Psoriasis and Comorbities, their Prevalence and Impact on the Treatment

Patients with severe psoriasis suffer frequently from associated comorbidities and therefore might have reduced life expectancy, which explains the necessity of corresponding screening and treatment recommendations in cooperation with general practitioners and other specialists. The aim of this study was to evaluate patients with moderate to severe psoriasis regarding associated comorbidities and compare the obtained information with nationwide data. In a subgroup of patients with concomitant cancer we tried to prove the safety of biologic treatment. Our study involved 3 groups of patients. First group included 1961 patients with psoriasis treated at our Department within last 5 years. Second group consisted of 268 patients with psoriasis, who voluntarily filled out our questionnaire during a 2-year observation period. The third group contained all 141 patients treated to date in our Centre of biologic treatment. Based on our real-world clinical data, we proved and confirmed the known and lately discussed cardiometabolic comorbidities associated with psoriasis. The questionnaire survey demonstrated overweight and/or obesity in 74% of all male respondents and in 60% of females, with average BMI 28,7. Phototherapy in dermatovenereology - methods and mechanisms

I choose the topic of phototherapy because of my longlasting interest in this field. Phototherapy is defined as the therapeutic use of light, primarily ultraviolet light. The mode of action on the skin is described as photobiological one and it has been continuously investigated. At its most basic level phototherapy represents the application of energy to skin, which means the delivery of photons to chromophores, the absorbtion of photons in chromophores and the biologic reaction of photon's absorbed energy. This causes the formation of heat and various biochemical effects in the skin. Previously the main role of UV light in phototherapy was thought to be the antimitotic and antiproliferative. Nowadays it is clear that beside them the mode of action is rather immunomodulatory and immunosuppressive. Further possible mechanism of the UV light are detected with increasing investigation of the immune system of skin. The rules and usages of phototherapy in practice at individual dermatological departments and offices differ and there is a need to improve and/or coordinate the protocols in order to reach the optimal risk/benfit ratio. Cutaneous Side Effects of Targeted Oncological Treatment

1 Abstract Introduction In recent years, new oncological treatments with targeted effects on cellular level have been introduced into the clinical practice. Some of these agents can cause specific changes affecting skin, skin appendages and mucosa. Currently, the most significant cutaneous side effects in the clinical practice are observed during treatment with inhibitors of the epidermal growth factor receptor (EGFR). Papulopustular exanthema is the most frequent adverse effect of EGFR inhibitors. Skin changes have a crucial impact on the quality of life can cause an interruption or even termination of the oncological treatment, thus worsening the patient prognosis. Objectives The objective of this study was to evaluate the incidence, severity and time of onset of papulopustular exanthema in patients with non-small cell lung cancer (NSCLC) treated with erlotinib. The study also aimed to assess the correlation between papulopustular exanthema and patient prognosis and the incidence of EGFR gene mutations. Sexually transmitted infections (STIs) are a major public health problem. Gonorrhoea is the second most common STI and if untreated can lead to serious complications. Early detection and treatment is commonly recommended as an important means of HIV prevention. The Czech Republic (CR) has had a long-established STI surveillance program since 1959. Although the Institute of Health Information and Statistics provides important data for STI surveillance, it contains no further characterisation of patients, particularly with regard to risky sexual behaviour. At the same time there are very few such data from the CR in the literature. This work presents view of STIs, especially of gonorrhoea, syphilis, chlamydia and HIV infection; examines their correlations and influences of their transmission in different groups of venereological patients, informs about incidence and epidemiological factors. The objective of this study was to evaluate clinical, epidemiological and behavioural characteristics of patients with diagnosed gonorrhoea with respect to risky sexual behaviour, and to compare these parameters by gender and sexual orientation. The prevalence of contact sensitization in children in the Czech Republic

Background The frequency of allergic contact dermatitis in children is not as negligible as had been originally thought. During the last decades, many reports have been published abroad concerning allergic contact dermatitis in children. However, the data about the Czech Republic were lacking. Aim of study The aim of the study was to determine the frequency of contact sensitization to the allergens of European baseline series in unselected child population. Material and Methods A group of 236 unselected schoolchildren aged between 6 and 16 years were patch tested with European baseline series in the years 2005 - 2006. Patch tests were evaluated in 218 (114 boys, 104 girls) according to the criteria of International Contact Dermatitis Research Group. Results One or more positive reactions had 67 children (30.7%). The top allergens in children were nickel sulfate, neomycin sulfate, potassium dichromate, cobalt chloride, Myroxylon pereirae resin (balsam of Peru) and fragrance mix I. Conclusion Results of patch tests in unselected set of Czech children are not very satisfactory. They indicate that the incidence of contact hypersensitivity in children is relatively frequent. It appears that the patch tests in the Czech Republic are not carried out on all children in whom they should be performed....

Analysis of work satisfaction

There are many factors around us that change our satisfaction, motivation and decisions. We all spend much time at work during our lifes and that is why I want to do a research of working satisfaction. I will realize this research by using a list of questions and oral interview in a company that employs about 60 people. I will focus on factors - wage, working environment, interpersonal relations - and their impact on the satisfaction rate. Main goal of my work is to improve care of eployees by proposing a recommendation to management of the company that may lead to lower fluctuation of employees, more effective work and higher prosperity of the company.
