description intervertebral disc herniation, etiology, diagnosis and teratment of intervertebral disc herniation. The aim of practical part is to elaborate case of study of physiotherapy treatment of the patient after operation of lumbar spine for L4/L5 disc prolapse.
