Enthesis and Enthesopathy

Background: Enthesopathies affect entheses, highly specialized connetive tissue, connecting tendon to bone. Enthesis and adjacent structures form a functional unit. It is prone to degenerative and inflammatory tissue damage caused by enthesis overuse or diffuse inflammatory disease. Methods: First task is to compose a current narrative review concerning enthesopathies with emphasis on disease etiology and efficacious therapies without focusing on a specific pathology. Thorough description of enthesis and its functions is included. Second task is to complete one detailed case study of a patient suffering from enthesopathy affecting a medial epicondyle of humerus. Results: A narrative review has been completed attempting to offer a more comprehensive perspective on entheses and related pathologies. One patient has been selected and treated for pathology of a medial epikondyle of humerus. Namely eccentric contraction exercises, radial pressure wave modality, and motion patterns modification were used to affect the lesion. The patient has improved rapidly and significantly. It is prone to degenerative and inflammatory tissue damage caused by enthesis overuse or diffuse inflammatory disease. Second task is to suplement the narative study with a case study of a patient suffering chronic elbow enthesopathy, in accordance with the department requirements. This paper takes a form of narrative study. Results: One patient has been treated for lateral epikondyl enthesopathy. Namely FSWT, RPW and eccentric contractions therapy were used. The patient has improved. A narrative review has been completed attempting to offer a more comprehensive perspective on entheses. Conclusions: Not enough up-to-date information about ethesopathies and treatment options is available in Czech publications. There are still some inefficient therapies being currently used, and there is lack of awareness of entheses complexities and various causes of enthesopathies. Case Study of physiotherapy treatment of a patient with the diagnosis of ankylosing spondylitis

The rehabilitative treatment insists foremost upon spinal flexibility maintenance which is herewith a part of the diseas progression prophylaxis. In case of disease acute phase, I carry elimination the complications associated with pains caused by fault motional stereotypes in particular. After pains alleviation, I focus on eliminating fault motional habits.
Throwing ability in human evolution

The aim of this thesis is to describe the origin of bone markers that characterize individuals who throw habitually and the usability of these markers in research of past populations. The thesis also summarizes knowledge of the origin and development of projectile weapons in Pleistocene. The effectiveness (accuracy and speed) of throw is probably related to differences in morphology of pectoral girdle, torso and hand. Therefore, a part of the text summarizes the morphology of pectoral girdle, torso and hand in the species H. erectus, which was probably the first group of hominins able to throw with effectiveness close to the one of anatomically modern humans. The development of projectile technology, which came after the adoption of effective throwing ability, is usually inferred from archaeological findings of spears and arrows. Various interpretations of archaeologically found weapons bring different conclusions regarding the usage of these weapons. The usage may be inferred from bone characteristics originating as consequences of moves that individual executes when manipulating weapons.
Enthesopathies and physical activity in the Great Moravian population (9th-10th century) with regard to social status and living conditions.

7 ABSTRACT The occurrence of enthesopathies is associated with muscle insertion sites and physical activities. The aim of this work is focused on two basic target areas. In the methodical section, I tested two methods of enthesopathy evaluation (Crubézy, 1988; Villotte, 2006) and I monitored the influence of age and physical stress on the incidence of enthesopathies in a documented skeletal collection from Greece. The main aim was then the evaluation of enthesopathies in the Great Moravian population from the settlement agglomeration Mikulčice-Valy (9th -10th century) using the selected method and the subsequent interpretation of the results attained, taking into consideration the archaeological facts relating to the given locality. Based on the observed degrees of concordance between the repeated evaluations, I used Villotte's method (2006) for final evaluation. I proved the correlation between the prevalence of enthesopathies and age in individuals from a recently documented skeletal collection. I was unable to demonstrate the influence of physical activity on the incidence of enthesopathies. This is apparently due to the small number of individuals with manual and non-manual occupations. Physiotherapeutic procedures of enthesopathies of lower limb

For my bachelor thesis I chose the topic concerning the problem of insertion pain on lower limb called "Physiotherapeutic procedures of enthesopathies of lower limb". I chose this theme, because I had also a period, when I had primary symptoms of insertion pain, and moreover, I did not came across any other bachelor thesis, which would devote to enthesopathies of lower limb so widely. The disease is caused by tendon overload, which happens for many reasons, such as sudden training, poor quality of footwear, pathological movement stereotype and previous injuries. The clinical picture depends on the location and duration of the overload. There is great effort to remain in conservative therapy, which is often successful. It involves soft techniques and mobilizations, compensatory exercises, physical therapy and other methods. However, sometimes it is necessary to accede to surgical solution. Prevention is very important, because it can help us avoid the problems. In the theoretical part I begin with a detailed description of anatomy of the bones and muscles of the lower limb. Then I write about examination methods, where I mention anamnesis, observation, palpation, examination of joint mobility and imaging methods. In the next part I deal with the problems of enthesopathies, their origin, risk factors and description of clinical picture according to individual localization. Last but not least, I introduce reader with possibilities of therapy, where I gave more space to conservative treatment. There are also mentioned preventive measures. In the practial part I worked with patients, who were suffering from insertion pain in the area of Achilles tendon. I performed an initial and final kinesiological analysis, which I compared at the end of research. After the initial examination I performed 10 therapies within 5 weeks consisting of soft techniques and mobilizations, lasertherapy, compensatory exercises and taping. I educated patients in autotherapy and preventive measures. The research group consisted of 3 patients in age between 25 and 45 years. Considering the results, it can be said, that the suggested therapy had positive effects and has brought relief to patients. I also devised exercises, that are good as prevention and treatment of selected enthesopathies of lower limb, which you can find in attachments. This work could be used as educational material for healthcare workers, athletes and the laic public.
Osteobiography of Early Medieval Warrior from Pohansko

The aim of this thesis was to construct an osteobiografical profile of two individuals from the early medieval locality of Kostice-Zadní hrúd, identified, on the basis of grave inventory, as warriors. In order to obtain a clear picture of early medieval warriors and to provide a basis for comparison, we also tried to find out what characterised an average early medieval Great Moravian warrior. Individuals from the early medieval locality Pohansko-Břeclav were chosen as comparative samples of regular population and warriors. The following methods were used to construct an osteobiographical profile: tracking of entheseal changes of muscle attachments, analyses of cross-sectional geometry of long bones and a Fourier analysis of acetabular morphology. We found out that warriors from Pohansko-Břeclav were horse riders trained in close combat, they did not wear heavy armour and did not train with bow. Due to bad preservation, only one of the individuals subject to our study, the individual labelled KZH H003, could be analysed. The man was short in stature, he did not perform hard manual work, but he trained with sword and bow and rode a horse. The examination is described, including medical history and special functional tests.
