Posttraumatic stress in women after premature birth in psychosocial contexts

The thesis discusses premature birth as a set of events that can lead to development of posttraumatic stress disorder (PTSD), or that can support the development of partial symptoms of PTSD. Since the topic of PTSD in connection with premature birth is not yet anchored in the Czech environment, the theoretical part of the work is based on previous foreign findings. The thesis focuses on the circumstances of the development of PTSD in women after premature birth, summarizes the findings on the occurrence and severity of PTSD symptoms in this group of women, discusses the negative effects of PTSD symptomatology, some space is devoted to the possibilities of diagnosing PTSD in the context of premature birth. The research part is based on a quantitative cross-sectional survey, which showed significantly higher scores of PTSD severity in the group of women after premature delivery compared to women after delivery at term. In the group of women after premature birth, PTSD severity scores were significantly associated to some anamnestic data (gestational week of the child's birth, child's birth weight, length of the child's postpartum hospitalization, operative delivery management). An important predictor of parenting abilities and well-being is parental self-efficacy. Recent studies show that mothers' parental self-efficacy can be influenced by childbirth satisfaction, which, in turn, is predicted by social-psychological aspects of childbirth experience, especially support by caregivers. In a longitudinal 2-phase research, we have followed parental self-efficacy determinants working around 1st childbirth as well as childbirth experience determinants. Using multiple linear regression analysis we determined that although parental self-efficacy is mainly prenatally determined, caregivers' support and baby's soothability can impact it positively. We have also confirmed the decisive influence of caregivers' support on childbirth satisfaction in our sample, which is representative for Czech primiparas. ...
Early parent-child interaction focused on gender specifics

This disertation i s a part of the grant task number 406/01/220 of the GAČR (Grant agency of the Czech Republic ). The solver of this task is doc. PhDr. Lenka Šulová CSc. The teoretical part of this work is devided in 6 main chapters. The first one is is focused on the analysis of early parent-child interaction from the point of attachment. The second and the third chapter i s focused on early interaction of mother-child and father-child. The fourth chapter is dedicated to the family from the point of the early interaction and we also try to define the main reason of the desruption of this early interaction. The fifth chapter is focused on the specifics of toddler' s age. The last chapter i s dedicated to the main methods and research in the field of attachment. The research of this work consists of an analysis of recorded play interaction of toddlers (age 12, 24, 36 months, +/- 2 moths) at home. The research sample consist of 30 children, 1 O from each age . The recorded play interaction were analysed according pre-defined categories (look, vocalisation, manipulation with a toy, physical contact, motorical activity, emotions) in computer programme V.I.P.
